[Biliary ileus].

This is a report on nine cases presenting biliary ileus. All patients undergo operative management. Two female patients present Bouveret's syndrome. The inference is reached that gallstone induced intestinal obstruction is characterized by atypical clinical patterns, conditioning in turn the diagnostic difficulties encountered, and the necessity to undertake operative treatment against the background of advanced ileus. The latter is one of the rarest forms of intestinal obstruction. Biliary ileus treatment consists in the removal of small intestinal impatency. The concretion is pushed in oral direction, and its evacuation is successfully accomplished by enterotomy, duodenotomy or gastrotomy. Two fatal outcomes due to severe advanced form of ileus are reported.

[Biliary tract carcinomas. II. Biliary tract cancer].

The authors discuss the biliary tract carcinoma in the second paper on biliary tract carcinomas. It ranks second as regards frequency, being responsible for 4 per cent of the biliary operations. The analysis is based on 48 observations during the period 1952-1979 at the Chair of Abdominal Surgery - 31 males and 17 females (2:1). Morbidity rate is the highest among the patients aged from 40 to 60. The patients are admitted for operation up to the 1-2 months since the beginning of their complaints, mostly due to the lasting jaundice, present in all the patients (100%). Regardless of the application of the present methods, biliary ducts carcinoma remains a serious diagnostic and surgical problem. Those methods, however, are the only possible ones, for an earlier detection of the tumors, hence, for the improvement of the unsatisfactory, so far, operability, lethality and survival.

[Biliary tract carcinomas. I. Gallbladder cancer].

In three successive papers the clinical, diagnostic and surgical possibilities of biliary tract carcinoma are discussed on the base of 119 patients, undergone operation at the Chair of Abdominal Surgery for the period 1952-1979: 52 (44%) with gallbladder carcinoma, 48 (40% with biliary ducts carcinoma and 19 (16%) with duodenal papilla carcinoma. The authors report that gallbladder carcinoma is most frequent - 4.4 per cent of all biliary operations. The ratio males: females is 1:3 and the highest number of the patients is over the age of 60. The diagnosis is usually made with 6 months later due to the atypical clinical picture, progressing under the mask of cholelithiasis, found in 70-100 per cent of the cases. That is the reason in 5 per cent of the cases that carcinoma involved the whole gallbladder, metastases and infiltration of the adjacent organs being frequent. Resectable proved to be only 9 patients, the mortality rate kept high. A complex diagnostics is recommended with present-day methods and early prophylactic cholecystectomy in case of calculosis.
