[The slope clearance method in renal functional diagnosis. J-131-hippurate clearance is used as an example].

The clinical relevance of the clearance of radiopharmaca is controversial in literature. The cause for this are in our opinion the different methods of evaluation which either make difficult or make impossible a comparison of individual results. The late phase of the disappearance (at least 70 and 100 min p.i.) possesses a high value under the aspect of renal diagnosis, since by its means with a high approximation the renal function may be established itself.

[Biological modelling of potassium metabolism].

1. General principles of the modelling of compartments were described with the aim to use this methodology for the investigation of the potassium metabolism. 2. For the modelling of the potassium metabolism the necessary processes of investigation and calculations in the analysis of 42-potassium-kinetics were described in detail at an example. 3. By analogy references to the haemodynamics the general significance of the modelling of compartments was explained.

[Complex genesis of anemia in chronic liver diseases].

36% of a total of chronic liver patients suffered from anaemia and 50.5% of patients affected with liver cirrhosis. In most cases the anaemias were normochrome and hypochrome or hyperchrome only in some cases. In analyzing possible single factors the reductions of vitamin B12 absorption could be made probable by means of the Schilling test and sometimes a folic acid deficiency in macrocyte anaemia with normal vitamin B12 absorption by determining the folic acid content in the serum and by successes of test treatment 82% of patients with liver cirrhosis showed a latent or manifest haemolysis. However, it was only in 1/3 of the patients with liver cirrhosis that the spleen turned out to be the place of an increased degradation of erythrocytes. In some cases an increased erythrocytoclasia into the liver could be identified. Predominantly, however, an increased degradation of erythrocytes in the total RHS had to be assumed. Twice an ineffective erythropoiesis could be found by ferrokinetic examinations. As a whole ferrokinetic examinations cannot be interpreted easily, because their static and dynamic values of iron transport in the plasma volume of liver patients will undergo considerable changes. Patients with disturbances of haematopoiesis and with haemolysis remaining in the latent stage may develop a manifest anaemia because of the influence of additional factors, such as increase of the plasma volume at lowered haematocrit value or microbleedings. The cause of anaemia cannot be concluded with sufficient probability from the type of anaemia; in a single case all pathogenetic factors will rather have to be analyzed. Therapeutic possibilities for hepatogenous anaemia of complex genesis are discussed.

[Information value of the compartimental analysis of potassium metabolism for clinical medicine].

1. The special information value of the dynamic compartment analysis consists in functional evidence concerning the potassium metabolism. 2. Basis of the dynamic compartment analysis is an open central 3 compartment model. 3. The rate of metabolism which is determined by size of the pool and constant of speed is the most important parameter. 4. The mathematical modelling of the tracer course renders visible the compartmental effects of inflow and outflow of the speed constant. 5. Disturbances of the very quickly and quickly exchanging constituents of the potassium metabolism reflect themselves perhaps directly as a functional loss of the permanent and latent functional capacity of the organism.

[The spontaneous mediastinal and skin emphysema].

A report is given on four patients with spontaneous mediastinal and skin emphysema. In three cases younger patients from 14 to 21 years were concerned, in the fourth case a 63-year-old patient was concerned. Two of the younger patients revealed a long lasting malnutrition. As favouring or triggering factor, respectively, for the pathogenesis of the mediastinal emphysema in accordance with Macklin an increase of the intrathoracic pressure by retching, pressing or vomiting is regarded. Apart from this a vasoconstriction of the alveolar vessels (due to decrease of the plasma volume) and nutritional disease might be of importance. In all patients the mediastinal and skin emphysema receded under symptomatic therapeutic measures within some days.
