Long-term survival of patients with Hodgkin's disease. Treatment with cyclophosphamide, vinblastine, procarbazine, and prednisone.

Thirty-eight patients with advanced Hodgkin's disease were treated with a combination of cyclophosphamide, vinblastine, procarbazine, and prednisone (CVPP); the minimum period of observation for surviving patients was five years. Twenty-eight patients (74%) achieved complete remission; in 17 (61%), remissions lasted at least five years. Twenty-five (66%) of the 38 patients survived more than five years from the initiation of CVPP. There were no differences in either rates or duration of response when evaluation was performed for multiple pretreatment clinical features. However, survival was adversely influenced by advanced age, nodular sclerosis histologic subtype, and pretreatment bone marrow involvement. Two patients died, in remission, of overwhelming viral infections, and acute nonlymphocytic leukemia developed in one patient. In another patient, aseptic necrosis of the heads of both femora developed. Our data suggest that treatment with CVPP may result in long-term disease-free survival for patients with advanced Hodgkin's disease.

Inhibition of lymphocyte growth by spermidine in medium containing fetal bovine serum.

The effect of spermidine and fetal bovine serum on DNA, RNA, and protein synthesis in phytohemagglutinin-stimulated human lymphocytes was investigated. At 10(-4) M spermidine, DNA, RNA, and protein synthesis ceased and 70% of the original cell population died within 62 hr. Lower spermidine concentrations had no significant effect on DNA and protein synthesis, but caused an early, unexplained increase in the rate of RNA synthesis. Heating at 56 degrees C for 30 min had no effect on the plasma amine oxidase activity in fetal bovine and horse sera but abolished the activity in human plasma. It is concluded that low amounts of aminoaldehydes and acrolein produced by plasma amine oxidase at spermidine concentrations below 10(-4) M do not noticeably alter lymphocyte metabolism. However, the aminoaldehydes and acrolein produced become abruptly cytotoxic at 10(-4) M spermidine.

Cystic fibrosis serum effect on the short circuit current of rat jejunum.

Sera from 100 subjects were tested for the presence of the cystic fibrosis (CF) "factor" utilizing the short circuit current rat jejunum bioassay. Comparing the mean percent decrease in short circuit current, the presumed normal group differs from the homozygous CF group at P = 0.001 and from the heterozygous group at P = 0.05. The two CF genotypes are statistically different at P = 0.004. There is, however, a large overlap among the three groups, which limits the assay's utility as a guide for factor purification as well as clinical use.

Helping the hypertensive patient to help himself.

The control of blood pressure in patients with hypertension is an important challenge in everyday medical practice. It can do much to reduce complications, but too often we fall short of our therapeutic goals. Much more can be done to help our patients help themselves if physicians, along with their paramedical staffs and their patients, make the effort. The results are gratifying in terms of motivation and successful long-term control of the blood pressure and the risks associated with hypertension. This paper outlines objectives and practical approaches to patient education and self-care of this common medical problem.
