[Hemodynamic mechanisms of ECG changes in congenital fistulas of the coronary arteries].

ECG changes were analysed in 31 patients with congenital coronary arterial fistulas with reference to the site of coronary arterial lesion and the level and magnitude of blood discharge. Two hemodynamic factors, conditioning ECG changes, were identified as typical for coronary arterial fistulas: 1) volume strain on cardiac compartments making up the fistula receptacle chamber; 2) perfusion deficiency in the distal segment of the affected coronary artery. Fistulas with large-volume shunts may be accompanied by both ischemic changes in the affected-artery basin and regional perfusion disorders in adjacent basins, the latter being due to "stealing" of the adjoining coronary trunk or the type of coronary blood supply.

[Thyroid hormone content of the blood plasma and tolerance for physical loading in ischemic heart disease patients].

Blood thyroid hormones were measured before and after the rationed exercise test in 79 patients with angiographically verified diagnosis of coronary heart disease. A relationship was demonstrated between postexercise blood T3 variation and functional coronary circulation reserve. Hormonal baselines, and exercise-induced blood T4 changes were basically similar in patients with coronary disease of different severity. The results are suggestive of pathogenetic importance of thyroid function in these patients.

[Quantitative ECG analysis in diagnosing ischemic heart disease: a comparison with coronary angiographic data].

By collating the results of qualitative and quantitative analysis of the ECG with the findings of selective coronary angiography obtained from 32 patients with coronary heart disease, considerable informative value of the planimetric method of the quantitative analysis of the ECG has been proved. It is concluded that the quantitative interpretation of changes in the areas of the initial and end fragments of the ventricular complex of the ECG contributes to early diagnosis of coronary heart disease and in some cases it helps to specify the localization of coronary impairments.

[ECG manifestations in severe involvement of the coronary arteries in patients with chronic ischemic heart disease].

Correlation of electrocardiographic and coronarographic findings in patients with severe chronic coronary heart disease and massive coronary arterial lesions has demonstrated a tendency to increasing proportion of clinically normal electrocardiograms as more arteries are drawn into the pathologic process. Relationships between ECG voltage and different variants of coronary arterial damage are reviewed: the involvement of the right and part (at least one branch) of the left coronary arteries corresponds to low-voltage ECG. The association between left-ventricular aneurysm and the number of affected coronary arteries is examined.

[Late results of the surgical treatment of a postinfarct interventricular septal defect].

Surgical treatment of patients with post-infarction defect of the interventricular septum is highly effective. Despite the complexity and large volume of operation in this contingent of patients results of the operation are much better than in patients treated conservatively. At late periods after operation fairly high physical activity of patients is observed, which permits return to work to some of them, and to patients of pension age, enables them to do house chores, while the survived nonoperated patients are hardly capable of self-help.

[Electrocardiographic diagnosis of patent ductus atrio-ventricularis].

The paper deals with the diagnosis of defective development of the patent ductus atrioventricularis. Electrocardiographic changes in 81 patients with firmly established hemodynamic form of the patent ductus atrioventricularis, ascertained during surgery and catheterization, are summarized. Characteristic electrocardiographic signs of the defect distinguishing it from other congenital diseases and differential-diagnostic symptoms of diverse anatomo-hemodynamic forms of the patent ductus atrioventricularis were educed.
