Low grade basaloid adenocarcinoma of salivary gland in childhood--the so-called hybrid basal cell adenoma--adenoid cystic carcinoma.

Two parotid gland tumors in young children are described. These tumors superficially resembled monomorphic adenomas but were unencapsulated and showed pushing invasion of parotid gland and adjacent adipose tissue. The patients remained well with no recurrence 8 and 13 years after surgical removal. Among 10 similar cases in the literature, 4 demonstrated local invasion or recurrence and 1 had nodal metastasis, but there were no tumor related deaths.

Adenocarcinoma

Primary muco-epidermoid carcinoma arising in a parotid lymph node.

A case of well-differentiated muco-epidermoid carcinoma in a parotid lymph node is described. This tumour apparently arose within and was confined to the lymph node. Salivary ductal and acinar inclusions are often found in parotid lymph nodes and these may occasionally give rise to salivary gland tumours, both benign and malignant. The finding of a malignant salivary gland tumour within such a site does not necessarily imply a metastasis from a primary neoplasm elsewhere in the parotid gland.

Aged

Differentiated thyroid carcinoma, intermediate type.

A thyroid carcinoma showing histological and histochemical features of both follicular and parafollicular cell carcinoma was diagnosed in a 56 year old male. Previously the two cell types have been considered to be derived from embryologically distinct and separate precursors. The unusual combination of the two cell types in this tumour suggests the possibility that the cell lines may be derived from a common stem cell with the potential of differentiating into both follicular and parafollicular cells. The tumour was aggressively invasive and had metastasized widely to lymph nodes at the time of presentation. It represents an example of the recently described entity of differentiated thyroid carcinoma of intermediate type.

Adenocarcinoma

Left coronary artery dissection: an unusual presentation.

Primary dissecting aneurysm of the left main stem coronary artery is a rare cause of coronary arterial occlusion that usually presents as sudden death or myocardial infarction in a young woman. A 32 year old woman with no signs of cardiac disease who died as the result of a right cerebral infarction was found to have coronary dissection and myocardial infarction at necropsy.
