Differentiation of smooth muscle in the genital tract of the female mouse and its temporal relation with the development of the Wolffian nerve.

The development of the smooth muscle in the genital tract of the female mouse was studied by light and electron microscopy before and after birth. These studies showed that: a) between 13 days of fetal development and 2 days after birth the cells surrounding the Mullerian duct were undifferentiated and showed a fibroblast-like appearance; b) between 3 and 10 days after birth the cells acquired several characteristics of smooth muscle but they did not seem fully mature; c) between 30 and 180 days after birth the cells acquired a mature appearance; and d) the Wolffian nerve reached the Mullerian duct surrounding tissue before the start of smooth muscle differentiation.

Percutaneous bacillus Calmette-Guerin perfusion of the upper urinary tract for carcinoma in situ.

Ten pyeloureteral systems in 8 patients (mean age 74 years) with cytologically proved ureteral carcinoma in situ (1 combined with ureteral papillary tumors) were perfused with bacillus Calmette-Guerin via a percutaneous nephrostomy tube. In 4 patients cytology results remained negative after 1 treatment course during an observation time of 18 to 28 months. In 1 patient a papillary tumor persisted while cytology results became negative for carcinoma in situ. Two patients with bilateral disease had repeated perfusion of bacillus Calmette-Guerin until cytology results became negative and they remained negative during observation for 18 months in 1. The other patient had a multifocal recurrence of carcinoma in situ, combined with a stage T1, grade 3 urothelial cancer in the bladder after 12 months and a recurrence of carcinoma in situ in 1 ureter after 24 months. In 1 patient treatment was stopped prematurely after severe septicemia. Although our short-term results are promising, percutaneous perfusion of bacillus Calmette-Guerin for carcinoma in situ of the upper urinary tract should be considered as an investigational treatment modality until long-term results are available.

[Macrogeodic coxitis in ankylosing spondylitis].

The authors report 6 cases of hip disease with macrocavities during ankylosing spondylarthritis, including 5 cases from a series of 57 coxites which occurred in 34 patients out of 340 ankylosing spondylarthritis, observed at Hospital Begin, between 1973 and 1982. In light of their observations, they propose a classification of the hip manifestations of ankylosing spondylarthritis. The hip involvement with microcavities in ankylosing spondyloarthritis is quite different from coxites with macrocavities present in rheumatoid polyarthritis, and represent more likely a post-inflammatory arthritic type of hip disease than a particular form of coxitis. Hip disease with macrocavities in ankylosing spondylarthritis is a severe form of hip disease, and frequently requires the installation of a total prosthetic replacement.

Development of the innervation of the urogenital complex in the fetal mouse.

The developing urogenital complex of the fetal mouse was studied by means of silver impregnation and electron microscopy. These studies showed that: 1) the mesonephric field is innervated during prenatal stages (Wolffian nerve); 2) nerve penetration precedes the differentiation of the gonads and related ducts; and 3) the Wolffian nerve arises during the earliest stages from the first pair of abdominal rami communicantes. The identity between the fetal Wolffian nerve and the nerve of the suspensory ligament (higher pathway) of the adult is discussed.
