CD4+ and CD8+ subsets: naive and memory cells in the peripheral blood of patients with systemic sclerosis.

Systemic Sclerosis (SSc; scleroderma) is associated with several immunological abnormalities, including altered proportion between lymphocyte subsets. Peripheral blood lymphocyte subsets from 25 patients with SSc were studied by two-colour flow cytometry using monoclonal antibodies against CD45RA and CD29 markers, which allow a dissection of CD4+ and CD8+ populations into 'naive' and 'memory' subsets. A decrease of the percentage of CD8+ (p < 0.05) and of CD8+CD29+ (p < 0.001) cells was observed compared to that in 20 age and sex-matched controls. These abnormalities were not significantly associated with the extension of cutaneous disease or other clinical features of SSc nor with treatment, pattern of autoantibodies or HLA phenotype.

Reversible hepatotoxic effects of diphenyl: report of a case and a review of the literature.

A case is reported of chronic persistent hepatitis in a 46-year-old woman poisoned with diphenyl, an aromatic hydrocarbon contained in the paper used to pack citrus fruit, with which this woman was in contact for 25 years. The etiology of the disease is discussed, along with its probable link to simultaneous digestive and cutaneous absorption of the product over many years.

[Coronary atherosclerosis: a long natural history].

In this paper we have tried to explain the natural history of atherosclerosis as a condition marked by intimal vessel expansion tending to the endoluminal proliferation until to the obliteration of the lumen because of the presence of atheroma with fixed stenosis. During the course of the life, the various histologic alterations are responsible of the different ischaemic syndromes, ranging from instable vasospastic angina, to chronic ischaemia caused by fixed stenosis, until to myocardial infarction. Besides, we have outlined that in the young (< 40 years old), the most frequent and important event is the vasospasm owed by the fibromuscular proliferation that could cause an infarction with "intact" coronary arteries. On the other side, in the elderly, are observed specially degenerative atherosclerotic lesions with fixed stenosis and consequent thrombosis, cracking of the plaque as cause of acute myocardial infarction.

[Role of Helicobacter pylori in digestive disease in AIDS].

The frequent combination of chronic antral gastritis with the presence of Helicobacter pylori has been known for ten years. On the basis of studies carried out so far, Helicobacter gastritis is very infrequent in patients suffering from AIDS with digestive pathology. Our study of 317 patients with AIDS confirms the low incidence of this disorder which leads to the conclusion that the treatments intended to combat HIV are apt to modify the physico-chemical conditions of the stomach thus impeding Helicobacter colonization.

[Physiopathologic aspects of non-ulcerous dyspepsia. Considerations of a new diagnostic method].

The above study is an analysis of different dyspeptic syndromes outlining their pathophysiology and clinical features. On this basis, the authors suggest a diagnostic strategy, stressing the importance of certain elements such as age at onset, duration and variability of symptoms. This allows to evaluate the indications for endoscopy or biopsy, or to decide that it is sufficient to continue observation while reassuring the patient that the dyspeptic condition is benign.

[Prokinetic drugs in digestive disorders].

In this paper the authors review the main prokinetic drugs analyzing their pharmacological properties, therapeutic indications and adverse effects. In addition, some more recently introduced substances are taken into consideration which, although they were not initially used for digestive tract dyskinesias, can nevertheless be used in certain conditions. From this point of view, calcium blocking agents employed in spastic conditions of the esophagus and the new antiserotoninergic drugs for treatment of anticancer chemotherapy-induced vomiting are of particular interest.

[The importance of follow-up of colorectal polyps].

In this paper, we have considered the indications for coloscopy for the screening of colon polyposis. On the basis of its sensitivity in detection and follow-up of colo-rectal cancer, we tried to determine the exact procedure to be followed subsequent to the discovery and ablation of a polyp. We have outlined the following parameters to be observed during the period of follow-up: dimension, number, histologic features of the polyp, age, general conditions and familiarity of the patient for colorectal cancer.

[Constipation. New therapeutic trends].

Surgical treatment is warranted only once medical management has proved unsuccessful and should preferably be preceded by psychological evaluation of the patient. So far, surgery has yielded few convincing results in view of the high percentage of failures. After surgery, many patients complain of abdominal pain and diarrhea. The contribution of surgery is highly important in patients suffering from "outlet obstruction", "inertia coli" or Hirschsprung's disease in whom myectomy or Duhamel's operation yield good results.

Abnormal recovery systolic blood pressure response for detecting coronary artery disease in men and women investigated by upright bicycle exercise.

We investigated the clinical significance of recovery systolic blood pressure (SBP) ratio, obtained dividing the recovery SBP at 1st (R1/A) or 3rd min (R3/A) by the peak exercise SBP (before stopping), during upright bicycle exercise in 530 subjects (ranging from 17 to 73 years). Our results may be summarized as follows: 1) we found a higher value of R1/A in control subjects with exercise induced ST depression; 2) the normal range in women was higher than in men; 3) the use of recovery SBP ratios gives a lower sensitivity and a higher specificity than ST segment analysis in detection of CAD; 4) this pattern may be useful particularly in patients with previous myocardial infarction and not detectable ST segment analysis during exercise.

[Gastric and duodenal villous tumor: new diagnostic and therapeutic possibilities].

With reference to recent data reported in the literature, the authors stress the importance of new techniques for the diagnosis and management of gastrointestinal villous adenoma. This is a comparatively infrequent tumor which usually occurs in adults above the age of 50 and has no familial incidence. The tumor is apt to evolve towards malignancy. Radiology, and more recently endoscopy permit to ascertain the diagnosis. Endoscopic resection is the treatment of choice while surgical ablation is indicated in sessile or extensive forms.

Exercise time: a possible source of misleading results during long-term pharmacological studies by multiple stress testings in coronary artery disease.

The assessment of chronic pharmacological treatment of stable angina requires serial exercise stress testings. It is well known that exercise tolerance can be improved by the training effect of performing repeated testings. Our study investigated the values of heart rate, systolic blood pressure, rate-pressure product, and duration of exercise at 0.1 mV ST depression during exercise and the same parameters plus the maximal ST-segment depression at peak exercise, collected from three different tests. The first and second were performed at one-week intervals before, and the third (75 days after the first), was performed after a double-blind study with a drug versus placebo. We found a significant increase of exercise duration at 0.1 mV ST depression and at peak exercise, while 6 of 12 patients increased exercise duration from the second to the third test. Individual variability of exercise duration showed increasing values, ranging from 0 to 71% (first vs. third test). In contrast, the ratio of heart rate and systolic blood pressure did not differ between the tests. Our data criticized the use of mean values of exercise time for pharmacological studies; moreover, individual variability could affect results independently of drug or placebo administration. These findings should be taken into account in order to exclude misleading results.

HLA-DR antigens and anticardiolipin antibodies in northern Italian systemic lupus erythematosus patients.

Eighty systemic lupus erythematosus (SLE) patients attending 3 clinical centers were evaluated immunologically and immunogenetically. No HLA class II antigens were found to be significantly associated with SLE in these patients. A highly significant (P = 6.17 x 10(-7) association was observed between anticardiolipin antibodies and DR7. A lesser association (P less than 0.025) was also observed between DR2 and/or DR3 and anti-Ro (SS-A) antibodies. No relationship was found between any DR antigen and anti-Sm/RNP, anti-double-stranded DNA, or anti-La (SS-B) antibodies.
