[Treatment with rufloxacin of complicated urinary tract infections].

Eighteen patients of either sex (14 M; 4 F), ranging in age from 23 to 79 years, with clinical diagnosis of complicated cystitis due to Rufloxacin sensitive pathogens, were enrolled. Rufloxacin was administered orally at the dosage of 400 mg/die the first day; 200 mg/die the following 6 days or more. The mean duration of treatment was 7.25 +/- 0.78 days. No concomitant antimicrobial therapy was administered during the study. At the end of therapy 5/14 evaluable patients recovered, 9/14 evaluable patients improve; 4 patients were considered by Investigator as "not evaluable". Causative pathogens were isolated in all patients and eradicated in 18 out of 18 bacteriologically evaluable patients (eradication rate = 100%). Neither reinfections nor superinfections occurred. No clinical adverse event related to study medication was reported. The results indicate that Rufloxacin at the oral dose of 200 mg/die is well tolerated and effective in the treatment of complicated cystitis.

[Involvement of the urogenital system in HIV virus infection].

Despite the genitourinary system is prone to infections by the Human Virus of Immunodeficiency, so far poor interest has been focused on urological manifestations of AIDS. We thoroughly reviewed the Literature and examined the different patterns of HIV infections on the kidneys, the prostate and the testes. An extensive description is given.

[Critical considerations on the implant of penile prosthesis: indications, choice of prosthesis, surgical technique, and follow-up].

The surgical techniques for penile prothesis implantation are well known and standardized. Multicomponent hydraulic prostheses offer, without any doubt, the best guarantees in terms of function and aesthetic results. Nevertheless their insertion is sometime troublesome and a superficial knowledge of the technical problems may lead to complete and disappointing failures. In the present work the Authors report their personal experience with various types of penile prostheses and describe in details, the different maneuvers which are needed for their correct implantation. Some variation of the original technique will be also described. The results obtained will also be described.

Value of selective parathormone radioimmunoassay in primary hyperparathyroidism.

Nine cases of primary hyperparathyroidism (PHP) in patients with urinary calculi are reported and discussed. Selective venous catheterization and parathormone (PTH) radioimmunoassay confirmed the diagnosis in all cases, preoperatively discriminated between adenoma and diffuse parathyroid hyperplasia, and permitted exact preoperative localization of 5 of 7 adenomas. The interest of the urologist in PHP and the usefulness of selective PTH radioimmunoassay are discussed and emphasized.

Transcatheter arterial embolization in urological tumors: the use of isobutyl-2-cyanoacrylate.

Transcatheter arterial embolization is a useful technique to manage urological tumors. Herein we describe a new embolic material (isobutyl-2-cyanoacrylate) in 5 cases of renal cancer embolization and in 1 case of bilateral hypogastric embolization for bladder cancer hemorrhage. Isobutyl-2-cyanoacrylate provides a complete, distal and irreversible embolization, making it suitable material in cancer embolization.

Gelatin foam and isobutyl-2-cyanoacrylate in the treatment of life-threatening bladder haemorrhage by selective transcatheter embolisation of the internal iliac arteries.

Three patients with severe haemorrhage from inoperable bladder cancer were treated by selective transcatheter embolisation of both internal iliac arteries. The technical details, results and a rare complication are described. Both gelatin foam and isobutyl-2-cyanoacrylate (IBC) are suitable embolic materials, but we prefer IBC when possible in patients with tumour.

T lymphocytes inhibiting factors in renal cancer: evaluation of differences between arterial and renal venous blood levels of E rosette forming cells.

The cellular immune response has been reported to be impaired in patients with renal cancer and to be increased by removal of the tumor bulk. T lymphocyte levels in the vein of the neoplastic kidney were investigated in 4 patients to assess the presence of a T lymphocyte inhibiting factor produced by the tumor. T lymphocyte levels resulted in significantly decreased tumorous venous blood, whether compared to arterial, contralateral or peripheral blood. A similar result was not found in 10 control patients. Some hypotheses on the nature of this T lymphocyte inhibition are discussed.

Humans

Cyanoacrylates in transcatheter renal embolization.

Isobutyl-2-cyanoacrylate and N-butyl-2-cyanoacrylate were used in experimental trans-catheter embolization of rat kidneys and in two patients as preoperative embolization of renal carcinoma. The agents produced an efficient and lasting embolization, with no evident complication; however, their usage is difficult and an appropriate dosage is also difficult.

Clots, oxycel, gelfoam, barium, and cyanoacrylates in transcatheter embolization of rat kidney.

Transcatheter arterial embolization is an interesting technique; its usefulness in urology has not yet been well assessed. The properties and characteristics of some embolic materials among the more commonly used were investigated by means of a new experimental model, i.e., transcatheter embolization of rat kidney. Autologous clots and Oxycel were found to give a transient embolization, whereas Gelfoam, barium, and cyanoacrylates did not allow recanalization or revascularization. Only barium was found in venous vessels. The findings are discussed in view of the clinical application of the embolic materials considered.

[PTH radioimmunoassay on selective venous samplings as a method of localization of parathyroid adenomas in primary hyperparathyroidism (author's transl)].

Preoperative localization of parathyroid adenomas by various methods is today in increasing clinical use. The practical value of the methods is discussed, with particular reference to the PTH radioimmunoassay on selective venous samplings, which is the most reliable. The authors report on their 38 cases with suspected primary hyperparathyroidism during the last six years.

Evaluation of different types of emboli in transcatheter embolization of rat kidney.

The properties and characteristics of a variety of embolic materials (Clots--surgicel--gelfoam--barium--cyanoacrylates) were investigated by transcatheter embolization of the rat kidney. Clots and surgicel result in temporary embolization, with return of normal renal function. Gelfoam barium and cyanoacrylates produce definitive embolization. Only barium was found in veins.
