Myoencephalopathies with abnormal mitochondria: a review.

Myoencephalopathies with abnormal mitochondria comprise a heterogeneous group of diseases and syndromes with a large diversity of clinical signs. Thus, their identification and classification pose many difficulties. The underlying biochemical disorders of energy metabolism evoke non-specific structural alterations of the mitochondria in skeletal muscle and brain, but also in other organs. In this paper the morphologic and biochemical background of confirmed cases is reviewed and the position of these cases within the concept of mitochondriopathy is discussed.

Adult↗

[Molecular pathological aspects in spongy encephalopathies].

Spongy encephalopathies are rare diseases of infancy and childhood. Apart from cerebral spongiosis in the forms described as Canavan's disease and the Kearns-Sayre syndrome, necrosis in the cerebral tissue is also found in Alpers' disease and Leigh's syndrome. The common pathogenetic factor may be sought in the sphere of mitochondrial energy metabolism, as suggested by the results of molecular pathological investigations. Biochemical enzyme deficiencies in energy metabolism obviously provoke structural alterations of mitochondria.

CNS cysticercosis--a problem of differential diagnosis.

Although a well-known disease worldwide, cerebral cysticercosis in European countries is a rare event. Due to increased travel to East-Asian and Latin American countries it is possible that the occurrence of brain cysticercosis will become more frequent. These facts should remind us of the possibility of cysticercosis of the nervous system. A case of cerebral and spinal cord cysticercosis is presented, causing symptoms of progressive dementia with ventricular occlusion and hydrocephalus. Differential diagnosis causes a major problem. It is shown that myelography may demonstrate characteristic features of the disease.

Ultrastructural study of medullomyoblastoma.

A case of cerebellar medullomyoblastoma in a young boy was investigated by electron microscopy. The neuroectodermal component shows the characteristics of a desmoplastic medulloblastoma. The mesodermal component consists of more or less differentiated cross-striated muscle cells. Undifferentiated muscle cells are very similar to proliferated endothelial cells of blood vessels within the muscular component, so that an origin of this component from pluripotential endothelial cells of the vessel wall is suggested. This tumor is considered a malignant teratoid because of the derivation from two blastodermic layers and because of the midline localization in children suggesting a malformative origin.

Cerebellar Neoplasms↗

Shunt metastases in posterior fossa tumors.

Permanent (CSF-drainages for treatment of hydrocephalus in posterior fossa tumors have been used in the last few years, increasingly. The rare incidence of extracranial metastases and it's higher frequency in the same period of time has been remarkable. An additional case of metastasis via ventriculoperitoneal shunt is presented. Such deleterious complications of malignant CNS-tumors should be avoided, as progressively improving results with combined radio-chemotherapy have established a more encouraging prognosis.

Abdominal Neoplasms↗

Histochemical differentiation of endocrine gastrointestinal cells in test animals, with special reference to chicken embryos.

Based upon the characteristics of argent-affinity and argyrophilia and the application of specific immune histochemical methods, endocrine cells can be differentiated in the most prominent species of test animals. These methods are demonstrated in chicken embryos, because embryological studies made in order to reveal the still unknown endodermal or neuroectodermal origin of these cells are highly important.

Disseminated endocrine cells of the gastrointestinal tract and their possible influence on gastrointestinal disease.

The role of the disseminated endocrine cells of the gastrointestinal tract is reviewed. Some of these cells are scarce or absent in pernicious anemia, ulcerative colitis and Sheehan's syndrome, but are increased in number in peptic ulceration, chronic gastritis and celiac disease. A subgroup of the diffuse endocrine cells with both endocrine and exocrine functions, the so-called amphicrine cells, suggests that these cells originate from the entoderm. A study of the diffuse endocrine cells in the chronic intestinal diseases of the developing countries is suggested.

Appendiceal Neoplasms↗

[Cerebellar astrocytoma and optic glioma--a comparative ultrastructural study (author's transl)].

An ultrastructural comparison of 8 cerebellar astrocytomas and 4 optic gliomas shows that the morphological patterns of both tumor-groups are identical. The confusion in nomenclature of these tumors is discussed and reasons for a preference for the term "pilocytic astrocytomas" are given. Further, pilocytic astrocytes seem to be a special, phylogenetically older, group of glial cells.

Marchiafava-Bignami disease. First case in germany.

The first case of Marchiafava-Bignami disease in a German is reported. The female patient was a heavy drinker and died after a fortnight of coma. Necropsy showed a typical degeneration of the corpus callosum and of the subcortical white matter of the right cerebral hemisphere. Furthermore, this case is the fourth of the cases of Marchiafava-Bignami disease in a woman described up to now.

[Extraneural metastasis of an oligodendroglioma in ventriculo-peritoneal shunt].

The clinical and pathological report presents a seven-week old girl suffering from signs of raised ICP. A ventriculo-atrial shunt was inserted and replaced by a ventriculo-abdominal shunt due to occlusion at 15 months of age. At 16 months, craniotomy and resection of an oligodendroglioma was performed and the shunt was preserved. The child died at 18 months due to extensive local recurrence. Autopsy revealed massive peritoneal metastases of the tumor. This is the eleventh published case of canalicular metastases of glioma.

[Development and function of endocrine cells in the proventriculus of the chicken (author's transl)].

An argyrophil endocrine cell type with typical intraepithelial development is seen initially on the 8th day of incubation in the epithelium of the main lumen of the chicken-proventriculus. During the embryonic period, rapid development of these cells can be observed with a quantitative maximum on the 13th day of incubation. At this time increased digestive efficiency is necessary for the chicken embryo with the start of gastric secretion combined with the first swallowing of albumen. There is a subsequent decrease in the number of endocrine cells in the main lumen epithelium, their function in hatched or adult specimens is largely taken over by cells which appear in the epithelium of the glands of the proventriculus on the 16th and later days of incubation. On the 2nd day after hatching the adult distribution pattern of endocrine cells in the proventriculus is attained.

Age Factors↗

The influences of methylphenidate on heart rate and behavioral measures of attention in hyperactive children.

Reaction-time performance and heart-rate responses associated with attention were used to assess the hyperactive child's attentional deficit and his response to methylphenidate. Attentional deficits shown by long response latencies were reflected in heart-rate responses theoretically incompatible with sustained attention. Subjects exhibiting the greatest attentional deficit displayed the most favorable response to methylphenidate in both reaction-time performance and physiological measures. However, subjects who showed the greatest improvement in social behavior were those who showed the least improvement in reaction-time performance.
