The prognostic significance of marrow micrometastases in women with early breast cancer.

Twenty-five patients with early breast cancer (T0-T2, N0-N1, M0) have been studied prospectively to determine the relationship between marrow micrometastases, disease-free interval and survival. Marrow specimens were aspirated from three sites immediately prior to breast surgery. An immunocytochemical technique using monoclonal antibody LICR.LON.M8.4 was employed to detect micrometastases. The minimum follow-up was 38 months. Twelve of the 25 patients (48%) had micrometastatic lesions in their marrow at presentation. Four of these patients developed distal recurrence during follow-up, causing death in two of them. Five of the 13 patients with no evidence of micrometastases developed distant recurrence and four of them have died. There was no correlation between the state of the marrow and the development of metastatic disease, although axillary lymph node status, disease stage and tumour volume correlated significantly with outcome (all P less than 0.025). Micrometastatic lesions appear to be common in the marrow of patients with early breast cancer. We have been unable to demonstrate that they have prognostic significance.

Calcitonin gene-related peptide-1 (CGRP-1) is a potent regulator of glycogen metabolism in rat skeletal muscle.

We investigated the effects of CGRP on glucose metabolism in intact rat skeletal muscle preparations that are largely composed of either type I (soleus) or II fibres (e.g. extensor digitorum longus (EDL) or epitrochlearis muscles). CGRP-1 inhibited insulin-stimulated glycogen synthesis in both soleus and EDL muscle preparations. Rat CGRP-1 was a potent stimulator of glycogenolysis only in muscles composed of type II fibres, which depend on high rates of glycogenolysis to produce high power outputs. These results may provide the basis for understanding how CGRP regulate glycogenolysis in type II fibres in vivo.

Anthelmintic efficacy of fenbendazole against Libyostrongylus douglassi and Houttuynia struthionis in ostriches.

Fenbendazole at a dosage rate of 15 mg FBZ per kg body mass, was 99,7% effective against adult and 82,5% effective against 4th stage larvae (L4) of Libyostrongylus douglassi. Fenbendazole at the same dose removed scolices and strobila of Houttuynia struthionis from 4 out of 5 treated birds and in the other there were only degenerating scolices, but all 5 untreated control birds were positive for H. struthionis.

The results of breast reconstruction following mastectomy.

This paper is a retrospective study of the techniques, complications and results of breast reconstruction following mastectomy in 82 patients, 66 of whom had breast cancer. The mean length of follow-up was 3 years 8 months. Of the 74 patients who were reviewed 61 were pleased with the results. Eight patients died, 7 of them from disseminated breast cancer. In our experience breast reconstruction is a very worthwhile procedure in motivated patients and may reduce the psychiatric morbidity of mastectomy.

Therapeutic retraction of the foreskin in childhood.

A retrospective study was conducted of 91 boys who had had a non-retractable but non-fibrosed prepuce treated by retraction under general anaesthesia. Of the 79 boys who had had symptoms, 67 (85%) obtained relief. Twelve of the 91 patients were later circumcised because of continuing problems. Retraction of the foreskin alone is a simple and effective alternative to circumcision in managing most boys with a symptomatic, non-retractable prepuce.

Pneumoperitoneum due to PCI in a patient with a strangulated femoral hernia.

Although pneumatosis cystoides intestinalis (PCI) is a well-recognised condition, an individual surgeon is unlikely to encounter it often. The collected experience of two surgical units in Glasgow teaching hospitals over a 15-year period consisted of five patients. Surgical and radiological trainees may therefore have little or no exposure to the diagnostic difficulties this condition can cause. We report an illustrative case.
