Diarrhea in neonates at the Subdivision of Perinatology Dr. Pirngadi Hospital, Medan.

A retrospective study on diarrheal manifestation in newborn babies was conducted at the ward of Neonatology, Dr. Pirngadi Hospital Medan from January to December 1988. There were all 3367 babies hospitalized during the study period. One hundred and thirty five (4.0%) had diarrhea, including 106 (78.5%) babies with birth weights of greater than or equal to 2500 gram. Of 2478 babies with spontaneous delivery, 34 (1.4%) babies had diarrhea, while of 889 babies with obstetrical intervention delivery 101 (11.4%) babies had diarrhea. The difference was statistically significant (p less than 0.001). Of 135 babies with diarrhea, there were 82 (60.7%) with moderate and 31 (23.0%) with severe dehydration. The mortality rate was 47.5% in patients with accompanying diseases and 0.0% in them without. The mortality rate in babies with birth weights less than 2500 gr was 37.9% and 16.9% in babies with birthweights greater than or equal to 2500 gram (p less than 0.001). The mortality rate increased with the increase of severity of dehydration (p less than 0.03).

Cerebral palsy.

Epidemiology of cerebral palsy is complex, being influenced by various factors, some of which are unknown and it is commonly accompanied by other abnormalities, thus involving a variety of specialization in their treatment. In a period of 1978-1985, a retrospective study of cerebral palsy cases was performed at the Child Neurology Subdivision Department of Child Health School of Medicine, University of Sumatera/Dr. Pirngadi Hospital, Medan. There were 40 cases (1.59%) out of 2,505 new child neurological cases, consisting of 20 males (50%) and 20 females (50%). The youngest was 4 months and the oldest was 7 years and 5 months of age. Thirty two cases (80%) appeared predominantly with the age less than 3 years. The predominant cause was neonatal asphyxia in 11 cases (27.5%), the uncertain causes were found 8 cases (20%). Spastic cerebral palsy was found in 28 cases (70%) and there were 13 cases (32.5%) accompanied by microcephaly.

Immunization in the Well Baby Clinic of Dr. Pirngadi Hospital Medan.

A retrospective study on an immunization programme was carried out in the Well Baby Clinic, Child Health Department, School of Medicine, University of North Sumatera, Dr. Pirngadi Hospital, Medan from January to December 1986. Of the 5951 total visits (an average of 495 visits each month) 3960 were primary and 1991 secondary visits. Most were first children in the family (39.9%) of which 37.6% were from one child families. Of 3275 live births 57.6% were immunized with BCG immediately after birth. The average BCG immunization visits to the Well Baby Clinic was 17 visits/day. When compared with DPT I and polio I visits we recorded a decrease of 27.1% for DPT II; 47.9% DPT III; 27.4% polio II; and 49.1% polio III respectively. Daily coverage of measles immunization visits was 4.6%. Complete immunization among the under-one-year-old children was 5.7% of which 44.9% were of the 9-12 months old group. The simultaneous immunization (BCG, DPT, polio, measles) comprised 0.5% of all visits. 4.1% of all visits were not immunized of which 36.5% was because they were beyond the immunization schedule.

Congenital malformation among newborns at Dr. Pirngadi Hospital Medan during 1981-1984.

A study on the incidence of congenital malformation had been assessed among 15,185 newborns delivered in the Neonatal Unit, Dr. Pirngadi Hospital Medan during 1981-1984. Still-births were not included in this study. Out of these 15,185 newborns there were 77 cases (0.51%) of congenital malformation. The four leading malformations were pes-equinovarus 7 cases (9.1%), labiognathopalatoschizis, hydrocephalus and anencephalus 6 cases each (7.7%). The number of congenital malformations was higher in the age group of mothers older than 35 years (0.78%) and in the group of babies born in the birth order as third and further (53.85%) and as first born babies (33.33%). From 77 cases with congenital malformation only 2 (2.56%) were operated soon after birth, while 49 cases (64.1%) went home without surgical intervention, and 28 cases (35.9%) died during hospitalization.

Rheumatic fever and rheumatic heart disease at the Department of Child Health, School of Medicine, University of North Sumatera/Dr. Pirngadi Hospital, Medan (1983-1985).

A retrospective study on rheumatic fever and rheumatic heart disease in children was conducted at the Department of Child Health, Dr. Pirngadi Hospital Medan, during 1983-1985. The patients consisted of 43 females and 30 males. Most of the patients were over 12 years of age. Of the 73 patients there were 60 patients (82.19%) accompanied by valvular disorders. The most frequent major criteria of Jones found in this study were carditis and polyarthritis. Thirty one patients (42.46%) had functional status (NYHA) of grade II-IV, and 43 patients (58.90%) had cardiomegaly. The major ECG findings were enlargement of the atria or ventricles and first degree AV block. Compliance was only achieved in 25 (34.24%) cases. Rheumatic fever and rheumatic heart disease still remain a challenge for the medical professionals to cope with in the region.

Perinatal mortality and morbidity at Dr. Pirngadi Hospital Medan 1985-1986.

A retrospective study was conducted on babies born during January 1985 to December 1986 at Dr. Pirngadi Hospital Medan. The aim of this study was to evaluate perinatal mortality and morbidity, and various possible factors related to the subject matter. The main results can be summarized as follows: There were 7102 deliveries during the study period consisting of 999 babies weighing less than 2500 gram and 6103 with body weight of 2500 gram or more. Perinatal mortality rate was 563.56% in the first group and 78.49% in the second while the overall++ mortality rate was 146.72%. Rate of perinatal demise was high in babies born from mothers in the age groups of below 20 and above 35 years, namely 681.82% and 202.19%. It was also high among primiparae (165.67%) and more so among grandmultiparae (246.46%). There were 1966 (30.49%) ill newborn babies with asphyxia neonatorum accounting for 44.91%, infection 30.42% and respiratory problems 9.21% of the main causes of illness, while respiratory problems (40.05%) and infection (28.68%) constituted the main causes of death. We concluded that the rate of perinatal mortality and morbidity is still high at this hospital. Quality of prenatal and neonatal care with extensive public health education is necessary to be enhanced for the reduction of perinatal mortality and morbidity.
