Morbidity of thyroid surgery.

BACKGROUND: Morbidity is today's concern in thyroid surgery. The purpose of this paper was to quantify risk factors' contribution to morbidity rates. METHODS: During 50 months, 1,163 patients undergoing 1,192 thyroidectomies at one hospital were reviewed at follow-up of 8 to 58 months. RESULTS: There was 1 death (0.08%). Wound morbidity included 19 hematomas (1.6%), 3 chyle leaks (0.2%), and 6 abscesses (0.5%). Mean hospital stay was 4.3 days after surgery without drain and 5.3 days with drain (P < 0.01). Temporary and permanent hypoparathyroidism (TH; PH) rates were 20% and 4%. Parathyroid autografting and excision rates were 19% and 9%. TH rates were higher after parathyroid autografting or accidental excision (P < 0.01). There was no correlation between the severity of TH and the number of lymph nodes at neck dissection nor between postoperative serum calcium levels and the number of parathyroids identified at bilateral surgery. Temporary and permanent recurrent laryngeal nerve (RLN) palsy (TRLNP; PRLNP) rates were 2.9% and 0.5% (0.3% of 2,010 RLNs at risk). PH and TRLNP (not PRLNP) rates were higher after completion or total thyroidectomy with node dissection (P < 0.01). TRLNP and PRLNP rates after RLN exposure and after nonexposure were not statistically different. Surgical volume had no bearing on hematoma, abscess, TH, PH, TRLNP, and PRLNP rates. CONCLUSIONS: High surgical volume, identifying parathyroids and RLNs, failed to reduce morbidity. Completion and total thyroidectomy with node dissection increased PH and TRLNP (not PRLNP) rates.

[Isolated thyroid nodule. Comparative value of cyto-puncture and scintigraphy].

OBJECTIVES: Nodular thyroid disease, indicated by the presence of single or multiple nodules within the thyroid gland is a common clinical problem, the main question remains the malignancy. Radionuclide scanning, ultrasonography and fine needle aspiration biopsy have been helpful to distinguish benign from malignant nodules and to select patients for surgery. METHODS: We performed a prospective study to assess the comparative value of fine needle nonaspiration biopsy and thyroid scinti scan in the management of 412 patients operated for solitary thyroid nodule. RESULTS: Sensitivity and negative predictive values were the same for both methods, but specificity of cytology was greater than that of thyroid scinti scan (80.53% vs. 10.47%, p < 0.001). DISCUSSION: Thyroid radionuclide scanning remains valuable in the evaluation of a cytological benign solitary thyroid nodule when TSH value is low, in order to distinguish toxic adenoma from cold nodule in Graves' disease.

[Mega-esophagus: treatment by celioscopic approach].

Heller's procedure is widely used as the most appropriate method for treating achalasia. We performed the procedure laparoscopically in three patients with good results. Two females and one male, age 40, 60 and 68 years, presented with long-term recurrent dysphagia due to achalasia which did not respond to endoscopic dilatation. Heller's procedure was performed laparoscopically in all three with no particular difficulty. In our cases we also attached the anterior border of the greater curvature to the left side of the myotomy and fixed it to the right diaphragmatic pillar to reduce reflux. The procedure would appear to have less risks than laparotomy since visualization of the operative fields is better. Laparoscopy will undoubtedly become the preferred procedure for achalasia.

Absorption of glycine irrigating solution during endoscopic transanal resection of rectal tumors.

PURPOSE: This study was undertaken to evaluate the potential metabolic complications of 1.5 percent glycine irrigating solution during endoscopic transanal resection (ETAR) of rectal tumors. METHODS: Thirteen consecutive frail patients (mean age, 81 (range, 57-91) years) undergoing 18 ETAR were prospectively studied from July 1993 to January 1995. Indications for ETAR included palliation of advanced rectal cancer (12 patients) and an extensive villous tumor (1 patient). A 27-French two-way Iglesias resectoscope was used. Packed cell volume, blood glucose, and serum concentrations of sodium, potassium, and creatinine were measured before, during (at 45 minutes), and after (at 6 and 24 hours) ETAR. Plasma concentration of glycine and hemoglobin levels were both measured preoperatively and at 45 minutes and 24 hours, respectively. Variables studied included resection time, volume and rate of irrigating fluid, height of irrigating fluid bag above operating table, resectate weight, occurrence of intraperitoneal and extraperitoneal perforation, blood loss, and clinical symptoms. RESULTS: Two patients were excluded. Mean operating time was 456 minutes. A mean of 192.3 liters of irrigant was infused into the rectum. Mean irrigation rate was 43,330 ml/minutes. Mean height of irrigating fluid bag was 692 cm. Extraperitoneal perforation occurred in two patients. Blood loss exceeded 200 ml in four patients, one of whom complained of nausea (operating time, 110 minutes). Mean rise in p-glycine at 45 minutes (10,028 mol/l; 387 percent of preoperative values) was significant (P = 0.006). Changes in packed cell volume, b-hemoglobin, b-glucose, s-sodium, and s-creatinine levels were not significant. There was significant correlation between p-glycine and s-creatinine levels at six hours (P = 0.033), between p-glycine levels and fall in s-sodium at 24 hours (P = 0.037), and between levels of b-hemoglobin and packed cell volume at 24 hours (P = 0.0004). There was a positive linear correlation between p-glycine and operating time (r = 0.7; P = 0.0026) and between p-glycine and volume of irrigating fluid (r = 0.5; P = 0.0386). CONCLUSIONS: Operating time best predicts increase of p-glycine in ETAR.

[Cancer of the gallbladder. A surgical experience. A propos of 143 cases].

The records of 143 patients with gallbladder carcinoma operatively treated between 1975 and 1995 were retrospectively reviewed. Abdominal pain was the most common symptom and was present in 103 patients (72%). Jaundice was present in 83 patients (58%) and weight loss in 68 (47.5%). Pre-operative diagnosis was made in 40.5% of the patients. Surgical procedures included cholecystectomy alone (16.8%), cholecystectomy with resection of the hepatic bed and lymphadenectomy (11%), cholecystectomy with biliary drainage (57.3%), and exploration with biopsy (14%). Surgery was considered as curative in 21.5% of the patients. Overall five-year survival rate was 11%. Actuarial 5-year survival rate was 100%, 29% and 23% in patients with T1, T2 and T3 tumors. The five-year survival rate was nil in patients with T4 and T5 cancers.

[Volvulus of the right colon. Plea for right hemicolectomy. Apropos of a series of 23 cases].

Twenty-three patients with volvulus of the caecum were operated on from 1982 to 1995. There were 17 men and 6 women, mean age 63 years. Preoperative diagnosis was obtained in 48% of the cases. A right hemicolectomy was performed in 22 cases with immediate anastomosis; in one case, a double ilial and transverse stomy was required. Necrosis was observe in 12 patients (52%). Post-operative mortality was 8.6% (n = 2) due to circulatory shock in 2 patients with necrosis of the colon. Post-operative morbidity was 21% (n = 5) including one case with fistulization of the anastomosis requiring a double stomy. On the basis of this series and a review of the literature, it appears that right hemicolectomy is the treatment of choice for volvulus of the right colon, as it avoids all risk of recurrence and mortality is lower than conservative treatment.

[Palliative treatment of rectal cancer by the trans-anal approach using a urologic prostatectomy device].

Palliative treatment for patients with rectal carcinoma affect more than 10% of the 8000 cases diagnosed each year in France. Twenty-four patients underwent endoscopic trans-anal resection using a urological resectoscope. The morbidity rate was 18,5% and there were no postoperative deaths. The median hospital stay for the procedure alone was 4 days. Good palliation was obtained in 89% of cases. This simple, minimally invasive and economic method should join palliative treatment for patients with rectal carcinoma beside laser destruction and external beam radiotherapy.

[Value of fine-needle aspiration and extemporaneous examination in thyroid surgery. A prospective study of 1235 fine-needle aspirations and extemporaneous examinations in 846 patients].

We report a prospective study conducted from 1993 through 1995 in 846 patients (mainly women) consulting for a thyroid nodule. There were 465 multinodular goiters and 373 single nodules. The clinical diagnosis was obvious in 8 patients who were excluded. Pre-operative fine-needle aspiration was performed in 1235 patients and per-operative extemporaneous examinations in 569. These two simple reliable examinations, with few significant complications in skillful hands, are complementary. Fine-needle aspiration gives reliable histology data for thyroid nodules and should always be included in the pre-operative workup. Extemporaneous pathology examination of the surgical specimen provides information for adapted surgery and avoids morbidity due to reoperation.

[Pyelo-ureteral dilatation revealing appendiceal abscess. Apropos of 2 cases].

The pyelo-ureteral dilatation revealing a appendiceal abscess is a uncommon situation. Its physio-pathological mechanism is not clear. The authors present two cases of which the diagnosis was not immediately made. The appendiceal abscess treatment is usually enough to make disappear the ureteral dilatation and the after effects stenosis are exceptional.

[Hypersecreting villous rectosigmoid tumors. Apropos of 14 cases].

Villous recto-sigmoidal tumors with severe fluid and electrolyte loss are rare. We report here fourteen new cases with severe electrolyte imbalance (hypokalemia, hyponatremia, renal failure). We analysed too electrolyte loss in stools and we researched electrocardiographic disorders. Finally, we reported a long-term evolution of these patients.

[Correction of substance losses in the abdominal wall by an aponeurotic graft].

A series of 123 patients presenting with abdominal eventrations were treated by an aponeurotic graft. Operative mortality was 0.8%. Recurrent eventration was observed in eventrations with a diameter superior to 4 cm (38% at 55 months). The authors recommended this technique only in case of small eventrations or for the parietal reparation following closure colostomies.
