Recombinant adenovirus-mediated gene transfer to genitourinary epithelium in vitro and in vivo.

Transitional cell carcinoma (TCC) of the bladder is associated with characterized lesions in dominant and recessive oncogenes. The understanding of the molecular basis of tumorigenesis in these instances makes possible the application of gene therapy strategies for TCC. In this regard, the ability to directly access the epithelium of the genitourinary (GU) tract via the urethra provides a practical means to implement these various gene therapy approaches. We thus explored vector strategies to accomplish direct in vivo transduction of GU epithelium. Initially, three human (HT 1197, HT 1376, T24) and one mouse (MBT-2) TCC cell lines were transduced using a recombinant adenoviral vector expressing the firefly luciferase reporter gene, rAd-CMV-Luc. In these studies, reporter gene expression was found to be significantly elevated above background for all four cell lines. Of note, the TCC cell lines HT 1197 and HT 1376 showed expression levels comparable with the cervical carcinoma cell line HeLa, a cell line previously shown to be highly susceptible to recombinant adenovirus-mediated gene transduction. An in vitro time course for T24 and MBT-2 using rAd-CMV-Luc showed peak expression 1 day after transduction for the T24 line and 3 days after transduction for the MBT-2 line, with detectable levels of expression persisting for at least 7 days. As a next step, human and mouse primary tissue deriving from the GU epithelium were transduced using rAd-CMV-Luc. In this assay, luciferase expression levels significantly above background were observed in both instances.(ABSTRACT TRUNCATED AT 250 WORDS)

Serum antinuclear antibodies in women with silicone breast implants.

OBJECTIVE: Recent evidence suggests that immunologic abnormalities are not uncommon in individuals with silicone breast implants. The purpose of our study was to evaluate in a consecutive manner, the prevalence of autoimmunity as assessed by the presence of antinuclear antibodies in a larger number of patients with silicone breast implants. METHODS: Antinuclear antibody (ANA) testing using an indirect immunofluorescence technique was performed on 813 individuals with silicone breast implants. All subjects except for 3 transsexual males, were female. The overwhelming majority, over 99%, were white. The average age of the subjects was 46.2, with a range of 17 to 72 years. RESULTS: ANA positivity was found in 244 of 813 individuals (30%) using a mouse kidney substrate; and in 470 of 813 (57.8%) using a HEp-2 cell line. The most common immunofluorescent pattern found using HEp-2 was speckled, present in 341 (72.5%) individuals, followed by homogeneous pattern in 113 (24%), nucleolar in 63 (13.4%), and 5 (1.06%) were anticentromere. Anti-dsDNA antibodies measured by an ELISA assay were found in 6 of 71 patients (8%). Rheumatoid factor and C-reactive protein were found above healthy controls in less than 10% of cases studied. The high prevalence of ANA found in patients with silicone breast implants agrees with similar observations by others. The finding of anticentromere and nucleolar patterns has great interest and relevance. These fairly distinct ANA patterns are most commonly seen in the idiopathic form of scleroderma and related conditions. CONCLUSION: These findings suggest that ANA positivity is relatively common in individuals with silicone breast implants, and may support the existence of autoimmune mechanisms in the pathogenesis of the clinical manifestations seen in this population.

Restoration by piroxicam of liver glutathione levels decreased by acute ethanol intoxication.

The effect of piroxicam, a nonsteroidal anti-inflammatory drug, on blood ethanol and on triacylglycerols, malondialdehyde and glutathione levels in liver of fed rats acutely intoxicated with ethanol was studied. As previously reported for fasted rats, piroxicam in fed rats decreases the concentration of ethanol in blood and inhibits the hepatic increase of triacylglycerols and malondialdehyde consequent to ethanol administration. In addition, the hepatic depletion of reduced and reduced plus oxidized glutathione observed 8 and 12h after gavage of ethanol was reversed when piroxicam was administered simultaneously with ethanol. The importance of the finding is discussed in light of the paramount participation of glutathione during the cellular oxidative stress.

A pregnane glycoside from the roots of Mandevilla pentlandiana.

A pregnane triglycoside containing a new genin 3 beta,14 beta-dihydroxy-21-methoxy-5 beta-pregnan-20-one has been isolated from the dried roots of Mandevilla pentlandiana. Chemical and spectroscopic evidence for the glycoside are consistent with the structure 3 beta, 14 beta-dihydroxy-21-methoxy-5 beta-pregnan-20-one-3-O-beta-D- diginopyranosyl-(1----4)-O-beta-D-cymaropyranosyl-(1----4)-O -beta-D-cymaropyranoside. It is noteworthy that 3 beta,14 beta,21-trihydroxy 5 beta-pregnan-20-one, biosynthetically related to the genin of this glycoside, has been proved to be a precursor of cardenolides, also produced by this plant.

Phase II study of the safety and efficacy of a 5-lipoxygenase inhibitor in patients with ulcerative colitis.

We examined the effects of an inhibitor of 5-lipoxygenase (A-64077) in 11 patients with mild to moderately active ulcerative colitis (UC). Treatment was a daily oral dose of 800 mg for 28 days. Physicians global rating (scale 1-50) included stool characteristics, rectal bleeding, abdominal and rectal pain, urgency, and general well being; sigmoidoscopic evaluation (scale 1-15) included mucosal vascular pattern, erythema, granularity/ulcerations, friability, and exudate. All of the patients experienced a decrease in discomfort of their disease, and the gross appearance in the sigmoidoscopy improved in eight of them, but were not accompanied by histologic improvement. No significant side effects occurred. A long-term controlled double-blind and dose-effect trial using 5-lipoxygenase inhibitors is needed which may lead to a new therapeutic approach in UC.

Evidence of the existence of a factor that induces C3 receptors on bone marrow cells.

The existence of a molecule responsible for the induction of C3R receptors (C3R) on murine bone marrow cells (C3R inducer, C3RI) is demonstrated in lung conditioned media. The inducer of Fc receptors on murine bone marrow cells (FcRI) and the colony-forming factor MGI (macrophage and granulocyte inducer) are also identified from the same source. The three inducing factors are shown by molecular sieving chromatography and by isoelectric focusing in granulated gel to be three different molecules. The molecular weights estimated were of 35,000, 22,000, and 10,500 for C3RI, MGI, and FcRI, respectively, with isoelectric pH values of 3.9, 6.6, and 7.6 for the same molecules. The C3RI is assumed to be of protein nature, because the molecule lost its biologic activity after trypsin or heat treatment. In the two techniques used, the three peaks of activity were completely separated, suggesting that the induction of C3 or Fc receptors and colony formation are mediated by different molecules and thus regulated independently at the cellular level.

Evidence of the existence of a factor that induces Fc receptors on bone marrow cells.

The existence of a molecule responsible for the induction of Fc receptor (FcR) on bone marrow cells (FcR inducer, FcRI) is demonstrated in conditioned media from the macrophage-like cell line WR19M.1 activated by bacterial lipopolysaccharides. The molecular weight obtained from molecular sieving chromatography in gel and density gradient sedimentation is found to be 18,500 daltons and 16,000 daltons, respectively, with an isoelectric pH of 7.4. The factor is found to be thermolabile and trypsin sensitive. The macrophage and granulocyte inducer (MGI), also known as colony-stimulating factor (CSF) or colony-stimulating activity (CSA), is identified from the same source and found to have a molecular weight and an isoelectric pH different from FcRI. The fractions that contained the MGI did not induce FcR on bone marrow cells, while the fractions rich in FcRI did not induce colony formation.

[Seroprevalence of viral markers of hepatitis B among health professionals. Multicenter study in Mexico].

To determine the prevalence of serological markers for hepatitis B infection among health care workers (HCW) in Mexico we surveyed 1072 volunteers from 26 hospitals in 12 states, from which only 1017 fulfilled the inclusion criteria: 82 patients (8.1%) were excluded because of lipemic and/or hemolyzed serum, leaving 935 persons in the study. The study population consisted of physicians, nurses, laboratory chemists, health laboratory technicians and odontologists. All of them had been working in their respective fields and in contact with biological materials for at least 12 months. None of them had been vaccinated for hepatitis B. We determined the presence of HBsAg and anti-HBs by the ELISA method. The participants' mean age was 31.4 years (range: 18-72) and their mean working time was 7.8 years. 615 were female and 320 male. The HBsAg was positive in 11 (1.2%) and the anti-HBs in 91 cases (9.7%). These results suggest that HCW in Mexico have a greater relative risk of becoming infected with the HB virus than the general population. Relative risks were particularly higher for the health laboratory technicians and the physicians. These results confirm that biohazard measures must be reinforced and that the application of HB virus vaccine could be recommended for health care workers.

[New strategies in the clinical evaluation of patients with colon cancer based on molecular studies].

During the last five years molecular studies allowed important advances in the knowledge of cancer colon with important clinical implications. The main finding was the identification and sequence analysis of the APC gen. Structural alterations of this gene have been detected in patients with Familial Adenomatous Polyposis and Gardner syndrome, which suggest a common disease. Furthermore, alterations of the APC gen appears to be also altered in cases of cancer of colon sporadic. Indicating that structural alteration of the APC gen can be inherited and/or acquired. Restriction fragment-length polymorphisms in the chromosome 5q21-22 can now be used clinically for premorbid diagnosis and counseling in familial adenomatous polyposis. The molecular studies allow the clinician to have a new approach in the management and screening of families with familial adenomatous polyposis. The sequence analysis and specific identification of the structural alteration of the APC gene is a more expensive and sophisticated study, although represent a more direct approach. In the Department of Gastroenterology of the INNSZ we are performing such molecular studies. The main purpose of our group is to proportionate integral clinical-molecular studies for families with hereditary colon cancer, create a national register of these diseases and investigate the molecular bases in order to generate new molecular diagnosis tools.
