Prevalence of high hematocrits in patients with interstitial lung disease in Mexico City.

Erythrocytosis, a known response to chronic hypoxemia, is considered infrequent in interstitial lung diseases. We studied the prevalence of high hematocrit (Hct) values and the relationship between Hct and SaO2 in 79 patients with chronic pigeon breeder's lung (PBL) and 34 with idiopathic pulmonary fibrosis (IPF), all of whom lived in the Mexico City metropolitan area (2,240 m above sea level). Lung biopsy was performed in 31 patients with IPF and 71 with PBL. We analyzed only one simultaneous measurement of Hct and SaO2 per patient (usually the initial measurement) before treatment. No additional cause for anemia or erythrocytosis was detected. Forty-eight percent of the patients with PBL (38/79) and 62 percent of those with IPF (21/34) had high Hct values (greater than 2 SD above mean values for Mexico City); in 14 (12.3 percent) of the 113 patients (nine with PBL and five with IPF), the Hct was above 60 percent. The Hct and SaO2 values displayed a poor correlation for the whole group: Hct = 65.7-0.16(SaO2), r = 0.24, p = 0.012. The correlation between Hct and SaO2 was nonsignificant if patients were separated by diagnosis. For an SaO2 of less than 80 percent, the slope of SaO2 vs Hct was zero. Half of our patients with PBL and IPF had Hct values that were high for the altitude. In most cases, Hct responses fell within the confidence limits reported as normal at high altitudes. We found a poor relationship between Hct and awake SaO2.

Malignant peripheral nerve sheath tumor arising in a "de novo" ganglioneuroma. A case report.

A case of a "de novo" ganglioneuroma showing an internal area of malignant nerve sheath tumor is described. The tumor arose in an 18-year-old girl without a history of von Recklinghausen's disease. Immunohistochemically, the ganglioneuromatous component was positive with anti-synaptophysin, anti-S100 protein and anti-vimentin antisera, whereas the malignant part was immunoreactive only with anti-S100 protein and anti-vimentin antisera. The patient is free of disease 4 years after surgery. The clinicopathologic features of this rare case are discussed.

Reversible prevention of platelet activation by (E,Z)-4,5,9-trithiadodeca-1,6,11-triene 9-oxide (ajoene) in dogs under extracorporeal circulation.

Ajoene ((E,Z)-4,5,9-trithiadodeca-1,6,11-triene 9-oxide), an organosulfur compound derived from garlic inhibits platelet activation induced ex vivo by all known agonists. The effect of ajoene on the thrombocytopenia associated with the circulation of blood through extracorporeal devices such as dialyzers or oxygenators was studied under the following conditions: Ex vivo using fresh, heparinized human blood, circulating through a dialyzer or oxygenator, and in vivo, in dogs subjected to extracorporeal circulation. In both experimental conditions ajoene proved very efficacious in preventing platelet loss (60-65% loss in controls vs. 15-20% loss in the presence of ajoene, p less than 0.01). Moreover, recuperation of platelet function was achieved after 3-4 h in the in vivo experiments.

Sacrococcygeal chordoma: histochemical findings.

Two cases of chordoma of the sacrococcygeal region have been histochemically evaluated. In both cases an irregularly shaped stellate cell, containing large amounts of glycogen, is the prevailing cell type. Neoplastic cells interact with an extracellular matrix consisting of a collagenous framework and a heterogeneous family of glycosaminoglycans with a prevalence of chondroitin sulfate chains and, to a smaller extent, of hyaluronate molecules. We emphasize the value of these data in order to differentiate this tumor from other sacrococcygeal neoplastic lesions.

37 degrees CE rosettes in various malignant and non-malignant disorders.

T lymphocytes, forming sheep erythrocyte rosettes at 37 degrees C, have been described in thymus glands, mitogen and allogeneic cell stimulated lymphocyte cultures, and acute T cell lymphoblastic leukaemia. This paper describes the finding of such lymphocytes in lymph nodes of a variety of disorders including Hodgkin's disease, Lennert's lymphoma, malignant lymphoma of large transformed T lymphocytes, immunoblastic lymphadenopathy, metastatic carcinoma, and other disorders. A large number of tonsils, but not reactive lymph nodes, also contained a significant number of those T lymphocytes. The significance of these findings in relation to interpretation of immune red cell rosettes and to the subclasses of T lymphocytes is discussed.

[Educational technology in nursing: the conceptual framework and the experience of the Latin American centers].

In the first part of the article the assertion is made that educational corpus of technology should not be regarded as something apart, but as a corpus of teaching methods, techniques and resources rationally organized to solve problems of material and conceptual situations and processes. Several educational technologies are explained on the basis of their underlying theoretical frameworks, and grass-root educational technology is advocated because, while it does not emphasize efficiency, it is capable of inducing innovations that are effective in that they actually accomplish changes. The second part reviews the results of the Latin American Program for Educational Technology in Nursing, which PAHO conducts through a network of centers in the nursing schools of several countries. The purpose of the Program is to disseminate modern educational technology and to endeavor to harmonize nurse training with demands and trends in the delivery of health services.

Child-feeding practices and the influence of educational level and mass media in Costa Rica.

This paper reports the results of a survey of child-feeding practices and related social phenomena in Costa Rica, a country representative of many maturing, less developed nations. The data were collected for use in developing a remedial nutrition programme and to provide a basis for a later evaluation of that programme.The results show that child-feeding practices are in need of improvement, particularly among mothers with relatively little formal education and especially during the first year of life and during periods of sickness. More mothers, regardless of their educational level, should be encouraged to breast-feed and to do so for longer periods. The data also showed a need to improve the general health environment in large families as a means of reducing infant mortality, especially when the mother is relatively uneducated. Radio is a particularly promising channel of health communication, especially for the relatively uneducated. Ways in which mass communications media could be used for health education should be explored.

[Evaluation of necrosis caused by preoperative chemotherapy in 9 cases of classic central osteosarcoma].

The authors describe 9 cases of classic osteosarcoma treated with preoperative chemotherapy according to the current schemes and consecutively treated by surgery. All the patients were between 12 and 17 years old. They initially had a needle biopsy or open biopsy to confirm the diagnosis. After 2 cycles of chemotherapy they were treated by limb salvage procedure in 7 cases and by ablative surgery in 2 cases. The surgical specimens were very accurately examined both macroscopically and histologically to evaluate the effects of preoperative chemotherapy. For this reasons the quantity of the tumoral necrosis, that represents the reaction of the tumor to the therapy, was researched and mapped. The intention was to decide whether or not to use the same pharmacological treatment after the surgical therapy. These evaluation criteria do not only have the objective of locally keeping under control the disease, but mostly to oppose micrometastasis. In fact they are almost always present at the beginning of the symptomatology but are not diagnosed with the usual exams. The sites of viable tumor after treatment are: soft tissues around the bone, areas in contact with cartilage and subcortical areas. In 6 cases out of 9 we found macroscopically a fibrous pseudo-capsule around all the tumoral area. The follow-up of the nine cases varies from six to fifty-two months and presents the 66% of disease-free survival.
