[Osteotomy of the calcaneus for pes cavus in childhood].

Osteotomy of the calcaneus (Dwyer procedure) was performed in 14 children (26 feet) aged between 5 and 15 years. On four occasions it was associated with an osteotomy of the first metatarsal and on three occasions with tarsectomy. The technique is described and the results are analysed. The authors conclude that the results were good in children under the age of 10 whilst the feet were still supple. In children over the age of 10, in whom the feet were more rigid, the gait was improved but the cavus deformity was not improved. The best age for operation is between 6 and 10 years.

[Progressive bone lengthening using Wagner's technique (author's transl)].

The authors have reviewed the results of 30 surgical procedures of progressive bone lengthening using the Wagner's technique. In 28 cases, internal fixation was applied at the end of the lengthening period. There were 17 femora and 13 tibae operated on. In 6 cases, lengthening was associated with a correction of the alignment of the femur, and in 2 cases was associated with the treatment of a non-union of the tibia. Complications are described; they were found to be relatively rare.

[Oral pristinamycin therapy for bone and joint infections in children. A report of 50 cases (author's transl)].

50 children with bone and joint infections (acute osteomyelitis, suppurative acute arthritis, osteoarthritis) were treated with pristinamycin. The responsible bacterium was most often but not exclusively Staphylococcus aureus; its sensitivity to various antibiotics is discussed. The value of biochemical tests for diagnosis and follow-up is emphasized; the potency of this drug avoids the need for intravenous therapy. A therapeutic protocol according to age, type of and sensitivity of the germs is proposed.

[Epiphysiodesis in the treatment of discrepancies in length of the lower extremities (author's transl)].

Thirteen children treated only by epiphysiodesis for inequality in the length of lower limbs were reviewed after maturity. The intention was to estimate the value of the predictions of growth used in this work and the effectiveness of the operation. The result of this procedure depends upon the accuracy in predicting the further growth in order to determine at what age the operation should be performed. The imperfect results of epiphysiodesis are attributable to prediction errors and to operation delay in some cases. Prediction can be made using only roentgenographic measurement of the femur and the tibia, skeletal age and growth charts of Green and Anderson. It is not necessary to make use of others factors such as the size of the child.
