Verrucous carcinoma with a dorsal sinus tract.

A verrucous carcinoma of the right foot was presented. A unique finding, a dorsal sinus tract at the base of the fourth toe, was found on examination, which communicated with the tumor. Other reports of a sinus tract extending to the dorsal surface of the foot could not be found in the literature.

Fracture blisters.

Fracture blisters can pose a problem when dealing with trauma. The potential to increase the complication rate, delay surgery, and prolong wound healing are possible sequelae. Edema has to be controlled, however, because the blisters can form in spite of ice, elevation, and compression. They may also appear even after prompt ORIF. High-energy trauma predisposes one to develop fracture blisters. Blisters can be discovered even after closed reduction and at a subsequent cast change. As with all pathologic conditions, the index of suspicion has to be high (Fig. 7).

Complications of foot surgery. Complications of anesthesia.

Anesthesia is a necessity when performing surgery. As with any component of the surgical task, there are complications associated with any of the various anesthetic agents and techniques. The most effective way to diminish a complication is through prevention. A detailed medical history and examination of the patient will alert the physician to possible hazards along the surgical path. Dialogue with the anesthesiologist to promote teamwork is always in the patient's best interest and can lower the complication rate.

Role of leukotriene C4 in pulmonary hypertension: platelet-activating factor vs. hypoxia.

The aim of this study was to determine whether leukotriene C4 (LTC4) is a mediator of hypoxic pulmonary vasoconstriction. We hypothesized that similar increases in LTC4, detected in the lung parenchyma and pulmonary vascular compartment during cyclooxygenase blockade with indomethacin (INDO), would be observed during an equal increase in pulmonary arterial pressure caused by acute alveolar hypoxia (HYP, 100% N2) or platelet-activating factor (PAF, 10 micrograms into the pulmonary artery). Rat lungs were perfused at constant flow in vitro with an albumin-Krebs-Henseleit solution. Mean pulmonary arterial pressure (n = 6 per group) increased from a base line of 10.9 +/- 1.2 to 15.8 +/- 2.1 (HYP + INDO) and 15.5 +/- 1.9 (SE) Torr (PAF + INDO). LTC4 levels increased only in response to PAF + INDO; perfusate levels increased from 0.4 +/- 0.07 to 5.3 +/- 1.1 ng/40 ml, and lung parenchymal levels increased from 1.9 +/- 0.07 to 22.8 +/- 5.3 ng/lung. Diethylcarbamazine (lipoxygenase inhibitor) reduced PAF-induced lung parenchymal levels of LTC4 by 68% and pulmonary hypertension by 63%. We conclude that 1) LTC4 is not a mediator of hypoxic pulmonary vasoconstriction and 2) intravascular PAF is a potent stimulus for LTC4 production in the lung parenchyma.

Treatment of open fractures of the foot and ankle: a preliminary report.

A retrospective study was performed on 33 patients who sustained isolated open-fracture injuries to the foot and ankle. A wide variety of open-fracture injuries of the foot and ankle are documented. Of the 33 cases, only 2 became clinically infected, translating into an infection rate of 6.06%. A literature review of open-fracture injuries is presented along with the preliminary results of the retrospective study.
