[The effect of Recormon therapy on the quality of life of patients on hemodialysis treatment].

Recormon was given to 34 patients on hemodialysis. The drug's effects on quality of life were judged by the results of clinical, laboratory, experimental and psychological dynamic investigations. The highest effect occurred on the therapy month 6-12. Recormon contributed to relief of asthenic and depressive syndromes, to an increase of activity and improvement of well-being, quality of life by Karnofsky scale within the first year of treatment. The dynamics of life quality changes depended on initial psycho-physical condition of the patients, the age, blood pressure, hematocrit. Recormon treatment is associated with the risk of euphoria.

[The characteristics of the prevention of HB viral infection in hemodialysis units].

Patients and medical personnel of some hemodialysis centres of the north-west region of the Russian Federation were examined for HB-virus infection markers using enzyme immunoassays. Subjects free of HB markers were immunized with ENGEPIX-B vaccine. Immunogenicity of the vaccine was assessed one month and one year after the immunization. Epidemiological HB factors in hemodialysis centers were studied on the simulation computer model. HB-virus-containing blood transfusion factor is not the only determinant of HB-virus infection prevalence. The virus can be transmitted via hemodialysis facilities. Immunization effects proved inferior in the hemodialysis unit patients. It is stated that vaccinoprophylactic policy in the patients of the dialysis centers and relevant equipment need serious improvement to prevent hepatitis B outbreaks.

[The course of chronic kidney failure during acetate and bicarbonate hemodialysis].

In patients with chronic renal failure (CRF) given acetate (n = 55) and bicarbonate (n = 45) hemodialysis, the dynamics of the progress of uremia symptoms estimated in marks, the index of the lean body weight, the rate of urea generation, the magnitudes of the dialysis indices according to creatinine and urea, TACur and TADur were studied on a monthly basis for many years. The actual survival rates were calculated. It has been demonstrated that as compared to acetate hemodialysis, in patients on bicarbonate hemodialysis, the progress of uremia symptoms is significantly decelerated (p less than 0.05) and hypercatabolism of proteins is decreased by the 4th year of the observation period. This enhances the patients' medical rehabilitation and noticeably increases the actual survival of the patients (p less than 0.001). Advantages of the treatment of CRF patients by bicarbonate hemodialysis are under discussion.

[Vapor-phase gas chromatographic determination of acetate in the blood].

The suggested gas chromatographic vapor phase analysis of the blood for the direct determination of the blood acetate concentration is performed with Soviet equipment and is based on expulsion of acetic acid from its salts and other derivatives after addition of potassium bisulfate and heating of the blood sample at +105 degrees C for an hour in a hermetically sealed flask. To directly measure acetate, a certain vapor phase volume is let into the gas chromatographic evaporator after equal distribution of acetate in the gas and liquid phases is achieved. This method was used to measure the blood acetate levels in the patients with renal insufficiency treated by hemodialysis. The total content of the blood acetate in health is 1.6 +/- 0.3 mmol/l; this value essentially grows in acetate hemodialysis. In some patients acetate levels persist increased between the dialysis courses. The suggested method may be useful for monitoring the blood acetate levels in the patients treated with hemodialysis and for the detection of subjects with acetate intolerance.

[The effect of acetate and bicarbonate hemodialysis on protein and lipid metabolism in patients with chronic kidney failure].

In 26 patients with chronic renal failure undergoing hemodialysis, use was made of an acetate solution (40 mmol/l) and in 7 patients, of a bicarbonate solution (35 mmol/l). The control group was made up of 20 healthy volunteers. In addition to better tolerance, the bicarbonate dialysis favoured adequate correction of acid-base balance, normalization of hypercholesterolemia, lowering of the rate of protein degradation. Among patients on the acetate dialysis, subgroups with normal (9 persons) and delayed (17 persons) acetate utilization were distinguished. A close relationship was discovered between blood acetate concentration before procedure and the rate of urea generation (r = +0.79, p less than 0.05). It is assumed that the acetate dialysis intolerance is determined by protein hypercatabolism under the conditions of delayed utilization of acetate by the peripheral tissues.

[Bicarbonate hemodialysis in the therapy of chronic kidney failure].

A method of random sampling was applied to 10 CRI patients to analyze the results of 24 hemodialyses with acetate solution for dialysis (35 mmol/l) and 34 hemodialyses with bicarbonate solution for dialysis (35 mmol/l). Significant reduction of complications like headache, nausea, vomiting, tachycardia, dyspnea, extrasystole was observed in bicarbonate dialysis. The concentration of mean molecular uremic toxins was decreased from 5.75 +/- 0.84 mmol/l in acetate dialysis up to 2.63 +/- 0.21 mmol/l in bicarbonate dialysis. The content of intracellular potassium returned to normal. The concentration of serum cholesterol was decreased from 5.6 +/- 0.4 up to 4.8 +/- 0.2 mmol/l. These data indicated a favorable effect of bicarbonate dialysis on intracellular metabolism. Preliminary data did not confirm the normalizing effect of bicarbonate dialysis on the development of uremic osteopathy.

[Iron reserves in patients with chronic renal insufficiency].

Hemorrhage values and the amount of iron entering the body with drugs and blood transfusions were determined in 107 patients with the terminal CRF stage. Of them 59 received regular hemodialyses. The level of serum iron and ferritin as well as iron reserves in the body were investigated at the start and end of the study. In the end a histochemical study of the content of hemosiderin in the bone marrow, liver and spleen was performed. A close interrelationship of iron reserves determined with a modified desferal test and the level of serum ferritin (r = 0.94) was established. The highest iron reserves were revealed in the patients receiving blood transfusions and parenteral iron drugs. Criteria for the assessment of iron reserves in patients with renal failure were determined by means of the modified desferal test and investigation of serum ferritin. Normal ferritin reserves in such patients corresponded to serum ferritin values within the range of 50-400 micrograms/l and indices of the modified desferal test ranging from 0.4 to 2.0/0.5 g of desferal. Of a degree of hemosiderosis one could judge on the basis of a histochemical investigation of tissue hemosiderin only. Iron drugs per os were proposed for the prevention of disorders of iron balance in patients with renal failure.

Haemoperfusion in the treatment of chronic renal failure in patients on haemodialysis.

The paper is concerned with materials characterizing physicochemical properties of haemosorbents of varying grades. Adsorption of different uraemic toxins was studied. The possibility of microembolization of internal organs and tissues with haemosorbent particles was explored in the course of prolonged treatment with the use of haemosorption. Different approaches to connecting the sorption column to the dialyzer were examined to disclose an optimal variant. The authors report the results of applying haemosorption in patients who had received haemodialysis therapy to prevent complications. The blood response to the prolonged haemosorption treatment was analyzed. It is recommended that long-term (6-7 hours) sessions of haemosorption might be performed using carbons having high sorption capacity.
