B lymphocyte markers in multiple myeloma and related plasma cell dyscrasias.

Multiple myeloma and benign monoclonal gammopathies are regarded as monoclonal B cell proliferations in which B lymphocyte maturation is blocked in the final stages of the differentiation cycle. "Blocked" cells accumulate in the body and produce large quantities of immunoglobulins. These are monoclonal, because they come from a monoclonal cell stock. This study presents the results of peripheral B lymphocyte and marrow plasma cell typing designed to reveal the postulated isotypical relationship of the proliferating cells and the serum paraprotein. A good, though not absolute correlation between the two immunoglobulins was noted in most patients. The data, however, was not used in the diagnosis and treatment of these diseases. Further studies with MoAb's will, perhaps, provide a finer subclassification of the B lymphocyte proliferation diseases and assist in their diagnosis and treatment.

Immunoregulation and differentiation markers in monoclonal gammopathies.

Multiple myeloma and benign monoclonal gammopathies (BMG) are regarded as monoclonal B cell proliferations in which B lymphocyte maturation is blocked in the final stages of the differentiation cycle. Further studies with monoclonal antibodies (MoAbs) of the FMC series and of the 791T/36 MoAb will perhaps provide a final subclassification of the B lymphocyte proliferation diseases and hopefully assist in their diagnosis and treatment.

Immunochemical identification of antigens of Brucella melitensis by means of CRIE.

A crude extract of Brucella melitensis was obtained by sonication, centrifugation and dialysis, and analyzed by quantitative immunoelectrophoresis. CIE analysis, using a high titer purified rabbit antibody anti-brucella fraction, showed that the Brucella melitensis extract contained at least 22 antigens that all moved towards the anode. The antigens of the crude extract able to interact with IgG of patients affected by brucellosis, were identified by means of CRIE using sheep 125I-anti human IgG. Fifteen sera of patients affected by brucellosis were analyzed by CRIE and three (Ag6, Ag12, Ag15) of the 22 antigens were able to bind specific human IgG to their corresponding immunoprecipitates. Ag6 was the only one present after one day of the CRIE plate exposure. After treatment of brucellosis patients with antibiotics for two weeks, the three antigens were still able to bind the specific IgG, after two months of treatment the antigens were reduced to two (Ag6, Ag15), while after ten months only one antigen (Ag6) was still able to bind specific human IgG. The brucellosis titer determined with common methods gave normal values after ten months of therapy. These results suggest that Ag6 is the most important antigen specific for human IgG. A preliminary determination of the molecular weight range of these antigens was carried out by a combination of size exclusion chromatography and CIE analysis. The Kav values were all above 66 kD.

[Ultrasonic aspects of combined hamartoma of the pigment epithelium and retina].

The combined hamartoma of the pigment epithelium and retina is a rare developmental tumor characterized by a plaque of thickened retinal pigment epithelium, gliotic and disorganized retina, tortuous retinal and disc vessels, preretinal fibrosis. The ophthalmoscopic and fluorescein angiographic features of the combined hamartoma of the pigment epithelium and retina are widely described in the literature, while the echographic findings are very rarely reported. In the case described here A-scan echography showed that, in the tumor area, the retinal peak was separated from the choroido-scleral peak by an acoustically silent space. B-scan echography revealed a thick and detached retina. The echographically silent subretinal space corresponds to a serous detachment of the macular retina. This lesion, infrequently reported in association with the combined hamartoma, probably depends on the exudation of the retinal vessels and the traction of the hamartoma on the macula. In addition, the echography allowed us to exclude an extension of the lesion to the choroid and the sclera. The electroretinogram and the electro-oculogram were normal as in such localized lesions of the fundus.

N-phenylpiperazine derivatives with hypocholesterolemic activity.

A series of new 4-(4-phenyl-1-piperazinyl)-1-(4-fluorophenyl)-2-(acyloxy)-1-butanones and 4-aryl-5-[omega-(4-aryl-1-piperazinyl)alkyl]-1,3-dioxol-2-ones were synthesized and tested preliminarily for hypolipemic activity. Plasma cholesterol-lowering activity in normal rats was found especially in several dioxolones, two of the most active compounds (6 and 8) being more potent than clofibrate. 4-(4-Chlorophenyl)-5-[2-(4-phenyl-1-piperazinyl)ethyl]-1,3-dioxol- 2-one (8, LR-19,731) has been selected for clinical trials.

Diphasic or prolonged course of viral hepatitis A in children.

A study on nine cases of diphasic viral hepatitis A was carried out in 130 children admitted to pediatric hospital from January to December 1982. One hundred and eight children (83.0%) showed IgM anti-HAV (one of them was a chronic HBsAg carrier), 19 (14.6%) were HBsAg positive at the admission and 3 (2.3%) became positive for anti-HBc IgM marker during the course of the illness. Nine anti-HAV IgM positive children showed an atypical course of their disease in that after a short period of progressive enzyme level normalization, a relapse occurred without signs of subsequent HBV, CMV or EBV infection. Probable although hypothetical interpretations of these cases are discussed.

Immunological studies on the origin of the precursor cells in Waldenström's macroglobulinemia.

Preliminary results obtained with OK anti-T and FMC anti-B monoclonal antibodies in a study of peripheral blood lymphocytes from eight patients with Waldenström's macroglobulinemia and of bone marrow lymphocytes from three patients, are reported. Endocytoplasmic immunofluorescence revealed a 1 to 3% marrow plasma cell fraction in all three cases, together with an approximately 10% increase in the monoclonal precursor compartment. Displacement of the peripheral blood helper: suppressor ratio was also observed using anti-T-lymphocyte monoclonal antibodies.

[Unusual clinical aspects of 3 cases of primary humoral immunodeficiency].

The clinical behaviour of three patients with primary antibody deficiencies was studied. Two of three were affected by X-linked agammaglobulinemia and one by common variable hypogammaglobulinemia that, besides clinical features common to most of the immunodeficiencies, showed pre-existent physiopathologic conditions completely different from the features traditionally found in primary antibody deficiencies. Moreover, the development of some immunologic parameters (monoclonal antibody analysis, surface markers and electrophoretic mobility of lymphocytes) has been carefully checked in these patients after immunotherapy by intravenous infusion of immunoglobulins. The lymphocyte electrophoretic mobility underlines how peripheral blood lymphocytes, faster than those normally controlled before therapy, normalized their velocity after immunotherapy.

[Cytological study of acid phosphatase activity of plasma cells in monoclonal gammopathies].

The activity of acid phosphatase in bone marrow plasma cells was investigated by a cytochemical method in 12 patients with multiple myeloma, in 12 patients with benign monoclonal gammopathy and in 5 normal controls. The activity of acid phosphatase was significantly higher in the multiple myeloma patients compared with the activity observed in controls and in benign monoclonal gammopathy patients (p less than 0.001). It is therefore suggested that this method may be a valuable adjunct in the differential diagnosis of monoclonal immunoglobulinemias .

[Immunology of non-Hodgkin lymphomas].

Cytological and kinetic immunological homogeneity is typical of such lymphomas. Their cytogenesis is broadly connected to the development and differentiation of lymphocytes. It is mainly a question of proliferating B lymphocytes of which the monoclonal membrane receptors are tumoral markers. The final stage of B lymphocyte differentiation is the lymphoplasmacellular proliferation involving monoclonal Ig secretion and consequently the rarefaction and finally disappearance of surface receptors.

omega-Amino-2-hydroxy-p-fluorobutyrophenones: synthesis and preliminary pharmacological evaluation.

A series of omega-amino-2-hydroxy-p-fluorobutyrophenones have been synthesized to evaluate the influence exerted by a hydroxyl group in alpha-position to the carbonyl on the persistence of their neuroleptic and collateral properties. The new compounds generally did not show any neuroleptic activity while some of them were still hypotensive; this last effect, however, did not seem to be related to their alpha-adrenolytic properties.

[Sea-blue histiocytosis. Clinical and immunological study of a familial case].

This case (a young man) presented a clinical picture of a hard periorbital edema, hepatosplenomegaly and a mild bilateral pulmonary fibrosis. The histopatological pattern of our case (as well as his sister affected with the same syndrome) was characterized by the presence of macrophages full of blue staining granules and bone marrow macrophagic infiltrations. In the pulmonary function tests the volumes and compliance test and diffusing capacity were normal. The enzymatic deficit of the macrophage leads to the increase storage of phosphoglicerides and phosphosphingolipids responsibles for the blue staining with Wright-Giemsa stain. Our case fit into the specific pathology of the macrophage cell in accordance with the more recent views of the autonomy of the MPS (Mononuclear Phagocytic System). The known relationship between macrophages and T and B lymphocites have prompted us to study the cellular and humoral immunological behaviour of our case. We have observed an increased IgM immunoglobulins and an increase of IgM surface membrane receptors. Our results will be illustrated and compared with the until known 70 publicated cases.
