Amlodipine and physiological responses to brisk exercise in healthy subjects.

Recent studies have questioned the safety of calcium antagonists in general, and short-acting dihydropyridine derivatives in particular. Reasons include excessive catecholamine stimulation after stress. We therefore wanted to assess whether amlodipine, a second generation dihydropyridine with a prolonged plasma half-life, would show a more favourable haemodynamic and biochemical profile after strenuous exercise. For this purpose, we studied 9 healthy volunteers in a double-blind, randomized, placebo-controlled trial. After 10 days of amlodipine, 5 mg orally daily or placebo therapy, volunteers performed a treadmill effort test; the sequence was repeated after a 2-week washout period. Amlodipine caused a significant increase in mean resting heart rate (HR) (from 65 +/- 3 to 70 +/- 3 beats/min, p < 0.05), without changing systolic or diastolic blood pressure (SBP, DBP). Post-exercise haemodynamic responses were similar while on amlodipine or placebo therapy. Amlodipine did not alter the normal profile of resting or exercise-induced metabolic [plasma glucose, serum K+, serum free fatty acid (FFA)] and hormonal [plasma cortisol, growth hormone, prolactin, insulin, epinephrine (EPI) and norepinephrine (NE)] responses--although plasma EPI concentrations dropped significantly lower (p < 0.05) at 5 min and 15 min post-exercise while on the calcium antagonist. We conclude that amlodipine has a largely neutral effect on the physiological profile after brisk exercise in healthy young subjects and that this may prove to be a useful property for a vasodilator drug.

[Magnetic resonance of the prostate: critical evaluation of diagnostic possibilities].

The authors retrospectively reviewed 140 MR examinations of the prostate to assess the presence of similar findings in the different patients affected with the same prostatic disease. In 35 patients with acute phlogosis, MR imaging showed enlargement of peripheral gland, with increased signal intensity on T2-weighted images, while reduced peripheral gland with low signal intensity was demonstrated in chronic phlogosis. Five patients presented roundish cystic lesions, with clear-cut outlines, clearly distinguishable from the surrounding parenchyma. Benign Prostatic Hyperplasia (BPH) was observed in 32 cases. 52% of them had multiple nodules with high signal intensity, 40% had hyperintense enlarged central glands, with no evidence of BPH nodules, and the remaining 8% presented single/multiple nodules with low signal intensity. In 56 patients with prostatic carcinoma the tumor extent ranged from 18-20 mm nodules to 5-6 cm masses with involvement of periprostatic adipose tissue or surrounding organs. The smallest lesions were always found in the peripheral gland and had low signal intensity on T2-weighted sequences. In spite of the different findings in the patients affected with BPH, the study proved MR imaging to be able to distinguish the various patterns of the different prostatic diseases.

[Surgical treatment of pulmonary metastases. A personal case].

A particularly interesting case of pumonary metastasis due to renal adenocarcinoma, treated with right medio-inferior bilobectomy is reported. The topicality and complexity of the problem of the surgical treatment of lung metastases is the reason for the present contribution. The criteria for choice, indications for treatment and the limitations of the form of surgery are reviewed in brief.
