[Quantitative analysis of bone density in multiple myeloma].

Bone lesions are the main sign of neoplastic proliferation of multiple myeloma (MM), a disseminated malignant disease which originates in, invades and replaces normal bone marrow. The most characteristic radiographic pattern is a focal lytic lesion, well-defined or "punched-out", generally with no surrounding bone reaction. The association is confirmed between MM and osteoporosis, as reduced bone density (osteopenia) and pathologic fractures (ribs, spine). This paper is aimed at evaluating the importance of osteopenia in both diagnosis and prognosis of MM. Eighteen patients affected with MM were examined with quantitative computed tomography (QCT) and dual-energy X-ray absorptiometry (DEXA) for bone densitometry in lumbar spine and proximal femur. The patients (12 males and 6 females) were classified according to Durie's clinical criteria and to the radiographic patterns suggested by Merlini. The results indicate the patients with an advanced clinical stage (III) and scintigraphic expansion of bone marrow to have low densitometric values on both QCT and DEXA. There was substantial agreement between the 2 methods, but DEXA had a higher number of false positives. Instrumental diagnostic protocol may be thus planned as follows: 1) conventional radiography; 2) bone marrow scintigraphy; 3) bone densitometry of lumbar spine, with QCT. The patient is then to be followed with conventional and/or digital radiography in symptomatic locations, and with bone scintigraphy.

Modification of number and of affinity of endometrial EGF receptors during the menstrual cycle.

We have analysed the distribution and the binding properties of the epidermal growth factor (EGF) receptor in human endometrium during the menstrual cycle to validate the proposal that steroids affect the growth rate of target cells via growth factor-related pathways. The total number of EGF receptors and their affinity for the iodinate exogenous ligand are significantly higher during the proliferative than the secretory phase; the binding kinetics appear to be hyperbolic and sigmoid during the proliferative and the secretory phase, respectively. These results suggest that endogenous sex steroids can regulate the levels and the properties of EGF receptors in vivo, and support the possibility that receptor-coupled events may play a role in estrogen-stimulated growth.

Studies on amniotic prolactin: chromatographic pattern and correlation with the lecithin content.

We have measured the concentrations of prolactin and lecithin in the amniotic fluid from 20 normal pregnant women in the 2nd and 3rd trimester. Prolactin is present totally as a low-molecular-weight form, 'little' prolactin, and appears to correlate negatively with lecithin during the 2nd and positively during the 3rd trimester. On the basis of these results and of the information that prolactin is present as high-molecular-weight isohormones in maternal blood, it is argued that amniotic prolactin is synthesized 'in situ' and that different mechanisms are involved in the regulation of prolactin production.

Regulation of endometrial transglutaminase activity during the menstrual cycle.

When human endometrial transglutaminase was measured a 10-fold higher activity was detected during the secretive phase. This change was not related to either differences in solubility of the enzyme or to selective contamination by plasma factor XIII and rather appears to depend on the expression of the tissue form of transglutaminase, suggesting that this enzyme is regulated in vivo by progesterone.

[An epidemiological evaluation of proximal femur fracture on a population living in the area of the Florence Local Health Trust].

Proximal femur fracture is a relevant issue of public health due to its high prevalence in elderly people and high social costs. We performed an epidemiological description of the event based on the data derived from the hospital discharge reports over a 5-year period, from 1997 to 2001, referred to a mean population of 796,990 persons. During this period, the disease incidence rate, estimated on hospital admissions, is high (higher than national average), although rather constant over years: average values are 777.92/100,000 in the >65 age group, 2627.08/100,000 in the >85 age group, and 1264.25/100,000 in the 80-84 age class. Incidence rate shows an exponential increase with age starting from the age of 50 and in the elderly age classes women exhibit around twofold values. Besides, the mean length of hospitalization is higher in the elderly age classes. Regarding 4 selected kinds of surgical intervention we made and verified the hypothesis of a correlation between length of stay in hospital before the intervention and two outcome indicators: length of stay in hospital after the intervention (data fail to confirm our hypothesis) and in-hospital mortality (data confirm our hypothesis: mean length of stay in hospital before the intervention is significantly higher in the group of dead patients than in the group of home discharged patients).

[Mobile phone use while driving in Florence health district area].

The practice of driving while using hand-held mobile phones is diffused in numerous countries, even though it has been demonstrated that they increase the risk of traffic accidents, and such use has consequently been prohibited. Because of the lack of Italian data on the spread of this behaviour a study was done in June 2004 on drivers' use of hand-held mobile phones in the Florence Health Authority area. The study was performed by direct observation from 4 watching-points (1 for each zone) which allowed for observation of several types of vehicles, on varying road conditions, in 5 hour time bands. Overall, 9387 vehicles were observed and the prevalence of mobile phone use while driving was found to be 1.8%. The greatest prevalence of mobile phone use was found in the following conditions: in the Northwest zone (2.8%), in the city suburbs (2.8%), on Tuesdays (3.0%), among drivers not using safety belts (3.0%), and among subjects driving alone (2.1%). Logistic regression analysis has shown a statistically significant association between mobile phone use and the zone, the number of persons in the vehicle, and seat belt use. Mobile phone use while driving is therefore a diffused practice that is particularly common among drivers who engage in other high-risk driving behaviours.

Leptomeningeal carcinomatosis presenting as progressive multineuritis: clinical, pathologic, and MRI study.

Two patients developed a persistent illness characterized clinically and electrophysiologically by asymmetric involvement of spinal roots, of cranial and peripheral nerves. In the first case the disease was not discovered clinically but only after autopsy. The primary neoplasm remained undetected at autopsy. There was profound infiltration of the leptomeninges by tumor cells with features of metastatic adenocarcinoma. In the second patient onset of neurological symptoms occurred 16 years after surgery for breast cancer, which may be reasonably considered the primary malignancy-CSF cytology was positive only in the second patient in whom Gd-DTPA MRI supported the diagnosis. Our cases demonstrate that diagnosis in leptomeningeal carcinomatosis may be a challenging clinical problem.
