[Primary heart neoplasms].

The work analyses the anatomomorphological characteristics of primary tumors of the heart in 106 patients; in 101 patients the tumor was benign (myxoma, neurinoma, rhabdomyoma) and in 5 malignant (angiosarcoma, liposarcoma). Surgical treatment was conducted in 99 patients; hospital lethality was 7.1%. Six patients who refused an operation died at other therapeutic institutions, one patient died on admission to the clinic from occlusion of the left atrioventricular orifice by the tumor. It is pointed out that the diversity and the various degree of the clinical manifestations of the disease are predetermined by the localization and morphological characteristics of the new growths, their size, and mobility. The authors emphasize that knowledge of the anatomomorphological features of primary tumors of the heart and their early diagnosis are conducive to a differential approach in each concrete situation and to the choice of the optimal volume of an urgent surgical intervention.

[Role of smooth muscle cells in reparative processes in a vascular suture of the rat aorta performed using a microsurgical technic].

The abdominal aorta in 56 white rats has been sutured by means of a microsurgical technique in order to study changes that the smooth muscle cells (SMC) of the vascular wall undergo during the restorative process of its integrity and intima formation. The material for histological investigation is fixed in 10% formalin or in Karnoy fluid and embedded in paraffin. For the electron microscopic investigation the specimens of the aorta are treated in 2% and 5% glutaraldehyde solution and in 1% tetraoxic osmium solution and embedded in epon and araldit mixture. The muscular wall SMC actively participate in the reparative processes when the vascular suture is performed by means of the microsurgical technique. The vascular wall SMC ensure the rat aorta restoration in such a way that already 3 weeks after the operation the cut line can be defined only by the presence of the suture thread and by a slight sheet of the connective tissue. Migration and accumulation of the SMC under the endothelium restored its integrity results in the intima formation. Simultaneously, an intercellular substance is forming, that is directly connected with the SMC development.

[Myocardial ultrastructure in the recovery period following aortocoronary shunting in chronic ischemic heart disease patients].

On the material from 96 puncture biopsies of the left ventricle performed on 32 patients with chronic ischaemic heart disease during aorto-coronary shunting under extracorporeal circulation with combined cold and pharmacological cardioplegia lasting from 15 to 92 min the ultrastructure of the myocardium has been studied at the 30th-40th min of the restorative period. After restoration of cardiac activity progression of changes in cardiomyocytes was recorded, more clearly seen in the quantitative stereological analysis of the membranous components of mitochondria, the degree of damage depending on the duration of cardioplegia. Statistically significant (p less than 0.02) decrease of the area of external membranes of mitochondria at the 30th min of the restorative period was detected in patients after cardioplegia exceeding 60 min. Such changes are due, according to the authors, both to enhanced demands of energy supply to cardiomyocytes during the overloads of the restorative period and to the disorders of the microcirculation and of the rheological properties of the blood during reperfusion.

[Results of surgical in undifferentiated small cell cancer of the lung].

The results of surgical treatment of 36 patients suffering from undifferentiated small-cell carcinoma of lung are discussed. During the first two years, 28 patients (78%) died as a result of tumor progression, while 8 patients (22%) survived for 3--20 years after operation. Retrospective study established that the majority of the patients had a small -- size peripheral tumor without metastases in regional lymph nodes. Histological examination revealed a considerable development of stroma having a structure of mature fibrous tissue, few mitoses and no necroses. The findings show that in some cases of early stages of undifferentiated small-cell carcinoma of the lung, surgery may assure a long survival of tumor patients.

[Carcinosarcomas of the lung].

The authors analysed the literature on pulmonary carcinosarcomas and described two own observations of this rare pathology. The predominant occurrence of this tumor in men of 40-65 years, rapid development of the process, frequent asymptomatic course and late surgical help are noted. Clinico-anatomical manifestations of pulmonary carcinosarcomas are nonspecific. Their histological structure is variable and depends on the nature of tissue components involved in the neoplastic process. The authors emphasize a great diagnostic value of prophylactic fluorographic examinations of the population.

[Ultrastructure of the contractile myocardium in chronic ischemic heart disease].

The ultrastructure of the contractile myocardium of the left ventricle of the heart in chronic ischemic heart disease was studied in 24 puncture myocardial biopsies. Marked dystrophic changes were revealed in the cell organelles, both in the membrane systems and in the myofibrillary apparatus, which the authors believe were caused by long-term increasing ischemia of the organ. Besides dystrophy, signs of intracellular regeneration were found in some myocytes. These signs were the presence of euchromatin in the nuclei of the myocytes, and increase in the amount of RNA granules and granular sarcoplasmic reticulum in the sarcoplasm. The prevalence of dystrophic changes on the level of the myocyte ultrastructure may be among the causes of cardiac insufficiency in patients with chronic ischemic heart disease.

[Intraoperative damage to the myocardium and heart failure upon radical correction of the tetralogy of Fallot].

The ultrastructure of contractile myocardium at the stages of anoxia and after switching in the coronary circulation was studied on the material of 128 biopsies of the myocardium from 51 patients with Fallot's tetrad operated radically under conditions of artificial circulation and anoxic cardiac arrest. Heterogeneity of cell organelles damage made it difficult to evaluate myocardial injury on the whole. To make morphological data more objective, a geometric method of quantitative analysis was used which permitted to distinguish two groups of damage and to correlate them with the pattern of cardiac activity restoration and the course of the postoperational period. The results indicate some relationship between changes in the ultrastructure of mitochondria and the development of cardiac insufficiency in patients with Fallot's tetrad.
