Weaning of epoprostenol in a small infant receiving concomitant bosentan for severe pulmonary arterial hypertension secondary to bronchopulmonary dysplasia.

Endothelin receptor antagonism is an important therapeutic tool of pulmonary arterial hypertension (PAH). Bosentan was the first orally active, dual antagonist of endothelin receptors in human adults, and has been recently considered for children as well. However, little is known about bosentan treatment in children weighing less than 10 kg. We describe the use of bosentan concomitantly to epoprostenol in an infant weighing 3.5 kg and affected with severe bronchopulmonary dysplasia (BPD) and PAH. At 5 months old, when she presented subsystemic PAH secondary to severe BPD, she was treated with oxygen, digoxin and diuretics. At 8 months old, due to severe PAH not responsive to 100% oxygen, high frequency oscillatory ventilation (HFOV) and nitric oxide (NO), we started epoprostenol and bosentan. Bosentan dose was doubled at 9 months old, when HFOV and NO were slowly discontinued due to improved oxygenation index. Regular echocardiographic measurements of systolic right ventricular pressure were recorded by the method of tricuspidal atrio-ventricular gradient. A four-month combined epoprostenol and bosentan treatment decreased systolic right ventricular pressure from 68% to 40% of the systemic level, till its normalization at 11 months old. Later, when bosentan and epoprostenol were discontinued and sildenafil was started, severe PAH was reported again. Our patient died due to septic shock and refractory hypoxia at 14 months old.

Characterisation of Escherichia coli adhesins in patients with symptomatic urinary tract infections.

The ability of bacteria to adhere to the epithelial cells of hosts has been shown to be mediated by adhesins. Many of these show readily demonstrable haemagglutinating activity. Of 109 Escherichia coli strains isolated from patients with symptomatic urinary tract infection, 11 (10.1%) were identified by their haemagglutinating properties as being P fimbriated, which was confirmed by the latex bead test. Other classes of adhesins, termed X and "other", were found in mannose resistant haemagglutinating E coli strains, which represented 4.6% (5) and 0.9% (1), respectively, of all the strains. Type 1 fimbriae were found in 40.4% (44/109) of E coli strains grown on colonising factor agar (CFA) medium. This incidence was 12.8% higher (53.2%, 58/109) when the strains were grown on CFA supplemented with urea, which suggested that urea may modulate the expression of type 1 fimbriae. Conversely, this phenomenon was not seen in P fimbriated E coli. Assays using trypsinised and non-trypsinised human erythrocytes showed no difference in the percentage of strains that haemagglutinated. Regarding the clinical correlation of fimbriated E coli strains, the X mannose resistant haemagglutinating adhesins were also found to be of clinical relevance. P fimbriated E coli strains were isolated from five out of the eight patients with pyelonephritis.

A new screening test for rotavirus infection.

A simple and rapid staphylococcal coagglutination test, using rabbit antisera prepared against Nebraska calf diarrhea virus (NCDV), is described for the detection of rotavirus in neonatal fecal specimens. When the samples were examined directly using the coagglutination test, more than 60% of the specimens agglutinated the control reagent. These non-specific reactions were markedly reduced by preincubation of the specimens with non-immune rabbit serum and further heating at 80 degrees C for 45 min. Such treatment did not reduce the specific activity in the coagglutination test when rotavirus-containing stools were tested. The coagglutination test was compared with ELISA in 290 stools positive or negative for rotavirus. The sensitivity of the coagglutination test was 92%, the specificity 91% and the predictive value 31%. These results indicate that coagglutination is a suitable test for rapid screening of rotavirus infection in clinical practice.

[Experience with pediatric cardiology in a regional hospital].

Since 1986 we have studied the occurrence of various congenital heart diseases in our neonatal population. 43 (9.7%) newborn infants were admitted in our Pediatric Department by cardiovascular involvement. We have also used echocardiography as diagnostic tool to reach clinical information.
