The Hamilton SPE Evaluation Tool (HSET): is it any good?

Presents the Hamilton Supervised Pastoral Evaluation Tool (HSET). HSET is a self-report that evaluates student learning in a basic SPE unit utilizing six areas: supervisory relationship, personal growth, professional growth, theological reflection, learning context, and overall growth. Reviews statistics involving seven regional units consisting of 18 SPE units with 101 students. Utilizes methodological, investigator, and data triangulation by drawing on qualitative study and CAPPE accreditation review. Discusses strengths and weaknesses of HSET and makes recommendations for further use.

Building community support for a school dental sealant program.

Children from low-income families experience more dental caries and are less likely to receive dental treatment than children from middle- and high-income families. An overwhelming amount of scientific evidence supports the effectiveness of pit and fissure sealants in deterring the development of dental caries. The authors describe a unique component of the Multnomah County, Oregon, sealant program in which local dentists volunteer to conduct dental screenings in schools. This program, which began in 1989, targets elementary schools with 40% or more of students on free or reduced-cost lunch programs. During the 1999-2000 school year, volunteer dentists screened 5,252 second- and third-grade students in 64 schools and other community programs. As a result of the screenings, dental hygienist/dental assistant teams placed a total of 11,087 dental sealants on 3,866 children (a mean of 2.87 per child). The dentists who conducted the screenings were asked to evaluate the dental sealants placed during the previous school year. The program is one example of how a school dental sealant program can be staffed to optimize the resources of a state or county public health department. An added benefit of this staffing pattern is that it builds linkages in the dental community and a foundation for collaboration between private-practice dentists and public health programs. This type of collaboration is essential to the practice of public health. Furthermore, the involvement of private dentists in a local school sealant program can contribute to the long-term oral health of low-income children.

The effects of denturism on denture prices.

In 1978, the country's first denturism initiative was passed in Oregon, legalizing the independent construction of dentures by non-dentists. The major campaign issue was the effect denturism would have on the cost of dentures. A review of Oregon dental insurance data shows that the costs of dentures, which had been rising at the same rate as other dental services, had a much lower rate of increase after passage of the denturism initiative.

Effects of arterial or portal ischemia on survival and metabolism of partially and totally depancreatized dogs.

The role of arterial blood flow in hepatic metabolic functions was compared to that of portal flow in two groups of totally depancreatized dogs. Survival times and glucose and nitrogen excretion were significantly greater in dogs with ligation of the hepatic artery than in dogs with an Eck fistula. The dogs with ligated hepatic arteries also showed a significantly slower rise in plasma ketones. The course of diabetes was compared in three additional groups of partially depancreatized dogs consisting of a) dogs with ligated hepatic arteries, b) dogs with Eck fistulas, and c) controls. Hepatic arterial ischemia: 1) increased survival, without insulin treatment (a--650, b--167, c--124 days) 2) did not decrease tracer-determined rate of glucose production 3) led to a greater urinary excretion of glucose, ketone bodies and nitrogen than portal ischemia. Partially depancreatized dogs with either arterial or portal hepatic ischemia maintained a high rate of glucose disappearance on acute deprivation of endogenous insulin (clamping of vessels of their pancreatic remnant) due probably to decreased insulin degradation by the ischemic liver. The dogs died in coma after losing all fat depots. There was severe fatty change in the livers of dogs with hepatic artery ligation, slight in those with Eck fistulas and no fat in the livers of controls.
