A new method to find a set of energetically optimal RNA secondary structures.

We present a computer method to determine nucleic acid secondary structures. It is based on three steps: 1) the search for all possible helical regions relied on a mathematical approach derived from the convolution theorem; it uses a tetradimensional complex vector representation of the bases along the sequence; 2) a 'tree' search for a set of minimum free energy structures, by the aid of an approximate energy evaluation to reduce the computer time requirements; 3) the exact calculation and refinement of the energies. A method to introduce the experimental data and reach an arrangement between them and the free energy minimization criterion is shown. In order to demonstrate the confidence of the program a test on four RNA sequences is performed. The method has computer time requirement proportional to N2, where N is the length of the sequence and retrieves a set of optimal free energy structures.

[Hypertension and syncytiotrophoblast: morpho-structural aspects].

Hypertension is a pathological condition that involves maternal fetal relationship. In hypertension placenta displays a syncytiotrophoblast plasmalemma with aspects of anomalous behaviour concerning Intramembranous Particles (IMP) and actin content of microvilli cytoskeleton. Decrease of syncytiotrophoblast microvilli IMP and microvilli actin further sustain the tendency of hypertensive placenta to show some features of immaturity that might deeply influence fetal-maternal exchanges during pregnancy associated with pathological status.

Molecular alterations induced by hypertension in pregnancy on syncythiotrophoblast plasma membranes from human placenta.

To evaluate the possible functional relationships between hypertensive status and syncythiotrophoblast plasma membrane behaviour we have carried out a freeze-fracturing and biochemical investigation to assess: 1) ultrastructurally, relations between number and diameter of Intramembrane Particles (IMP) and hypertensive conditions; 2) biochemically, actin content of microvilli in this pathological status. Our data in vitro show a decrease of IMP in hypertension before and after Ca++ addition and a decrease of actin in microvilli of hypertensive placenta. These observations seem to be in agreement with the hypothesis of a possible structural immaturity in hypertensive placenta and may represent morphological signs of placental insufficiency.

Squamous cell papilloma of the esophagus. Report on 35 endoscopic cases.

We herein report our experience of 35 cases of esophageal squamous cell papilloma (ESP), diagnosed among 8,095 consecutive upper gastrointestinal endoscopies, and histologically confirmed. The incidence (0.45%) was higher than previously reported. Macroscopically, ESP presented with several distinctive features, thus endoscopy permitted great diagnostic accuracy. Most of the papillomas were located in the middle-third of the esophagus, and no significant association with hiatal hernia and reflux esophagitis was seen. The lesion did not recur after removal either by forceps or by diathermic snare.

[Personal development in schizophrenia therapy].

Gaetano Benedetti introduced the psychotherapy of schizophrenics in Burghölzli in the early fifties. In this paper he reviews the development of his work and appreciates the significance of Bleuler's concept of schizophrenia. He then traces an outline of his communicative psychopathology: Of primary importance for the psychotherapist is the awareness that there are also anti-psychotic intentions in the patient which are trying to be expressed through the psychotic figurations. The psychotherapist accepts the psychotic experiences of his patients in order to transform them through his identification with them, resulting in the positivization of psychopathology. It is, as if here, in this field of psychotherapeutic duality, that the otherwise lost dimension of humanity in psychosis can find expression.

Ineffectiveness of 5-fluorouracil and cis-platin as second-line chemotherapy in head and neck cancer.

Seventeen patients with heavily pretreated head and neck squamous cell carcinoma were submitted to a combination of 5-fluorouracil, 500 mg/m2 on days 1-4, and cis-platin, 50 mg/m2 on day 5, repeated every 21 days. Before administration of 5-fluorouracil, N5,N10-methyltetrahydrofolate, 200 mg/m2 i.v., was given. Only 1 partial response and 4 stable disease were observed, and the median survival of the entire group was 5 months. Although all patients had been heavily pretreated and a considerable percentage (6/17, 35.2%) of these showed resistance to first-line therapy, this combination seems to be ineffective as second-line therapy in head and neck cancer.

[Psychodynamic aspects of schizophrenic negativism in individual psychotherapy].

This is a report of the individual psychotherapy of a schizophrenic woman who has been cured after years of treatment. The report deals mainly with psychodynamic problems such as resistance and negativism. It shows that only a common view of such phenomena, which includes the therapist as well as the patient and establishes a symmetry of experience, can lead to those processes of identification and counter-identification which ultimatively are helpful to both the patient and his therapist.

The structure of psychosomatic symptoms.

1. According to our findings, a psychosomatic structure is not an alternative to neurosis. The syndrome of alexithymia appears to be, in our cases, a transitional phase, the dynamics of which can be understood and treated by psychotherapy. In the course of psychotherapeutic treatment, alexithymia always disappears. 2. However, a special psychotherapeutic technique is advisable, one different from that applied in classical neurosis. 3. For this reason the term psychosomatic structure can be used as a way of expressing a variation of neurosis. 4. Modern concepts of psychosomatic disease describe pathological personalities that are different from the classic pseudoneurotic type because of their narcissistic and pregenital structures. These structures can be concealed underneath a facade of genital and psychoneurotic defense. 5. The old relationship between psychosomatosis and psychosis can be better understood on the basis of our findings, which reveal a borderline structure and splitting mechanisms in many psychosomatic patients. 6. Psychosomatic pathology can be better understood today by relating it to the structure of the psychosomatic family. 7. It appears that if, on the one hand, psychosomatic diseases of a more hysterical picture are frequent among preindustrial cultures and in low-income classes, on the other hand psychosomatic syndromes of a narcissistic and borderline type are characteristic for our civilization.

[Psychotherapy of schizophrenia].

Schizophrenic existence is the experience of an awful negativism, which comes from the depth of the Unconscious and appears at the surface of the conscience as the destruction of all positive symbols of life. Interpretations of psychodynamic mechanisms, of transference and countertransference, of dreams and fantasies must be rooted in the dominant therapeutic endeavour of creating with the remaining potentialities of the patient new positive symbols of the schizophrenic existence. The author's experience in the 30 years of his dealing with schizophrenic patients is here summarized.
