Boundaries in the workplace.

Employees in all health care settings are vulnerable to violations of boundaries: crossing that sometimes blurry line drawn between staff and patient. Accepting gifts, financial transactions, and romantic entanglement could lead to various negative consequences for the agency, staff, and patients. Supervisors need to be aware of early warning signs of boundary violations as well as training issues. The authors outline a successful program for addressing these issues and argue for rehabilitating the chronic boundaries violator.

Addressing ethical boundaries among nurses.

Sexual intimacy is the primary boundary issue studied and reported in the literature, though it is not addressed directly in the ANA Code of Ethics. However, perhaps equally important are behaviors which may stem from nurses' meeting their own social and emotional needs through their work. Results of "Boundaries" workshops for more than 3000 nurses and other healthcare professionals are summarized. Recommendations are made, relative to both personal and professional behavior, along with a list of suggestions for nurse managers.

Surgery to quieten the yelling of a demented old man.

The 84-year-old man's incessant yelling caused him to be unmanageable in all settings--at home with his aged wife, in nursing homes, and on the medical and psychiatric floors of a Veterans Administration Medical Center. A host of behavioural and pharmaceutical interventions were attempted unsuccessfully. Finally, it was proposed that a single, recurrent laryngeal nerve be crushed in order to 'render his voice to be a very acceptable soft tone'.
