Cell cycle-regulated modification of the ribosome by a variant multiubiquitin chain.

Ubiquitin is ligated to L28, a component of the large ribosomal subunit, to form the most abundant ubiquitin-protein conjugate in S. cerevisiae. The human ortholog of L28 is also ubiquitinated, indicating that this modification is highly conserved in evolution. During S phase of the yeast cell cycle, L28 is strongly ubiquitinated, while reduced levels of L28 ubiquitination are observed in G1 cells. L28 ubiquitination is inhibited by a Lys63 to Arg substitution in ubiquitin, indicating that L28 is modified by a variant, Lys63-linked multiubiquitin chain. The K63R mutant of ubiquitin displays defects in ribosomal function in vivo and in vitro, including a dramatic sensitivity to translational inhibitors. L28, like other ribosomal proteins, is metabolically stable. Therefore, these data suggest a regulatory role for multiubiquitin chains that is reversible and does not function to target the acceptor protein for degradation.

[Neurological spectrum of pre-eclampsia and eclampsia].

Preeclampsia is a disorder specific to pregnancy which can affect all maternal organs. Cerebral involvement with the occurrence of convulsions is the defining factor for eclampsia. The most prominent signs of cerebral dysfunction include headache, hyperreflexia, visual disturbances, confusion and/or altered state of consciousness. Patients usually recover completely however some patients remain disabled due to cerebrovascular damage. Intracerebral and subarachnoidal hemorrhage are severe however rare complications of eclampsia. TCD investigations regularly find vasospasm in all great cerebral arteries. Reversible hypodense lesions in the white matter on early CT-scans and increased signal intensities on T2-weighted MRT images indicate local edema. The etiology of preeclampsia and eclampsia remains unknown and its current pathophysiology is still hypothetical. The clinical picture may best be explained by an endothelial dysfunction with increased vascular sensitivity to circulating pressure agents as well as by a structural endothelial lesion with fluid loss from the intravascular compartment.

Mapping of G2/M-phase prevalences of chaperon-encoding transcripts by means of a sensitive differential hybridization approach.

The sensitivity of the differential hybridization approach is significantly increased by the application of size-selected probes. RNA from elutriated phase-synchronous Ehrlich ascites tumor (EAT) cells has previously been used to prepare cell cycle phase-specific cDNA libraries in the in-vitro transcription vector pBluescript. PCR amplification of the libraries with vector-fitting primer pairs generates amplified cDNA reflecting the mRNA complexities of cells in G1, S and G2/M phases. Probes with reduced complexities were recovered after side-by-side electrophoresis of equal amounts of PCR-amplified cDNA and elution of probes from parallel gel sections. Such size-selected probes release significant differential clones which escape their detection in the conventional differential hybridization approach. Three clones hybridizing preferentially with the G2/M phase-specific probe were further characterized. The genes were identified by their nucleotide sequences. They encode proteins known to be involved in protein folding: heatshock cognate protein, HSC 70; heatshock cognate protein, HSC 73; eta subunit of the chaperonin containing TCP-1 complex, CCT. The G2/M phase-prevalent expression of these genes were further verified on the mRNA and on the protein level by Northern and Western blot analysis which confirms the significance of the differential hybridization approach and which indicates that the expression of this group of proteins increases with cell cycle progression. The expression of the chaperonin-containing TCP-1 complex appears to be specifically linked with the S to G2/M phase transition of the cell cycle.

[Alcoholic delirium--a potentially life-threatening condition].

Full-blown alcoholic delirium is characterised by the symptoms of an exogenous psychosis and signs of vegetative dysregulation. Organic brain disease may initiate its development. The pathogenesis of the condition remains hypothetical. Therapeutic drugs of first choice have long been clomethiazole and benzodiazepines. Basic sedative treatment may be usefully supplemented by highly potent neuroleptic agents. This, together with improvements in intensive medical care, has reduced the mortality rate to 3 to 8%. Concomitant organic diseases may complicate and prolong the condition. A defective state is a possible outcome.

[Alcohol delirium--pathogenesis and therapy].

Alcohol-induced delirium tremens (DT) is a well-known disease with an unpredictable, either favorable or fatal, spontaneous course. The cardinal symptoms are disorientation, hallucinations and autonomic lability. The pathogenesis of DT is still unknown, and the numerous hypothesis have spawned as many therapeutic approaches. Drugs with a cross-tolerance to alcohol have so far proved best. Benzodiazepines and clomethiazole have been considered the drugs of first choice for the past 20 years and more. Improvements in intensive care has helped lower the mortality rate to 3 to 8%. Major tranquilizers with a strong antipsychotic effect and anti-epileptic agents can effectively expand the sedative management of DT. Cerebral convulsions at the beginning of DT appear to dispose the patient to a prolonged course, and preexisting or concomitant disease affecting other organs (complicated DT) clearly prolongs the duration of delirium. The risks and contraindications of clomethiazole are emphasized. The course and outcome of DT is unpredictable--in our patients we found a mortality rate of 3.7% and 12.3% end up in a defective state.

[The early prognosis of massive infarcts of the cerebral cortex. Follow-up of 49 patients].

Mass cerebral infarction have a high lethality already in the first week after onset. Surviving patients often have severe residual neurological deficits. We analysed the clinical course of 49 patients with space-occupying cerebral infarctions in a retrospective study. We differ subtotal infarction in the region of middle cerebral artery (MCA) from complete infarction and from complete infarction in the region of MCA with additional infarction on the same hemisphere. The quick development from depression of consciousness, the Babinski-phenomena on both sides and the central failure of respiration were the most valuable clinical predictors. Together with the clinical parameters, the Mathew-score and the CT and EEG-results it is possible, to estimate a reliable prognosis. The reduction and termination of no longer useful treatments will be discussed.

[Contact-free temperature measurement in the heart by means of infrared telepyrometry].

The actual temperature of the myocardium requires the surgeon for the assessment of the ischemia time under hypothermic conditions in operations on the heart. The problem of the contact free temperature measurement on heart was solved by means of the infra-red telepyrometer. Preponderant technical aspects of the application in the cardiosurgery were described. On the basis of the physics the function of an infra-red contactlesspyrometer was discussed and the first results of own investigations about the contact free measurement of the epicardial temperature.
