[Observation of an increase in "aggressive tumor forms" in increasing incidence of breast carcinoma].

Between 1986 and 1990 23% more patients with carcinomas of the breast were treated in the Gynaecological Hospital of the Municipal Hospital (Städt. Krankenanstalten) of Krefeld than during the comparative period between 1976 and 1980. Simultaneously, an increase of carcinomas metastasising the further course of the disease were observed. When analysing the possible reasons it can be noticed that in the first group (1986 to 1990) compared to the second group (1976 to 1980) multifocal and lobular carcinomas were observed more frequently with regard to their statistical significance. The same applies to the metastatic involvement of axillary and infraclavicular lymphodes. It was noticed that in the first group, lymphangiosis and hemangiosis carcinomatosa were seen more frequently than in the second group. The collected data suggest that in case of an increase in carcinomas of the breast, a larger number of tumours with prognostically unfavourable tumour criteria is seen.

[Transposition of the external genitalia].

The syndrome of complete peniscrotal transposition is rare. Combination with malformations of other organs is frequent. Sometimes, life is not possible with these malformations. The demonstrated case gives an overview of: Definition, Epidemiology, Etiology and the Diagnostic and therapeutic procedure.

[Malignant eccrine poroma].

We report on a 82-year-old woman suffering from malignant eccrine poroma on her back. The infiltrated surrounding tissue suggested lymphangiosis carcinomatosa; one axillary lymph node was enlarged. Both features, however, turned out to be only of inflammatory nature.

[Increasing anaplasia of an extramedullary plasmacytoma with loss of detectable paraproteinemia (author's transl)].

An epipharyngeal solitary extramedullary plasmacytoma with subsequent dedifferentiation is described. Immunohistochemically the plasmacytoma cells of the first biopsy were positive for lambda and gamma, immunoelectrophoretically only lambda was demonstrable. Three months later an anaplastic tumor reappeared whose cells were also immunohistochemically positive for lambda and gamma. However, paraproteins were no longer demonstrable in serum and urine. The observed changes were interpreted as a result of anaplasia. Furthermore, our case shows that the diagnosis of multiple myeloma is not always possible by means of immunoelectrophoresis. The most important implication of our case is the fact that a follow-up of serum or urine concentrations of paraproteins may not reflect the course of a given plasmacytoma.

Production of a nasoantral window with an Ar+-laser.

In vitro experiments on perforating and cutting 14 human nasoantral walls using an argon laser are described. The drilling velocity is proportional to the power density up to 100 W/mm2. The cutting velocity is measured using focussed spots of 40 micrometer diameter and powers from 5 to 40 W. Histological studies were performed at the edges of the lesion. The experiments show that the technical problem for producing a window in the nasoantral wall using a laser can be solved.
