[Diagnosis and clinical course of extra-uterine gravidity. Catamnestic survey of patients of gynaecological hospital of Jena University (author's transl)].

Reported in this paper are aspects relating to diagnosis and clinical course of 308 cases of extra-uterine gravidity, with reference being made to the authors' own experience and to comparable information in literature. The cases reviewed accounted for 0.86 per cent of all deliveries in the period considered. Tubal gravidity was diagnosed of 98 per cent of the cases and ovarian gravidity in 1.9 per cent, that is six cases. Concomitance of intra-uterine gravidity was established in two cases. The average age of the patients was relatively high, 29 years. Unambiguous correlations were established between the above courses, on the one hand, and parity as previous abortions or abdominal surgery, on the other. Four in 24 pregnancies of women with IUD were tubal pregnancies. Typical menstruation history was recordable from less than 50 per cent of the above patients. Early detection of unruptured tubal gravidity improved along with introduction of laparoscopy. Efforts have always been made to preserve fertility, whenever surgical treatment has been applied. There were no deaths.

[Effect of anesthesia on the condition of newborn infants following cesarean section].

A comparative study of the anesthesia at cesarean section with application of d-tubocurarine in 602 cases and application of only succinylcholine in 69 cases before delivery showed no differences in the Apgars-scores of the newborn. Only severe obstetric complications and long duration of the anesthesia influenced the clinical condition of the infants. There was no correlation between the doses of barbiturates or tubocurarine given to the mother and the Apgars-scores.

[Risks and trends in geriatric gynecologic surgery].

A comparative study of the geriatric gynecological operations of the two periods from 1958 to 1967 and 1968 to 1977 in the Universitäts-Frauenklinik Jena showed an increase of the number of women aged 60 years and more from 6.8% to 8.9% and in the last four years to more than 10%. In 52% a malignant tumor was the main disease. Prolapsed uterus and vagina occured in about 23%. Abdominal hysterectomy was the mostly performed operation. The mortality decreased from 6.6% to 3.3%. Pulmonary embolie was the most frequent cause of death.

[Acute abdomen due to ovarian hemorrhage].

Massive intraperitoneal hemorrhage from ruptured ovaries may occur not only during antikoagulant therapy. In 20 out of total 24 cases of the Universities-Frauenklinik Jena the origin of the hemorrhage was a ruptured corpus luteum, only in three cases an ovarian follicle and in one case a luteinic cyst. Differential diagnosis to ectopic pregnancy is nearly impossible. Explorative laparotomy has to be performed to assure the diagnosis.

[Gestation in patients with heart disease].

A brief report of the special management of pregnant women suffering from heart-disease has been given. There were 188 (0,72%) of all together 26233 deliveries in 170 women with heart-failure. Cesarean section by cardiological indications as a matter of course will be carried out only in few special cases. The management tends to vaginal delivery with shortening the last period by operating. In comparison with the period from 1946 to 1962 the mortality of the mothers decreased from 7,8% to 0,5% the mortality of the infants from 9,8% to 3,2% and the prematurity by weight from 25,5% to 8,5%.

[Appendicitis as a pregnancy complication].

The special problem of appendicitis in the gestationperiod and its management according to the 7 principles recommended by Möbius has been discussed. From 1960 to June 1974 in the gynaecological hospital of Jena University appendectomy must be performed in 44 pregnant women and in 2 cases after delivery. In relation to 32849 deliveries at the same period there was an incidence of 0,14 per cent. Out of them 8 appendectomies were necessary in connection with ectopic pregnancies and in 5 cases with interrupting the gestation. Of 32 pregnant women one had an abortion and in 8 cases with simultaneous cesarean section two infants died. There were no maternal deaths.
