[Surgical modalities in germinal tumors of the testicle].

Radical orchidectomy via a kelotomy incision in the first diagnostic and therapeutic procedure in germ cell tumours of the testis. The traditional extensive bilateral lymph node dissection responsible for ejaculation failure and significant complications has now been replaced by limited lymphadenectomies using nerve-sparing and nerve-preserving techniques with a simpler post-operative course. The resection of residual masses after chemotherapy should be performed as a true lymphadenectomy and is associated with certain difficulties, particularly in advanced seminomas.

Dysgerminoma↗

[Therapeutic indications for germinal testicular tumors].

The therapeutic indications for germ cell tumours of the testis depend on the histology (pure seminoma: 45%, non-seminomatous germ cell tumour: 55%), the extension and the severity of the prognosis. The well standardised approach to pure seminomas is less clear for non-seminomatous germ cell tumours. Stage I, IIAB pure seminomas (95 to 98% of cases) should be irradiated. The dose and target volume are adapted to prophylactic (I) and curative (II) objectives. Rare seminomas with a large tumour bulk should be treated with chemotherapy. Survival is close to 100%. Stage I non-seminomatous germ cell tumour offers several theoretical possibilities. Radiotherapy is not very popular and chemotherapy appears to be to aggressive, lumboaortic lymph node dissection is being replaced by new imaging modalities and simple follow-up requires a rigorous and disciplined approach. At the Val-de-Grâce hospital (France) since 1987, we perform simple orchidectomy in favourable stage I disease: 80% are cured with no other treatment, 20% relapse and are cured by chemotherapy, in the unfavourable stage I cancers (histology, markers) or with uncertain follow-up, limited chemotherapy is performed (3 cycles of EP). Stage II and more advanced non-seminomatous germ cell tumours are divided into moderate forms (IIA, B, III, IVL1) and major forms (IIC, IVL2, L3, H+, CNS+). In the exclusive infradiaphragmatic involvement of moderate forms, some authors propose bilateral lumboaortic lymph node dissection which is invasive surgery with an efficacy declining from 90% (IIA) to 50% (IIB). The majority of teams, particularly Val-de-Grâce, administer 3 or 4 courses of BEP followed by assessment (CT scan - markers) and salvage surgery.(ABSTRACT TRUNCATED AT 250 WORDS)

Antineoplastic Combined Chemotherapy Protocols↗

[Mature teratomas of testicular origin. Discussion of 12 cases].

Between 1979 and 1989, 135 patients with a non-seminomatous germ cell tumour of the tests were treated in the Val de Grâce Military Hospital in Paris. In twelve patients, histological examination of a residual mass after chemotherapy revealed the presence of mature teratoma. Although this tumour, considered to be benign, generally represents a favourable form of the initial tumour, it nevertheless has a potential for progression. The possibility of long-term recurrences, which are sometimes difficult to eradicate, justifies very careful long-term surveillance of these patients. The authors report three particularly demonstrative cases and present a review of the literature.

Adult↗

[Testicular prosthesis. Study of complications, apropos of 63 cases].

The authors report 63 cases of insertion of testicular prostheses (Lattimer model in Silastic, manufactured by Dow Corning) in patients who had undergone castration for testicular cancer. A study of the early and late post-operative complications, which were relatively rare and always benign, allows several simple rules to be proposed regarding this surgical procedure. There does not seem to be any problem with long term tolerance to medical silicones, after a ten year follow up period for the earliest cases. The insertion of an implant does not in any way delay continuation of treatment for testicular cancer.

Adolescent↗

[Hydronephrosis and ureteropelvic junction syndrome. Apropos of 93 cases].

So called "congenital" hydronephrosis, a relatively frequent condition, is increasingly benefiting from modern investigative techniques such as ultrasound. This should enable a reduction in the time interval between clinical symptomatology and treatment. In a series of 93 cases, the authors demonstrate that pathological anatomy is of little value as regards etiology, while plastic surgery of the pyelo-ureteral junction still remains the most simple and reliable treatment with a low associated morbidity.

Adolescent↗

[Villous tumor of the appendix].

An infrequently diagnosed lesion, villous tumor is extremely rare in its appendicular localization. Its diagnosis is then fortuitous and its secondary treatment is still debatable if surveillance is univocal. A further case of appendicular villous tumor is reported.

Adenoma↗

[Encapsulating peritonitis. Apropos of a new case].

The physiopathology of the rare affection encapsulating peritonitis is poorly elucidated, but its etiologies are now more clearly defined. Diagnosis is usually at operation, as in the present case, a 65 year old man with known alcoholism and wearing a LeVeen type peritoneo-jugular valve, probably the cause of the peritonitis.
