[Cerebral infarct following trauma].

Vascular occlusions due to cerebral trauma have always been regarded as great rarities. However, we have found hypo-dense foci of vascular distribution in 3.5% of 3500 CT examinations for trauma during the late phase. Lesions in the vascular territory of the posterior cerebral artery are usually the result of supratentorial pressure rise from epidural and subdural haematomas, leading to compression of the vessels against the edge of the tentorium. Typical infarcts in the territory of the medial and anterior cerebral arteries were found only rarely by CT after cerebral trauma. Infarcts at the watersheds between the three vascular territories were found with surprising frequency and small infarcts were found in the basal ganglia. It is assumed that these were due to ischaemic or hypoxic events due to cardiac or pulmonary complications during the initial phase.

Carbon Assimilation Characteristics of the Aquatic CAM Plant, Isoetes howellii.

The relationship between malic acid production and carbon assimilation was examined in the submerged aquatic Crassulacean acid metabolism (CAM) plant, Isoetes howellii Engelmann. Under natural conditions free-CO(2) level in the water was highest at 0600 hours and (14)CO(2) assimilation rates in I. howellii were also highest at this time. After 0900 hours there was a similar pattern in (a) rate of free-CO(2) depletion from the water, (b) reduction of carbon assimilation rates, and (c) rate of deacidification in leaves. Rates of daytime deacidification increased under CO(2)-free conditions and as irradiance intensity increased. Nighttime CO(2) uptake was estimated to contribute one-third to one-half of the total daily gross carbon assimilation. CO(2) uptake, however, accounted for only one-third to one-half of the overnight malic acid accumulation. Internal respiratory CO(2) may be a substrate for a large portion of overnight acid accumulation as leaves incubated overnight without CO(2) accumulated substantial levels of malic acid. Loss of CAM occurred in emergent leaf tips even though submerged bases continued CAM. Associated with loss of CAM in aerial leaves was an increase in total chlorophyll, a/b ratio, and carotenoids, and a decrease in leaf succulence. delta(13)C values of I. howellii were not clearly distinguishable from those for associated non-CAM submerged macrophytes.

[Cervical osteoplastic laminectomy for an intramedullary astrocytoma in an eight-year-old child (author's transl)].

In view of the hazards of extensive laminectomy in children, an osteoplastic laminectomy was used in an eight-year-old child suffering from an intramedullary astrocytoma extending from C4 to C7. The laminae were first of all divided bilaterally about 2 mm medial to their origin, with a fine spherical cutter. After dividing the ligamenta flava and the interspinous ligaments the dorsal wall of the spinal canal from C4 to C7 was removed en bloc. The dura was opened and the spinal cord split in the midline after which the microsurgical excision of a spindle-shaped cystic astrocytoma was carried out. After closing the dura drill holes were made bilaterally in the stumps of the pedicles and in the ends of the resected laminae. The entire piece, consisting of laminae and ligaments was replaced and fixed to the stumps of the pedicles with four pairs of wire sutures. Included in the sutures was a small block of bone measuring 2 x 3 x 5 mm, which replaced the bone lost in the saw-cuts and thus prevented any narrowing of the spinal canal. The ligaments were sutured and the wound closed in layers, with drainage. Immobilization in a Minerva plaster for three months.

[Clinical studies on chronic subdural haematomas in 140 adults (author's transl)].

Out of the 140 adult patients with chronic subdural haematoma 17% were female. An overall relationship of CSH cases to the total number of subdural haematomas was 27%. In only 78% of the CSH could a history of trauma be traced. Most of the patients were between the ages of 50 and 70. The main symptoms were headache, abnormal psychic behaviour and paresis. Alcoholism and anticoagulant therapy accounted for only 13% and 3% respectively. Recurrent bleeding occurred in 25% of cases not only after craniotomy but also after burr-holes. The total mortality was 15%. With regard to diagnosis and the indications for operation, angiography which was previously used has, since 1977, been superseded by the CT-scan.

Anterior fusion for cervical spondylosis.

Anterior fusion was performed on 138 patients as treatment for degenerative changes of the cervical vertebral column. The results were checked after up to 11 years in 122 patients and were found to be good in 55%, fair in 3895% and poor in 6.5%. There were complications of phonetic paralysis in one case, Horner syndrome in two cases, 2 wound infections, and complaints about the iliac crest in 3 patients. Kyphosis at the fused segment occurred 26.1% of cases, the average angle being 15.3 degrees, but it did not influence the clinical results. Mortality was 2%. Inadequate visualization of the nerve roots at operation was probably the reason for the segmental deficits and suggestions were made to avoid this by EMG, microsurgical technic, uncusectomy and hemifacetectomy. The results in the presence of myelopathy were much poorer, presumably because of associated degenerative changes in the older patients.

[The use of photon absorptiometry in renal osteodystrophy (author's transl)].

In 58 patients with end-stage renal failure before commencement of regular hemodialyses treatment (RTD) and 58 patients under RTD bone mineral content (BMC) was determined by the use of the photon absorptiometry. Further the effect of a treatment with bitamin D3 and 5,6-trans-25-OH-vitamin D3 on BMC was studied. There existed a negative correlation between the duration of chronic renal failure or of RDT as well as of the serum parathyroid hormone level to BMC. No correlation was found between BMC and serum calcium level. During a 14 months-course treatment with vitamin D3 or 5,6-trans-25-OH-vitamin D3 BMC increased. The method of photon absorptiometry presented itself as an easy and well reproducible technique for routine examinations.
