Role of International Classification of Diseases in medical record management.

The Medical Record is the major source of Health information. Therefore, it is necessary to maintain good patient records by documenting all the required information, e.g. history, physical examination, progress notes, investigations, treatment including surgical procedures, complete diagnosis and end result, etc., as per the guidelines stated in the International Classification of Diseases published by World Health Organisation. If the documentation is accurate and complete, the health care information could be obtained and classified accurately and completely according to International Classification of Diseases published by WHO. Hence, International Classification of Diseases plays an important role in managing good medical records.

Periodic evaluation of medical record organisational needs.

There is a need periodically to evaluate each and every department in the hospital, to assess and take appropriate action on time, to provide the best possible service to patients and to meet the set goals of the hospital. The medical records department cannot be an exception to this. In fact, medical records is one of the most important service departments in the hospital and one which plays a very crucial role in rendering efficient patient care. As such it is imperative regularly to evaluate the medical record organisational needs, including the work performed by the Department and its staff. In Riyadh, a quarterly report in the following prescribed proforma has to be submitted to the following: Medical Records Committee; Quality Assurance Committee; Hospital Administrator; Government; by specified dates.

The role of medical records in infection control.

The concept of "infection" has been well known to the medical world ever since an interest began to be taken in medicine. Infection has taken many hundreds of thousands of lives, and even today it is a very serious threat for human beings. Hospital, or nosocomial infections (NCI) are a major health problem in every medical institution in the world. In the light of the seriousness of the problem, this paper is to demonstrate how medical records can play an important role in effective alerting and infection control programmes before an outbreak turns into an epidemic.

Medical records in leprosy programme.

It is an established fact that Medical Records play a vital role in the care of various diseases but it has a very important and significant role in a disease like leprosy. Its importance should not be underestimated as Leprosy poses many problems which have some unique characteristics and special features. First of all it is to be accepted that our knowledge of Leprosy is still none to no where, in spite of the age old nature of the disease it is imperative to have proper records of the disease which may lead to invaluable results.
