Gerstmann's syndrome.

Although Gerstmann's syndrome has been well documented since it was characterised in the latter half of last century, there has not been much literature on it in the last few years. We present a classical case in a patient who was admitted into hospital for an unrelated problem. We conclude that clinical examination still has a valuable role in neurology, despite the availability of excellent imaging techniques.

Vibramycin programme in the United Kingdom. Winter 1973-1974.

A total of 204 patients between the ages of 20 and 80 and suffering from respiratory tract infections were treated by general practitioners with doxycycline. The infections were representative of those usually encountered in practice except that there were only three cases of pneumonia. The response to treatment, both subjectively and objectively, was good; by the third day, 84% of patients were improved. 89% of patients were cured by the tenth day. Failure to improve was noted in only 5% and failure of cure in 11% of patients. The participating physicians assessed the result of treatment as good or very good in 87% of patients and as moderate in 8%, only 5% of the patients had a poor or negative result.

Retroperitoneal fibrosis associated with Riedel's struma.

A case of retroperitoneal fibrosis with bilateral ureteral obstruction in association with Riedel's struma of the thyroid is reported. There has been a definite increase in incidence of retroperitoneal fibrosis, but with prompt recognition and adequate treatment the mortality rate has been decreased from the original 14%. The association of Riedel's struma with retroperitoneal fibrosis has been noted in the past and its association with sclerosing cholangitis has also been mentioned. It is not known whether Riedel's thyroiditis originates in the thyroid gland and spreads or whether true thyroiditis is part of a generalized process. The temporal relationship of thyroiditis and retroperitoneal fibrosis suggests an extension of fibrosis from the thyroid, but one cannot be sure which condition occurred first. Whatever the cause, the treatment remains the same as for retroperitoneal fibrosis from other causes.

A second international co-operative investigation into thioacetazone side-effects. 1. The influence of a vitamin and antihistamine supplement.

As part of a large-scale international, co-operative investigation into the side-effects produced by thioacetazone employed in the treatment of tuberculosis, an evaluation has been made of a supplement incorporating vitamins and an antihistamine as a prophylactic.Over a 12-week period of treatment, the additive supplement failed to reduce the over-all frequency of side-effects or the frequency of side-effects leading to a major departure from prescribed treatment. There was also no evidence that the more serious side-effects, particularly rashes, jaundice and agranulocytosis, were reduced by the additives, although the occurrence of vomiting, which was however infrequent, was reduced.In view of this lack of appreciable benefit, as well as the higher cost and impaired keeping properties of tablets containing thioacetazone plus isoniazid when the supplement is added, the use of the supplement as a prophylactic cannot be recommended.
