[Digestive diseases in the elderly].

This study was carried out in two phases. First a survey was done, interviewing 57 apparently healthy persons, who were older than 60 years. Information was collected about previous illnesses and any digestive symptoms. Then a prospective study was developed in 43 out patients seen previously in a gastroenterological centre (aged 60 or older). The frequency and type of digestive diseases for this group of patients was established. Nineteen (33.3%) of the former 57 patients from the survey referred frequent digestive discomfort, predominantly symptoms related to the upper gastrointestinal tract, like epigastric pain, meteorism, early satiety, abdominal distention and pyrosis. These symptoms could be in part expected due to the physiological changes seen in the elderly, mainly in the esophagus. In the prospective study, digestive diseases related to the upper gastrointestinal tract were the most common (46.5%), followed by diseases from the colon and rectum (23.2%) and those of the liver, biliar tract and pancreas (16.2%). Taking into account the high incidence of gastrointestinal symptoms in the elderly, we must make special efforts to extend and or complete studies related to the geriatric population in our country. We need to establish rules for the control, prevention and early detection of such illness in order to promote higher standard in the quality of the life of population which will increased considerably in the near future.

[Famotidine in the treatment of duodenal peptic ulcer].

A multicenter, open study was designed to evaluate the efficacy and tolerability of a novel and potent H2 receptor antagonist, Famotidine, in the treatment of duodenal ulcer. Thirty-four patients with endoscopically proven active duodenal ulcer were treated with Famotidine, 40 mg, one tablet, at bedtime, for six weeks. Follow-up endoscopic exams were done every two weeks until ulcer healing was obtained. One patient was excluded from further analysis as he failed to follow the protocol. Healing rates achieved were 48.5%, 91% and 97% by the second, fourth and sixth week of treatment respectively. After 72 hours 52% of the patients were asymptomatic and 79% at the end of the first week of treatment. Famotidine was well tolerated, no clinical neither laboratory adverse events were observed during the study. Famotidine, a novel H2 receptor antagonist appears to be effective and well tolerated in the treatment of most patients with duodenal ulcer.

[Prevalence of hepatitis B surface antigen in general population of coast, mountain and forest regions of Peru. A preliminary report].

A prospective study was performed in order to establish HBsAg prevalence on population living in urban areas of three geographical regions of Peru. There were included 100 apparently healthy people of each city, with a residence major to 5 years, no migrate neither transitory. Each subdivided group in 50 adults and 50 children. Of these last ones 15 between 1 to 7 years old and 20 cases between 7 to 14 years old. Samples were obtained by venipuncture, then they were centrifuged and the serum obtained was stored at -20 degrees C before being sent to Lima for processing. Determination of HBsAg was made by the Elisa's technic (Abbott Lab-III.-USA). In this preliminary report we informed about results in 7 cities (Lima, Iquitos, Chiclayo, Arequipa,Ica, Chachapoyas y Tarapoto) that include 680 persons of which 373 were adults and 307 children. The HBsAg, was positive in 26 cases (3.8%). In the adult group were 13 positives (3.4%), and in the children also 13 cases (4.2%). Result of all sample shows figures of prevalence (3.8%) major to that reported before at national level. The same phenomenon was observed in the cost while in the jungle our results were similar to those found before. In the andean area not studied previously there were obtained similar results to the rest of the country. It is also interesting to mention the high percentages of prevalence in children. These results indicate that hepatitis B is a serious problem of public health in our country that merit to take immediate prophylactic measures.

[Serologic markers of viral hepatitis B in drug addicted++ patients at the Hermilio Valdizán Hospital].

A study was done in 36 drug addict patients at the H. Valdizan Hospital We studied serological markers for Hepatitis B: HBsAg Anti-HBc, and anti-HIV was also made. They were not IV abusers. Five patients (13.8%) were positive for HBsAg and 6 (16.6%) for anti-HBc. Anti-HIV was negative in all the cases, that obtained from our results compared with the general adult population from peruvian coast are high. In relation to the information obtained, the transmission route would be diverse, considering an important one the sexual route. 44 drug addicts were seen in 1988 at this Hospital, 36 cases could be a representative number, although we think we need to extend this study to a bigger population to establish the actual prevalence of this infection among these particular patients. This information could help to advice preventive and control health measures.
