We studied 20 patients with cluster headache (11 males, 9 females) and 10 subjects free from this disorder. Nasal cavities were endoscopically studied to evaluate the mucociliary transport, pH, IgG, IgA and IgM levels in nasal secretion and in serum, and Na, K and Cl serum levels. We observed in all patients in the study group in comparison with the control group an increase of pH, and of serum IgA and IgM levels (but not of IgG). Na, K, and Cl serum levels in the patients with cluster headache were lower than those in the control subjects.
In an ecographic survey for gallstones, executed on a systematic sample from the municipal electoral roll of a town in Southern Italy, 164 subjects were found with ALT more than twice the upper normal limit (unl). Five years later 138 of these were re-examined; 76 still had ALT greater than 2 unl (group A), 41 still abnormal (group B) and 21 normal (group C). Anti-HCV antibodies were found in 52 subjects of group A (68%). 18 of group B (44%) and 2 of group C (9.5%). The odds ratio of ALT greater than 2 unl (A vs C) in anti-HCV+ was 20.6 and of a still elevated ALT (A + B vs C) was 14.1. Logistic regression was used to eliminate the effect of possible confounding factors (sex, age, alcohol, drugs, HBV markers) on the relationship chronic ALT increase and anti-HCV positivity but the odds ratio was still 18.9 (A vs C) and 11 (A + B vs C). These findings suggest that anti-HCV antibodies are strongly associated with chronic hypertransaminasemia at the population level in Southern Italy.
A case of IgD myeloma that began with high paraproteinaemia, BJ proteinuria of type and 40% marrow plasmacellular infiltration is reported. These onset data suggested a large tumoral mass and hence a poor prognosis. On the other hand, after subjecting the patient to 4 polychemotherapy cycles (alkeran and prednisone) CM disappeared from serum and urine and bone lesion progression stopped. It is also stressed that this from presents clinical (frequent extra-osseous growth, presence of amyloidosis), laboratory (severe anaemia, renal insufficiency, BJ proteinuria) and particular prognostic features.

The authors report five cases of dermatomyositis in which positivity for hepatitis C virus was ascertained. A virus-related (ECHO virus, Coxsackie) dermatomyositis is known, but a hepatitis virus C-related dermatomyositis was reported only in one case in a Letter to the Editor of Journal of Rheumatology (1994).
BACKGROUND/AIMS: Migraine is a commonly unilateral throbbing headache, which has been associated with disorders of the vascular tone. Helicobacter pylori, the most relevant cause of gastritis and peptic ulcer, has been recently associated with a typical functional vascular disorder such as primary Raynaud phenomenon. The aim of this study was to assess the prevalence of H. pylori for patients affected by migraine and the effects of H. pylori eradication on migraine symptoms. METHODOLOGY: Two-hundred and twenty-five patients were consecutively enrolled between October 1996 and January 1997. H. pylori was assessed by 13C-urea breath test. Infected subjects were eradicated of the bacterium; frequency, intensity and duration of attacks of migraine were assessed during a 6 month follow-up period. RESULTS: H. pylori was detected in 40% of the patients. Eighty-three percent of the patients who underwent therapy were eradicated. Intensity, duration and frequency of attacks of migraine were significantly reduced in all eradicated patients. CONCLUSIONS: H. pylori is common in subjects with migraine. Bacterium eradication causes a significant decrease in attacks of migraine. The reduction of vasoactive substances produced during infection may be the pathogenetic mechanism underlying the phenomenon.
The authors report three cases of dermatomyositis, which ha been erroneously diagnosed as "chronic fatigue syndrome" due to the presence of elevated titers of serum anti-Epstein Barr antibodies.
