Lymphomatoid papulosis: remission following intravenously administered acyclovir.

In a 54-year-old male patient suffering from Hodgkin's disease, lymphomatoid papulosis occurred. Complete clearing of the skin lesions was observed immediately after intravenously administered acyclovir. The patient had numerous relapses of his skin eruption with complete responses after each course of intravenously applied acyclovir. This striking therapeutic effect parallels reports of regression of mycosis fungoides and chronic generalized lymphadenopathy after acyclovir application. The mode of action of acyclovir in these disorders is not known. They all are characterized by involvement of the T cell system, and in all these diseases, a virus etiology has been proved or is suggested. Thus, a specific effect of acyclovir on T lymphocytes or selectively on helper T cells is discussed. Alternatively, the virustatic effect of acyclovir could be responsible for the therapeutic success.

[Results of a pilot study of hyaluronidase as an adjunct to cytostatic therapy in malignant diseases].

46 patients (17 myelomas, 11 malignant lymphomas, 8 mammary carcinomas, 7 head and neck carcinomas, 2 gastrointestinal carcinomas and 1 ovarian carcinoma) were treated with Permease prepared of bovine testes by Sanabo. 7500 i.u. were given either intramuscular one hour before cytostatic chemotherapy or intraperitoneally with cytostatic agents. There were 2 cases of local irritation on the site of injection and 1 case of reversible anaphylactoid reaction. Results achieved in patients treated with the same chemotherapy in spite of resistance, but with addition of Permease: myeloma CR 2/9, subjective improvement 7/9; 5 patients expired, median observation time: 13 months; non-Hodgkin-lymphomas CR 2/5, PR 2/5; 2 patients expired, median observation time: 9 months; breast cancer PR 2/4, 2 patients expired, median observation time: 5 months, 1 patient with Morbus Hodgkin CR, expired after 24 months. The other patients who received systemic treatment had either primary chemotherapy with addition of Permease, or chemotherapy was altered because of resistance against the prior therapy before Permease was applied. Intraperitoneal application of Permease together with cytostatic agents, usually not used for local therapy because of high rate of irritation like cis-platin, was well tolerated. Complete regression of ascites was achieved in all cases. In 1 of the 4 patients duration of remission was 7 months. Hypotheses concerning the mechanism of action of hyaluronidase in malignant diseases are discussed. The effectiveness of Permease might be related to resistance phenomenon of tumor cells or to alteration of pharmacokinetics of cytostatic agents.

[Clinical effects of high-dose immunoglobulin therapy in adults with idiopathic thrombocytopenic purpura].

15 patients suffering from idiopathic thrombocytopenic purpura were treated in our department with high-dosage immunoglobulins. The daily dosage amounted to between 0.13 and 0.4 g/kg body weight, administered for 4 to 15 days consecutively. The platelet count in 10 patients increased within the first week of treatment, but this increase was maintained for more than 4 weeks in only 3 patients. The average age of these 10 patients amounted to 41.2 years and was significantly lower than that of the remaining 5 patients (66.6 years), who failed to respond. Only one of 4 splenectomized patients responded with an increase in platelet count. Two different immunoglobulin preparations were used. No difference in efficacy was found and both preparations were well tolerated.

Non-convulsive status epilepticus after abrupt withdrawal of hypnotic-sedative drugs.

Four patients with severe addiction to sedative-hypnotics and with acute withdrawal symptoms of these drugs are described. They developed latent confusional states with characteristic EEG patterns (bilateral slow and sharp waves of high amplitude). Following small doses of benzodiazepines the EEG became normal together with a reduction in the clinical symptoms. It is suggested that the confusional states were of an epileptic nature.

[Leg ulcers in congenital dyserythropoietic anemia].

We report on an 18-year-old male patient suffering from leg ulcers due to congenital dyserythropoetic anemia and thrombocytosis. Based on this case, we discuss the importance of hematologic disorders for the development of leg ulcers as well as the pathogenetic mechanisms involved.

Illusory contours and cortical neuron responses.

Figures in which human observers perceive "illusory contours" were found to evoke responses in cells of area 18 in the visual cortex of alert monkeys. The cells responded as if the contours were formed by real lines or edges. Modifications that weakened the perception of contours also reduced the neuronal responses. In contrast, cells in area 17 were apparently unable to "see" these contours.

[Mitral ring calcification in dialysis patients. Echocardiographic diagnosis and etiological factors].

Echocardiography for assessment of presence of mitral ring calcification was performed in 96 dialysis patients. The control group consisted of 1758 consecutive patients without renal disease. For the first time it was shown that dialysis patients with mitral ring calcification (group I) had aortic valve sclerosis, renal osteopathy and peripheral angiopathy more frequently than dialysis patients without mitral ring calcification (group II). There were no significant differences between both groups for calcium, phosphate, calcium-phosphate product, magnesium, parathormone and lipids. There were equal numbers of hypertensive patients in both groups. In comparison to patients without renal disease dialysis patients had calcifications of the mitral ring more frequently (P less than or equal to 0.001) an at an earlier age.

[Acute delirium in bismuth poisoning].

Typical clinical signs and symptoms of bismuth intoxication are illustrated in a cases of a 45-year-old woman. Initially psychasthenia appears followed by acute delirium with ataxia, myoclonic jerks and occasionally coma. If patients survive the acute phase they recover only gradually following discontinuation of bismuth medication. In cases of extreme intoxication, permanent memory deficits may occur. Etiology, pathogenesis, laboratory findings, differential diagnosis and therapy of this rare iatrogenic encephalopathy are discussed.

[Progress in the internist treatment of bladder tumors].

First prognostic factors which are relevant for the variable course of bladder carcinoma like histological grading, blood group antigens and multiplicity of bladder tumors are discussed. Furthermore, therapy and recidive prophylaxis of not infiltrating carcinoma by intravesical cytostatic chemotherapy are discussed. In contrast to Thiotepa, a substance which has now been used for 20 years by many oncologists and should not be used any more because of very frequent systemic toxicity and only variable effects against the tumor, Adriamycin, Mitomycin C and Epodyl reduce the frequency of recidives used as prophylactic therapy after transurethral resection. With these substances pronounced systemic toxicity has not been seen until now. Cisdiaminodichloroplatin is a cytostatic agent which is also effective used systemically in cases with generalized bladder carcinoma. Used in combination with Adriamycin and Cyclophosphamide, remission rates could be increased to 80%. On the other hand toxicity is pronounced and the prolongation of survival times could not be proved until now as no results of controlled studies are available at this moment.

Vertical glaze paralysis and the rostral interstitial nucleus of the medial longitudinal fasciculus.

The rostral interstitial nucleus of the medial longitudinal fasciculus in known to participate in the generation of fast vertical eye movements in the monkey. A cell group homologous to this nucleus has been identified in the human brain. In man the nucleus lies dorsomedial to the anterior pole of the red nucleus, rostral to the interstitial nucleus of Cajal, and lateral to the nucleus of Darkschewitsch. Reconstructions of lesions in patients with different types of vertical gaze paralysis show that destruction of the rostral interstitial nucleus of the MLF bilaterally leads to an impairment of fast vertical eye movements. We propose that the nucleus participates in the production of vertical saccades and quick phases of nystagmus in man. Preservation of its integrity is not necessary for the production of vestibular compensatory eye movements in the vertical plane.

Adjuvant immunotherapy (BCG) in stage D prostate cancer.

Thirty-three patients with advanced prostate cancer were prospectively randomized to receive either Bacillus Calmette-Guerin (BCG) adjuvant immunotherapy plus conventional therapy or conventional therapy alone. Conventional therapy consisted of estrogens or observation. There was a statistically significantly (p less than 0.05) longer survival in the 16 BCG-treated patients (42 weeks) than in the 17 control patients (29 weeks).

Analysis and restitution of visual function in a case of cerebral amblyopia.

In a patient suffering from a severe bilateral dysfunction in both occipital and parietal areas we have found: (1) A loss of contrast-sensitivity, being strongest in the range of spatial frequencies where the normal eye is most sensitive; (2) Virtually normal grating resolution, a severe loss in letter acuity (about 15-fold), and a very strong loss in vernier acuity (about 100-fold); (3) Rapid partial recovery of contrast sensitivity due to stimulation; (4) Slow partial recovery of letter acuity due to CAM rotating grating treatment; (5) No improvement of a 10-fold temporal retardation in the speed of reading. Evidence is presented that the residual impairment of vision depends upon improper synthesis of spatial frequency components originating from a heavy loss in processing speed and abnormally long store duration for high spatial frequency information.

Development of stereopsis and cortical binocularity in human infants: electrophysiological evidence.

Dynamic random-dot stereograms and correlograms were used to elicit visually evoked brain potentials from human infants, and these potentials were compared with potentials evoked by classical checkerboard pattern reversal. The results indicate that infants begin to produce stereoscopically evoked potentials at the age of 10 to 19 weeks, several weeks after showing classical checkerboard-evoked potentials, and suggest that the onset of cortical binocularity precedes stereopsis.
