Loss of heterozygosity on chromosome 13 is common only in the biologically more aggressive subtypes of ovarian epithelial tumors and is associated with normal retinoblastoma gene expression.

We examined the frequencies of loss of heterozygosity on chromosome 13 in 77 primary ovarian epithelial tumors subdivided into cystadenomas, tumors of low malignant potential, low grade carcinomas, and high grade carcinomas. Such losses were found in approximately 50% of high grade carcinomas but were not detected in any of the other tumor subtypes (P < 0.0001), suggesting a strong association between these abnormalities and the high grade carcinoma phenotype. The tumors were also examined for abnormalities in expression of the retinoblastoma susceptibility gene (RB). This was assessed by immunohistochemical staining of archival tumor sections with a polyclonal antibody directed against both the phosphorylated and the underphosphorylated forms of the RB protein. Most of the tumors, including those with allelic deletions on chromosome 13, showed normal RB nuclear protein staining patterns. We conclude that loss of RB expression is not a frequent event in primary ovarian carcinomas and that this gene is probably not the target of the frequent allelic deletions on chromosome 13 found in high grade ovarian carcinomas.

Nuclear reactivity for estrogen. A possible diagnostic tool in differentiating colon cancer metastases in the ovary from primary mucinous ovarian tumors.

Tissue distribution of carcinoembryonic antigen (CEA) and estrogen was analyzed in eight primary ovarian mucinous carcinomas, three primary mucinous colonic cancers, and four ovarian metastatic lesions from colonic and pancreatic adenocarcinomas. Primary ovarian mucinous carcinomas and metastatic colonic carcinomas had similar localization of CEA within the epithelial cytoplasm. Estrogen localization, in contrast, was shown within the epithelial nuclei of seven of eight primary ovarian mucinous tumors; whereas the nuclei of metastatic and primary mucinous colonic carcinomas did not have intranuclear estrogen. The cytoplasm and the stroma of both primary ovarian and metastatic colonic carcinomas stained for estrogen.

Size and weight determinations of nongravid enlarged uteri.

Uterine size was estimated in 66 women before hysterectomy by bimanual examination, uterine sounding, and pelvic ultrasound. The results were compared with weight and dimensions recorded after removal. There was greater error in estimation of uterine size using bimanual examination (standard error of the estimate [SEE] = 2.6 weeks) or uterine sounding (SEE = 2.2 weeks) than with ultrasound (SEE = 1.06 weeks). A close correlation existed between uterine size and weight (r = 0.95).

Pregnancy complicated by ruptured endometrioma.

Pregnancy complicated by ovarian endometrioma is rare; only four cases have been reported. Although infertility and endometriosis are closely related, endometriosis may be found with pregnancy and, although it has been written that endometriosis will regress with pregnancy, occasionally complications may occur. This report illustrates such a complication with the rupture of an ovarian endometrioma.

Hemodynamic observations in evacuation of molar pregnancy.

The high incidence of pulmonary complications following evacuation of molar gestation at 16 weeks' size of greater (27%) prompted us to institute hemodynamic monitoring in seven of these patients in an effort to determine etiologies and possible modes of therapy for this potentially life-threatening complication. Our data indicate that following suction curettage with general anesthesia there appears to be impairment of ventricular performance of a transient nature as manifested by increases in central venous pressure, mean pulmonary artery pressure, and pulmonary capillary wedge pressure, despite a slight elevation in cardiac index and a decrease in systemic vascular resistance. The possible role of general anesthesia in the development of these changes, as well as the role of the colloid osmotic pressure to wedge gradient in the development of pulmonary complications, is discussed.

Listeria monocytogenes chorioamnionitis: diagnosis by transabdominal amniocentesis.

Two cases of Listeria monocytogenes chorioamnionitis are presented in which transabdominal amniocentesis was used to confirm the diagnosis and to obtain reliable intrauterine culture material free from lower genital tract contamination. The mechanisms of infection with Listeria monocytogenes in the pregnant patient are discussed.
