Results of transluminal angioplasty of infra-popliteal arteries.

Transluminal angioplasty is rarely used to treat arterial diseases at leg level. During the last 5 years we have performed 25 distal artery angioplasties in 24 patients. Average age was 70 years, half of them were diabetics. Twenty patients had critical ischemia and 4 asymptomatic patients had critical stenosis distal to a previous patent infra-genicular bypass. Arteriography showed a single leg artery stenosis in 23 cases and a two distal artery stenosis in one case. No material other than guide wire was used to pass through the stenosis. All the dilation was performed in the operating room. Immediate results were good in 20 patients (21 stenosis). Four early failures occurred: one technical, one for progressive limb gangrene with secondary amputation, one postoperative thrombosis and one residual stenosis over 50%. Six patients had recurrent stenosis at mean 9 months. Five of them were treated with a new balloon angioplasty and the last with femoro-distal bypass. The 2 years cumulative primary and secondary patency rates were respectively 46 and 64%. With only guide wire and balloon catheters it is possible to treat distal artery lesions with rather good results.

[Emphysematous cholecystitis].

Emphysematous cholecystitis (EC) is a rare and dramatic disease that requires prompt therapeutic procedures. The authors report a case of a 70-year-old-woman, admitted to the Intensive Care Unit for Infectious Diseases of the Santa Maria Hospital, with the diagnosis of EC. The literature on EC is also reviewed.

[Penicillin G in the treatment of pneumococcal meningitis. Its real importance at the present time].

The authors carried out this work with the purpose both of verifying the present importance of penicillin G for the treatment of pneumococcal meningitis and of comparing it with the clinical effectiveness of other medicines presently available for the same purpose. They have studied 27 patients with clinical and laboratory diagnosis of pneumococcal meningitis and which were admitted in the "Serviço de Infecto-Contagiosos" of Santa Maria Hospital, Lisbon, for the period from 1.1.81 until 31.05.86. Of these patients 18 were males and 9 females, ranging from 8 to 80 years old. All of them were treated exclusively with penicillin G, the medium dose being 400,000 IU/kg per day and for a medium period of 15 days. The final results accounted for: 23 patients healed (85.2%), 2 decreased (7.4%) and the remaining 2 (7.4%) with neurological sequels by the time of discharge from the hospital. These, in our opinion, are values for the minimum equivalent to those in other series of the same pathology and under treatment by the different medicines presently available in alternative. Bearing in mind these results and considering it is more expensive to use these new antibiotics, the authors think they may state that penicillin G keeps ahead for the treatment of pneumococcal meningitis, except for cases of hypersensitivity to this medicine or penicillin-resistance.

["Off pump" coronary surgery and simultaneous carotid endarterectomy].

The occurrence of significant carotid artery disease in patients requiring coronary artery bypass grafting (CABG), results in a dilemma regarding the best surgical management. Our philosophy has been to perform simultaneous carotid endarterectomy and CABG off-pump. We reviewed the efficacy of this therapy in six patients treated in our hospital between January 2001 and December 2002. The result was satisfactory and there was no hospital mortality.
