Evaluation and management of foot and ankle disorders: present problems and future directions.

Recent research has raised serious concerns regarding the reliability and validity of the evaluation and treatment scheme proposed by Root et al. Although the Root et al theory is widely referenced in the physical therapy literature and commonly taught in continuing education courses, current issues of concern include: 1) measurement technique reliability, 2) the criteria proposed for normal foot alignment, and 3) the position of the subtalar joint between midstance and heel-off during walking. The intent of this paper is to review these three problem areas which have been identified with the Root et al theory as well as to propose the use of a "tissue stress model" which the authors have found to be an effective alternative for evaluating and treating foot disorders.

Hindfoot pain treated by a leg-hindfoot orthosis. A case report.

This case report describes an orthotic management option for the treatment of persistent subtalar and ankle joint pain in a patient with rheumatoid arthritis. Orthotic design, principles of control, and methods of application are discussed in addition to the advantages and disadvantages of prefabricated and custom-formed orthotic devices.

Strength-duration curve: intrarater and interrater reliability.

The purpose of this study was to determine the coefficients of correlation for intrarater and interrater reliability for producing the strength-duration curve. The intrarater reliability curve was determined by a test-retest by the same examiner within a one-week interval. The interrater reliability was determined by using two examiners, each testing and recording his own results on the same subject within the same hour. The tibialis anterior muscle was tested in 24 healthy men. The interrater coefficients of correlation of 11 pulse durations of the strength-duration curve ranged from r = .979 to r = .771 (mean r = .945). The intrarater reliability coefficients ranged from r = .850 to r = .394 (mean r = .541). Both mean coefficients were statistically significant at p less than .01. The variance interpretation of the correlation coefficient data revealed that intrarater coefficients were not highly consistent (r(2) = .29). Interrater coefficients interpreted by variance estimates tended to be consistent (r(2) = .89). We believe that normal physiologic variation in the human subject was the cause of low intrarater values. The necessity of developing a normalized strength-duration curve is discussed.

Edentulous gnathologic recordings utilizing "vacustatics".

Securing clutches to edentulous ridges by vacustatics for a pantographic recording is useful for effecting a desirable result. This technique is a possible answer for those edentulous patients with occlusal problems who have never known comfort while wearing dentures. A procedure has been described utilizing a modification of the vacustatic technique to obtain accurate pantographic recordings on edentulous patients. These recordings enable the dentist to develop a gnathologic-oriented occlusion for the problem denture patient in an attempt to eliminate destructive loading forces on the residual ridges.
