Nutrition and HIV infection in children.

HIV infection has profound effects on a patient's nutritional status because it can modulate appetite, nutrient absorption and basal metabolic rate. In addition, HIV infection can lead to the depletion of a variety of vitamins and micronutrients including vitamins A, D, B2, B6, B12, L-carnitine, iron, zinc and selenium. This review article summarizes existing data regarding nutritional defects in HIV-infected patients and the results of clinical studies addressing the effects of nutritional supplementation in infected patients.

Liquid chromatographic determination of residual nitrite/nitrate in foods: NMKL collaborative study.

Nitrite and nitrate are used as additives in the food industry to provide color and taste and to control undesirable gas and flavor production by anaerobic bacteria by virtue of their antimicrobial properties. The analytical method that has been widely used to determine nitrite and nitrate involves the use of toxic cadmium. In response to a request from the Nordic Committee on Food Analysis, a study was performed to obtain an alternative chromatographic method to determine residual nitrite and nitrate in meat products. The study was done in 3 stages: (1) comparative evaluation of the performance of 3 liquid chromatographic methods, (2) internal validation of the selected ion chromatographic method, and (3) a collaborative study in which 17 laboratories from European countries participated. Furthermore, the applicability of the method to matrixes other than meat and meat products was demonstrated. The results of the collaborative study show that the European Prestandard prENV 12014-4 is well suited for the determination of nitrite and nitrate in different foods (e.g., meat products, vegetables, baby food, and cheese). The limits of detection for nitrite and nitrate ions are 1 and 10 mg/kg, respectively. Recoveries of residual nitrite/nitrate ranged from 96 to 108%. Repeatability and reproducibility were satisfactory.

Tremor in Parkinson's disease: 24-hr monitoring with calibrated accelerometry.

Monitoring systems enable the long-term registration of tremor in patients with Parkinson's disease This method is useful in the objective measurement of tremor during the course of treatments. Indeed, the symptoms of tremor as well as the aggravating and attenuating influences can be observed under real-life conditions. The methodology of data recording and analysis, described in previous investigations, was extended to automatically detect body position and certain movement patterns with calibrated 4-channel accelerometry. The main purpose of the present investigation was to apply this refined and extended methodology to patients in a clinical rehabilitation program, and to examine its practability with respect to the results of the treatment and the patients' compliance. The methodology was tested on 30 patients (17 male, 13 female) with Parkinson's disease. The mean age was 64.8 years (s = 8.9). The Hoehn-Yahr index ranged from 1 to 3 (m = 2.3, s = 0.7) and the overall UPDRS scale between 10 and 74 (m = 42.9, s = 18.1). The data recording included: (1) the registration of tremor under standardised conditions of rest and postural tremor test with and without distraction; (2) a standard protocol to obtain reference values for body position and movement; and (3) the 24-hr monitoring. 21 patients could be recorded a second time, on average 18 days after the first recording. Between the two registrations, patients received individually tailored drug treatment supplemented with specific activating physiotherapy, ergotherapy measures, and individual psychotherapeutic counseling. Changes between first and second recording were evident for the three tremor variables, but significant only for the 24-hr ambulatory monitoring. The between and within-subjects correlations of the tremor variables were rather low except the correlations between occurrence and amplitude (between-subjects. 87; within-subjects. 67). Conditions of rest and postural tremor test showed a correlation with corresponding segments of the ambulatory monitoring of about. 50 for the tremor occurrence. The best prediction of the day-time monitoring was made by the tremor tests with distraction, whereas the night segment was best predicted by the standard protocol.

Triptorelin (D-Trp-6-LHRH) in combination with carboplatin-containing polychemotherapy for advanced ovarian cancer: a pilot study.

In experimental and clinical phase II trials GnRH agonists were shown to possess antitumor activity in ovarian cancer. The outcome of prostatic cancer was also improved experimentally by a combination of cytostatic chemotherapy and GnRH analogs. Encouraged by these results, we administered micro-encapsulated triptorelin (Decapeptyl CR), 3.75 mg on the first day of each chemotherapy course, to 15 patients with advanced ovarian cancer in addition to adjuvant, carboplatin-containing chemotherapy (Carboplatin, Epirubicin, Prednimustine and Carboplatin, Etoposide) after radical surgery. Patients received a total of 6 courses. After the completion of combined chemotherapy GnRH analogs were continued for another 6 months at 28-day intervals. Results were compared with those obtained in a group of 15 ovarian cancer patients receiving the same chemotherapy regimen without GnRH medication. At a median follow-up of 36 months no significant differences were seen in terms of response, survival and time to progression. However, patients undergoing chemotherapy+triptorelin tended to show a more positive outcome than those on chemotherapy alone. G1 and G2 tumors in particular were found to respond better to the combined treatment regimen.
