[Diagnosis and prognosis of cerebral ischemic disturbances course using a method of artificial neuronal networks].

Based on the data of examination of 224 patients with different stages of cerebral ischemic disturbances (CID) and 84 age-matched controls, an artificial neuronal network was constructed and tried in differential diagnosis of CID stages according to the data of transcranial ultrasonic dopplerography. Diagnostic efficacy of the network was 80% for sensitivity, 100% for specificity and 82.7% for reliability. A modeling of the influence of the main risk factors for cerebral ischemia and of the reserve state of cerebral hemodynamics for establishing the stage of CID was performed.

[Optimization of cardiovascular function by peptide bio-regulators].

Effects of peptide bioregulators (thymaline, epithalamine, cortexine) on systemic hemodynamics in cardiovascular and cerebrovascular affections were studied. Assessment of functional stability and quality of cardiovascular system regulation was made according to methodological principles of automatic regulation theory. Peptide bioregulators showed high effectiveness in the above patients who markedly improved their hemodynamic parameters.

[Patterns in the change of the evoked skin autonomic potential in nervous system diseases].

The study of sympathetic skin response (SSR) was performed both in 30 healthy volunteers either in rest or after pharmacological preparations administration (reserpine, sydnocarb, gammalone) and in 48 patients with autonomic disfunction, syringomyelia, polyneuropathy. It was determined that two processes were presented in SSR structure, exactly: the first one which included I and III phases and was bound with trophotropic functions and the second process which was presented by II phase only and reflected the activity of ergotropic functions. The latent period of SSR reflected the tonus of the sympathetic nervous system. It was established that SSR changed regularly in the nervous systems diseases studied: thus, increase or decrease of the latent period as well as of the amplitudes of SSR phases were observed in autonomic disfunction while depression or absence of SSR took place in syringomyelia or polyneuropathies. The data obtained testified the results of the other authors about possibility of SSR usage for diagnosis of both central and peripheric autonomic disturbances.

[Cerebrovascular reactivity in the pathogenesis of ischemic brain lesions in patients of different ages].

Cerebrovascular indices (cerebrovascular reactivity, plethora of pulse, the degree of atherosclerotic damage of carotids) were evaluated in 445 patients with either transient disturbances of brain's circulation or ischemic stroke. The patients' age was 32-85 years. It was found that the alterations in the system of neurogenic regulation which were the consequences of some pathogenic factors' influence (infectious diseases, closed brain's trauma, latent intoxication), resulted in some cases in formation of lower level of cerebrovascular reactivity which in turn was one of the reasons of ischemic cerebral disorder development. Further influence of such factors as atherosclerosis, increase of blood coagulability, decrease of total arterial pressure may result in alterations in cerebrovascular system's stability and in development of cerebrovascular diseases at younger age.

[Autonomic paroxysms: their pathogenesis, diagnosis and treatment].

In the article the rules about etiology and pathogenesis of vegetative paroxysms are stated based on careful analysis of the publications of researches as Russian, so foreign authors, and also own experimental and clinical supervision. During experimental and clinical researches the modern methods were used, enabling to estimate from positions of the system analysis different parts of pathogenesis of vegetative paroxysms, and also to offer ways of differential diagnostics of the various forms of disease. The application of some new preparations and direction of therapy of vegetative paroxysms are substantiated, and also the various circuits of treatment of the patients with distinguishing forms of given pathology are motivated.

[A clinico-instrumental evaluation of cerebral hemodynamics in patients with occlusive lesions of the carotid arteries (its diagnostic and prognostic importance)].

Ultrasonic dopplerography and rheoencephalography were employed to investigate cerebral circulation in patients (n-123) with carotid artery atherosclerosis having transient disturbances of cerebral circulation or ischemic apoplexy. The ischemic foci were measured and localized at computed and NMR tomography. The above complex of examinations allowed evaluation of local, regional and overall blood filling of the brain, of collateral flow as well as conclusion on the type of cerebral hemodynamics. Focal lesions seem to arise in inadequate functioning of collateral circulation, that is in patients with unstable and decompensated hemodynamics. Active collateral circulation shown by the absence of noticeable interhemispheric asymmetry proved prognostically favourable. Administration of vasoactive drugs to patients with decompensated cerebral hemodynamics may induce stealing syndrome and aggravation of neurological symptoms.

[Cerebral evoked potentials in comatose states of various etiology].

The authors studied the evoked potentials (EPs) of the brain cortex in patients in comatose states of various etiology as well as the EPs of the brain cortical and subcortical structures of the animals during simulation of the conditions studied in clinical practice. A sharp drop in the amplitude of early waves and an increase in the amplitude and latency of late EP waves in all the cerebral structures irrespective of the coma genesis were disclosed. The changes were of a simultaneous and generalized character with a greater similarity between the EPs of the cortical zones but an increase of their difference from the subcortical structures. These facts are explained by the deprivation of the nonspecific systems of both cortical and local inhibitory influence which leads to the late activation of the brain specific systems. It is assumed that the space-time dissociation in the action of various cerebral systems is, regardless of the coma genesis, one of the principal mechanisms giving rise to disorders of information perception, analysis and processing.
