Prostatic carcinoma and benign prostatic hyperplasia: MR imaging with histopathologic correlation.

Twenty-eight patients with prostatic carcinoma who underwent magnetic resonance (MR) imaging and subsequent radical prostatectomy were studied. The resected prostates were sectioned axially, and the whole-mount prostatic specimen was compared with the corresponding MR images. The carcinoma could be identified in 20 cases (71%), but the tumor volume was underestimated in 12 patients (43%). In 19 of 20 visualized cases (95%), the carcinoma was seen as an area of low signal intensity within the peripheral zone on T2-weighted images. However, in one case the signal intensity of the carcinoma was higher than that of the remainder of the peripheral zone. All carcinomas were located within the peripheral zone. The accuracy of MR imaging in the prediction of extraglandular tumor spread was 82%, with a sensitivity of 37.5% and a specificity of 100%. Nineteen patients (67%) exhibited histologically benign hyperplasia, which could be identified on MR images in 10 cases (53%). The signal intensities of the nodules varied. All areas of benign hyperplasia were located within the central zone. The location of the pathologic changes must therefore be considered in differential diagnosis.

Chemical and immunochemical characterization of polymers of aggregates in preparations of human serum albumin.

The polymer/aggregate fractions of three human serum albumin preparations (HSA) from different manufacturers were isolated by size-exclusion high-performance liquid chromatography (HPLC) and analyzed by different electrophoretic and immunochemical methods. All of these polymer/aggregate fractions contained only 30-50% albumin. The rest seemed to be mainly heat-denatured haptoglobin which did not react with an anti-haptoglobin serum, whereas the albumin part reacted with an anti-albumin serum. It could be shown that these polymers or aggregates are formed by disulfide bonds between albumin and the small amounts of denatured impurity globulins (haptoglobin, transferrin) during the pasteurization step. The higher the amount of these heat labile globulins in the final preparation before the pasteurization step, the higher was the polymer/aggregate content of the pasteurized albumin preparation.

[Magnetic resonance tomography of prostatic cancer. A histopathologic correlation].

A total of 52 patients with prostatic carcinoma were examined by MR imaging; 14 of them subsequently underwent radical prostatectomy and were included in this study. The resected prostates were sectioned axially and compared with the corresponding MR scans with special reference to the location and extent of the pathological changes. The prostatic carcinoma was visualized in 10 cases, but the size of the tumor was underestimated in 6 patients. In 9 of 10 patients the carcinoma was identifiable as an area of low signal intensity within the peripheral zone on T2-weighted images. The MR features of benign nodular hyperplasia are discussed.

[Combined radiotherapy with cis- or carboplatin in advanced head and neck tumors].

This report reviews the treatment results of 111 patients with stage T3-4, N0-3, M0, biopsy proven squamous cell carcinoma of the oropharynx and oral cavity. All patients were treated by primary irradiation with 1.8 to 2 Gy per day for five days a week up to a target volume dose of 39.6 or 40 Gy. Simultaneously 20 mg/m2 cisplatin was given under hyperhydration and mannitol diuresis on days 1 to 5. In case of partial tumour regression radiotherapy was continued up to 70 Gy with another course of cisplatin. In case of minor response surgery was interposed followed by subsequent irradiation with 30 Gy and a second course of cisplatin. 67% of the patients showed an initial complete tumour involution and 27% a partial response. The five year actuarial survival rate with a minimum follow-up of two years is 47.6%. More than 96% of the long term survivors showed a complete response after the end of treatment. The results demonstrate that concomitant radiation and chemotherapy with cisplatin is a highly effective treatment in advanced head and neck carcinoma with tolerable toxicity. Carboplatin (CBDCA) is a second generation platinum analog and has shown comparable antitumour activity but less nephro- and neurotoxicity than cisplatin in head and neck cancer. In order to determine the feasibility and efficacy of simultaneous application of CBDCA and radiotherapy a phase I-II study is going on. Between September 1987 and March 1988 30 patients with advanced squamous carcinoma of the head and neck (T3-4, N0-3) have entered this trial. Patients were separated into three groups which received 60 mg/m2. 70 mg/m2 and 80 mg/m2 CBDCA from days 1 to 5 and 28 to 32. Radiotherapy was administered up to a target absorbed dose of 70 Gy. 5 X 2 Gy/week in shrinking field technique. The group which received 80 mg/m2 CBDCA reached the myelotoxicity limit so that subsequent patients were treated with 70 mg/m2. Among 30 patients who completed the treatment, 22 showed a complete (CR) and eight a partial remission (PR). Despite the short follow-up period the preliminary results appear to be comparable with those achieved after combined radiotherapy and cis-DDP application without any limitations for patients with renal or cardiovascular disorders.

[Use of contrast media in MR tomography of soft tissue sarcomas. Follow-up studies after multimodal therapy].

The value of MRI was evaluated retrospectively in 32 patients with clinically confirmed soft tissue sarcomata. The results were compared with the clinical findings and with computed tomographic scans. All patients were examined for recurrent residual tumor, the majority after surgery, radiotherapy and/or chemotherapy. Regardless of the tumor site, MR was superior to CT or at least equivalent. The normal postoperative anatomy was well delineated. Discrimination of recurrent or residual masses from post-treatment fibrosis was good. Bone destruction, especially tumor-induced cortical erosion, was better defined with CT. MRI is an appealing modality for the diagnostic work-up and for the follow-up following treatment of thoracic and abdominal sarcomata in children. The use of Gd-DTPA meant T2-weighted image sequences of the chest and abdomen were unnecessary.

[The use of contrast media in magnetic resonance tomography of head and neck tumors].

The ranking of MRT was evaluated retrospectively in 37 patients with clinically confirmed malignant neoplasms of the head and neck. The results were compared with the clinical and endoscopic findings and with computed tomographic scans (CT). MR consistently showed superior soft tissue definition and extent of disease compared with CT. However, the sensitivity of both methods was comparably high. With the exception of tumours of the floor of the mouth and the anterior third of tongue the use of paramagnetic contrast material improved image quality compared with T2-sequences. MRT was superior to CT in imaging the post-treatment head and neck. Even here, contrast enhanced T1-images improved contrast between recurrent tumour and adjacent soft tissues, although in some cases false positive diagnosis was caused by postradiation oedema and inflammatory changes.

[Results of whole lung irradiation and chemotherapy in comparison with partial lung irradiation in metastasizing, undifferentiated soft tissue sarcomas].

The poor prognosis of patients with unresectable pulmonary metastases of soft tissue sarcoma is well known. In order to evaluate the beneficial effect of radiotherapy, we have treated 44 patients with pulmonary metastases of grade 3 soft tissue sarcoma from 1980 to 1986. In 36 patients the treatment volume was restricted to the single metastases up to a dose of 50 to 60 (9 to 10 Gy/week). The survival rate at one year was 18% and at two years 6%. Eight patients were treated with a combined regimen, consisting of cisplatin and ifosfamide with simultaneous whole lung irradiation. Irradiation was performed with 8 or 16 MV photons at a hyperfractionation of 2 x 0.8 Gy/day (8 Gy/week). After a dose of 12 Gy, the single metastases were boosted up to 50 to 60 Gy, with a second course of chemotherapy. In six of eight patients complete remissions were achieved, one patient showed a partial remission. The survival rate at 27 months was 50%. The patients with partial remission died from pulmonary progression at 23 months. One patient died after twelve months from a loco-regional recurrence in the tonsillar fossa without evidence of pulmonary disease. Side effects included alopecia and moderate bone marrow suppression approximately twelve days after each chemotherapy cycle. Pulmonary fibrosis was observed only at the high dose volume without impairment of respiratory function. From these observations the conclusion is drawn that whole lung irradiation simultaneously with cisplatin and ifosfamide chemotherapy provides good palliative results without relevant morbidity in patients with high grade unresectable pulmonary metastases of soft tissue sarcomas.

[Determination of central hemodynamics in dynamic stress using impedance cardiography in comparison to radiocardiography].

77 unselected patients were examined synchronously by impedance and radiocardiography in recumbent position during dynamic exercise. In all cases the load of evaluable impedance curves was possible till 100 watts. Stroke volume, estimated by impedance cardiography, is lower about 30% at rest and during exercise compared to its estimation by radiocardiography. But the percentual increase during exercise is not significantly different between the methods. The stroke volumes, estimated by both methods, correlate significantly at rest and during exercise. This fact shows the usefulness of impedance cardiography for estimating central haemodynamics during dynamic exercise.

The value of neutron- and neutron-boost irradiation for the local control of advanced soft tissue sarcomas.

An analysis of neutron- and neutron-boost irradiation of 191 patients with soft tissue sarcomas is given. 101 patients had T3 (IVA) tumours. The actuarial disease free survival rate of all stages at 6.5 years is 59.8%. Patients with T1 and T2 tumours fared significantly better (73.5%) than patients with T3 tumours (31.3%, p = 0.016). Similar difference are seen according to grade, i.e. 75.1% for G1, 48.3% for G2 (p = 0.035) and 30.9% for G3 tumours (p = 0.024). Patients without microscopic tumour residue fared best (84%), followed by patients with microscopic tumour (72.3%) and patients with gross tumour (29.8%) left behind (p = 0.0003). A functional limb was preserved in 80.5% of patients with extremity lesions (95/118), 50 of them had T3-lesions. The highest local control rate of 76.5% was seen after neutron therapy of differentiated gross tumours. This finding underlines the biological advantage of densely ionizing radiation qualities in these tumours.

[Comparative studies of contractile heart function using impedance, echo-, radio- and apex cardiography].

89 patients (31 healthy subjects, 31 hypertensives, and 27 patients with coronary heart disease) were examined simultaneously by means of impedance (ICG)-, ultrasonic (UCG)-, radio (RCG)-, and calib. and diff. apex cardiography (cACG). There were found significant correlations between the ICG-, UCG-, and RCG-parameters, all describing the heart function in the auxotonic ejection phase. Whereas there were no significant correlations between the methods mentioned and the cACG. Of all investigated parameters in hypertensives only the quotient A/S of cACG is significantly increased compared with persons with healthy heart and circulation. In patients with coronary heart disease all the parameters of the auxotonic ejection phase are significantly decreased, the isometric contraction time and the quotient A/S are increased.

[Stability of blood coagulation factors in plasma frozen 6 and 18 hours after blood collection].

In 50 plasma samples flash frozen at -40 degrees C within 6 h and in 50 samples flash frozen within 18 h after phlebotomy, both coagulation factors II, V, VII, VIII, IX and X as well as AT III, APTT and Quick were measured. Only factor VIII in 18-h plasma showed an average decrease from 0,99 IU/ml to 0,85 IU/ml, i. e. 15% compared to 6-h plasma. None of the other coagulation factors or parameters exhibited any change beyond normal ranges. Furthermore, 150 6-h and 150 18-h plasma samples were taken from routine production and after thawing were tested for factor VIII- and factor-V activity. In a long-term study following a one year's storage at -30 degrees C of 30 samples of 6-h plasma and 30 samples of 18-h plasma, a decrease was found with factor VIII from 0,97 IU/ml to 0,93 IU/ml in 6 h-plasma and from 0,90 IU/ml to 0,86 IU/ml in 18 h-plasma. Our results demonstrate that only minor differences exist in activity of coagulation factors of 6-h and 18-h plasma. Hence follows that by observing the instructions described, freezing must take place within an 18-h period after phlebotomy which thus allows 12 more hours for production without losses in activity.
