Smoker's melanosis. A case report.

Smoker's melanosis is a benign pigmentation of the oral mucosa, predominantly observed on the attached anterior mandibular gingiva and interdental papillae. These macular lesions are independent of genetic factors, therapeutic medication usage, and various systemic disorders. As a group they are often seen after the third decade of life. Due to the onset in adulthood and the progressive darkening, malignant melanoma must be ruled out. A review of the literature and a case report of this interesting and unique entity is presented.

Bilateral traumatic bone cysts of the mandible: an unusual clinical presentation.

This case report describes an unusual clinical presentation of bilateral traumatic bone cysts of the mandible. These lesions presented clinically as bilateral dentigerous cysts associated with the mandibular third molar teeth. Because of this unusual presentation, a combination of clinical examination, thorough history, radiographic studies, surgical exploration, and microscopic evaluation was necessary in order to render a definitive diagnosis on this case. Knowledge and awareness of these features are important to assist in the initial diagnosis as well as patient management and follow up.

The effect of air abrasive polishing on blood pH and electrolyte concentrations in healthy mongrel dogs.

The recent re-emergence of air abrasive polishing as an adjunct to professional tooth cleaning has been accompanied by a substantial amount of research. Most of this research has focused on the efficacy of the Prophy-Jet and similar devices in the removal of plaque and stain and on the effects of such devices on restorative materials and the various hard structures of the tooth. Little attention has been paid to the effects of mucosal absorption of the sodium bicarbonate in the polishing abrasive. The purpose of this study was to measure any changes in arterial blood chemistry resulting from the use of the Prophy-Jet. The values for pH, sodium, and bicarbonate, as well as other electrolytes, were measured in 10 dogs over a 2 hour period following a 5 minute timed use of the Prophy-Jet. Of all the parameters examined, only the potassium concentration showed a statistically significant change, and the magnitude of this change was not felt to be clinically significant. The pH and concentrations of sodium and bicarbonate remained essentially unchanged with the use of the Prophy-Jet.

Differential diagnosis of localized tumors of the gingiva.

Localized tumors arising from the periodontal structures are commonly seen in the oral cavity. Although not a daily occurrence, these tumors will probably be encountered by all practitioners during their professional lives. The majority of localized tumors arising from the periodontal structures are benign, but early recognition and treatment are imperative to prevent invasion and destruction of the surrounding structures.

Ameloblastoma of the mandible: report of extensive destruction within 4 years.

A multicystic ameloblastoma of the mandible is reported in a 54-year-old white man who presented with a complaint of an ill-fitting partial denture due to a swelling in the posterior mandible. This case demonstrates the aggressive character of an ameloblastoma, causing extensive destruction of the mandible in a documented period of less than 4 years.

Histologic survey of the frena of the oral cavity.

The literature shows an apparent lack of agreement concerning the histologic morphology of the frena of the oral cavity, particularly relating to the presence or absence of skeletal or striated muscle. The purpose of this study was to do a histologic retrospective analysis of 40 oral frenal biopsy specimens. The results of the study showed that approximately 37.5% of the frena investigated contained skeletal or striated muscle.

Erythema multiforme following cardizem therapy: report of a case.

A 63-year-old male developed severe, painful ulcerations shortly after beginning therapy for angina with diltiazem (Cardizem). Erythema multiforme is a common adverse reaction to medications. The case report, as well as the role of the dentist in diagnosis and management of these clinical presentations, is discussed.

Fibrolipoma of the gingiva. A case report.

A case of gingival fibrolipoma is described. The lesion presented as a tumor-like mass with a broad base arising from the attached gingiva adjacent to the mandibular left first bicuspid. Microscopic evaluation of the lesion revealed an admixture of mature adipose tissue and fibrous connective tissue. Fibrolipomas are an infrequent variant of lipomas. A review of the literature indicates that a gingival location for such lesions is rare.

Adenomatoid hyperplasia of mucous salivary glands. Report of two cases.

Adenomatoid hyperplasia of the mucous salivary glands is an uncommon benign tumor of the oral cavity. Two cases of palatal involvement are presented. Since clinical differentiation between benign and malignant growth of the palate cannot be established, microscopic examination is necessary. The palate is the most common location of this lesion, and as such it is useful for the periodontist to be aware of this tumor.

Cyst of the incisive (palatine) papilla. Report of a case.

Cysts of the incisive papilla are uncommon variants of the nasopalatine duct cysts. They may be symptomatic or asymptomatic and usually are not seen radiographically. This case report presents a flat, darkly pigmented asymptomatic lesion which required differentiation from a possible malignant melanoma. Due to its location in the anterior palate, this lesion is of interest and clinical significance to the periodontist.

Gingival salivary gland choristoma.

A case of gingival salivary gland choristoma is described. From a clinical standpoint, the lesion appeared as a tumorlike mass of the lingual anterior mandibular gingiva. Microscopic evaluation of the lesion demonstrated the presence of seromucinous salivary gland tissue. This is thought to represent a unique clinicopathologic entity.

Flossing clefts. Clinical and histologic observations.

Clinical and histologic observations of 14 marginal tissue clefts associated with improper floss manipulation are presented. Newly acquired lesions were characteristically inflamed, ulcerated and symptomatic while clefts of chronic duration were asymptomatic and un-noticed by the patient. Cleft sites exhibiting varying degrees of epithelialization generally presented apical bifurcations suggesting reinjury by the patient during lateral positional shifts of the floss. The terminology "flossing cleft" is suggested by the authors to describe linear or V-shaped interdental marginal tissue deformities that result from dental floss-induced injury. While these clefts may serve as a potential source of bacteremia during vigorous flossing and associated oral tissue manipulation, in no instances were they determined to constitute a morphologic impediment to effective plaque control efforts.

Correlation of age and incremental lines in the cementum of human teeth.

It has been long recognized that cementum thickness increases with age. Much literature has recently been devoted to utilizing incremental lines in cementum as an aging criterion in animals, but only one study has been done suggesting this technique in humans. Thirty-one teeth of known age were histologically stained and sectioned to observe incremental lines. Direct predictions of age based on these lines underestimated the age of older specimens. However, there was a correlation between number of lines and age, and with a large enough specimen size, a computer-generated formula for age prediction may be possible.
