Use of a temporary heparin-coated ventriculofemoral shunt in 2 patients in the successful management of traumatic aneurysms of the descending thoracic aorta is described. Safe and effective bypass protection can be achieved by direct ventriculofemoral diversion when cannulation of the left subclavian artery or ascending aorta is hazardous. The use of a heparinized shunt bypass without systemic anticoagulation considerably simplifies the operation.
A four-year clinical evaluation of 133 patients with Lillehei-Kaster pivoting-disc valves has shown that patients with an aortic valve prosthesis have remained free from valve-related problems. Patients with a mitral valve implanted have a satisfactory survival record as calculated by actuarial methods; however, a 10% incidence of valve thrombosis was experienced. Analysis suggests that this may be related to inadequate anticoagulation or use of an inappropriate suturing technique or both. The patients enjoyed significant clinical improvement following operation, with no evidence of hemolysis. A history suggestive of postoperative embolization was present in only 1 patient in this series.
Forty-five patients with Lillehei-Kaster pivoting disc valves had intraoperative assessment of valve hemodynamic function by simultaneous recording of aortic, left ventricular, and left atrial pressures and cardiac output. Mean transvalve gradients and prosthetic valve areas were calculated and recorded. Comparison of the results with data previously reported on the caged-ball valves indicates that the Lillehei-Kaster pivoting disc valve has superior hemodynamic characteristics.
The unusual occurrence of an intramural hamartoma mimicking a peptic esophageal stricture in a child is presented. Early operative intervention is indicated to diagnose the condition and to avoid certain morbidity or death. The remarkable embryologic feature of this report is the presence of annular cartilage at the site of the esophageal stricture. Conservative resection of the involved esophagus and primary anastomosis is the treatment of choice.
Mitral regurgitation associated with secundum atrial septal defect is described in 4 patients, each with a different mitral lesion: rheumatic valvular disease, congenitally cleft valve, subacute bacterial endocarditis with disruption of the chordae tendineae, and traumatic valve rupture. The pathological spectrum of mitral valve disease associated with atrial septal defect is reviewed, and it is suggested that structural abnormality of the mitral valve may accompany the atrial septal defect. More general awareness of this association will allow the surgeon more accuracy in defining and repairing this rather unusual combination of lesions.

Erythrocyte filterability, an index of deformability, and lysosomal enzyme activity were studied in 34 patients requiring cardiopulmonary bypass. In addition fresh human erythrocytes were analyzed after incubation for filterability changes after exogenous lysosomal enzymes were added to the suspension medium. The results showed statistically significant elevations of the filterability index (decrease in filterability of the red cells) after 10 minutes on cardiopulmonary bypass as well as postoperatively. Likewise cathepsin D concentration after 10 minutes of cardiopulmonary bypass of 8.4 plus or minus 0.8 U. was significantly elevated over the preoperative level of 5.3 plus or minus 0.7 U. Furthermore, the pump prime was found to have the highest filterability index as well as concentration of cathepsin D. Supporting our hypothesis that lysosomal enzymes may be a factor affecting erythrocyte integrity was the elevation of the filtration index to 10.5 plus or minus 0.3 U. during incubation with exogenous lysosomal enzymes compared with the control index of 9.0 plus or minus 0.2 U. This stduy implies that the pump prime should be an area of further investigation if alterations of erythrocyte filterability (deformability) and lysosomal enzymes during extracorporeal circulation are shown to have clinical importance.

Coarctation of the aorta may be found proximal to or involving the left subclavian artery. In this situation, adequate collateral may not be present, making aortic occlusion unduly dangerous. Simple and effective bypass protection can be achieved with a heparin-coated temporary shunt as illustrated in two patients described in this report.
