Severe sensorineural hearing loss associated with viral hepatitis.

A 41-year-old man who developed a sudden, profound, irreversible, unilateral hearing loss while acutely ill with viral hepatitis is described. Residual damage consisted of a nonfunctional left cochlea, right positional vertigo, and tinnitus. It is postulated that the pathophysiologic defect was due to direct invasion of the endolymphatic structures by type A hepatitis virus.

Laryngeal microsurgery without intubation.

Reviewed are 200 cases in which endolaryngeal suspension microsurgical procedures were done using intravenous anesthesia and Venturi insufflation oxygenation. Patients included 177 adults and 23 children, ranging in age from 20 months to 88 years. No significant complications were observed. The technic allowed the glottic space to remain free of cumbersome intraluminal apparatus. Arterial blood studies revealed that adequate oxygenation is achieved while normal pH and PaCO2 levels are maintained.

Venturi insufflation oxygenation during nonintubated endolaryngeal microsurgery.

Two hundred patients are reviewed in whom endolaryngeal suspension microsurgical procedures were performed under intravenous anesthesia and Venturie insufflation oxygenation without significant complication. The technique allowed the glottic space to remain free of cumbersome intraluminal apparatus. Arterial blood studies revealed that adequate oxygenation was achieved while normal pH and PCO2 was maintained. It is a technique which should be considered in microsurgical laryngeal procedures.
