Monitoring of brain tissue pressure with a fiberoptic device.

Continuous monitoring of brain tissue pressure can now be achieved with intracerebral placement of fiberoptic microtransducers. This study was undertaken to test the safety, accuracy, and reliability of this relatively new type of intracranial pressure (ICP) monitoring. Initially, the fiberoptic device was compared with a concurrently functioning intraventricular catheter in 18 patients. The results from the two methods corresponded closely over a wide range of pressures, and the correlation coefficient approached 1.0. Subsequently, this monitor was used for routine measurement of ICP in a series of almost 200 neurosurgical patients at risk of intracranial hypertension. The tracings showed good wave forms and consistent absolute values of ICP. No instances of hemorrhage, mechanical failure, or other complications were associated with this monitor, except one case of infection, which was not directly attributable to the device per se. When bilateral intraparenchymal pressures were recorded in patients with unilateral mass lesions, significant transitory pressure differentials between the ipsilateral and contralateral sides were documented. It is concluded that monitoring intraparenchymal pressure with the fiberoptic device offers safe and reliable ICP recordings for routine neurosurgical practice. In patients with unilateral masses, ICP should be measured in close proximity to the lesion.

[Prevention of postoperative complications in vertebro-spinal cord pathology].

In 352 patients, affected by vertebral-medullary pathology, we evaluated the role played by the prevention or by the treatment of postoperative complications. We also evaluated their influence on the clinical outcome and neurological recovery. In every case we recorded a scrupulous post-operative neurological monitoring of the eventual complications, on the basis of the type, gravity and duration. From our results it appears that the complications, mostly secondary to infectious diseases, have an influence on the clinical and neurological recovery. We highlight the importance of an adequate nutritive management to prevent the complications, specifically those secondary to infectious diseases and cutaneous lesions.

Epidural spinal meningioma. Role of magnetic resonance in differential diagnosis.

A rare case of an extradurally growing spinal meningioma in an elderly woman is reported. Neuro-imaging, particularly magnetic resonance (MR), allowed to recognize the lesion, which, otherwise, could raise problems of differential diagnosis with a spinal metastasis. An emergency operation, required by a sudden neurological deterioration, was decisive in recovery of neurological deficits. In a review of the literature, extradurally growing spinal meningiomas appear to occur with a higher frequency than it is thought. Therefore, they are to be suspected when dealing with extradural spinal lesions.

[Early stabilization in severe cervical vertebral and spinal cord injuries].

In the cervical vertebral-myelic trauma a high incidence of bronchopulmonary, metabolic, thromboembolic complications and bedsores is present. The authors, in agreement with the most recent literature (Tator, 1987), affirm that adoption of the precocious stabilization, allowing an equally precocious mobilization, considerably reduces the above-said complications. Thirty patients with vertebral-myelic lesions needing operation for stabilization have been followed. Twenty-one of these, presented irreversible myelic damages and nine cases had partially or completely reversible damages. Only in five cases severe bronchopulmonary affections and bedsores occurred. Thrombo-embolic complications did not occur. The authors conclude that the precocious mobilization allowed by the stabilization realized in acute state, permits a drastic decrease in the complications typical of this kind of affections.

Computerized use of ICP data. Preliminary elements on the shunt patient.

An incorrect choice of valve may produce a delay in psychomotor development in shunted hydrocephalic children. The intracranial pressure was measured for 24 h in 20 subjects suffering from nontumoral hydrocephalus, obtaining a pressure/volume index with compliance and perfusion pressure data. Their data were entered into a computer and used to determine the correct valve to be chosen.

Cerebrospinal Fluid Shunts

[Experimental fetal hydrocephalus. Ventriculo-amniotic shunt].

When the original idea was conceived 8 years ago to develop a surgical technique for the treatment of fetal hydrocephalus, the pathway towards prenatal surgery for this affection was totally unexplored. The operation appeared to be feasible because of the possibility of a ventriculo-amniotic shunt. The most important steps in our experimental studies in the ewe were creation of a fetal hydrocephalus by injection of a suspension of kaolin powder into cisterna magna; the treatment of the hydrocephalus by a mini-ventriculo-amniotic shunt, by cutting the cardiac end of a low pressure Pudenz pediatric catheter. Lambs were delivered by Cesarean two weeks before end of gestation. Coronal sections of brains of control and treated lambs showed reduction in hydrocephalus in the latter. These findings were the basis for the application of the method to treatment of human fetal hydrocephalus.

A model of the early stages of the human visual system: functional and topological transformations performed in the peripheral visual field.

A model of the early stages of the visual system is presented, with particular reference to the region of the visual field outside the fovea and to the class of retinal and lateral geniculate nucleus cells which are most active in the processing of patter information (X-cells). The main neuroanatomical and neurophysiological properties taken into account are: the linear increase of the receptive fields diameter with eccentricity, the constancy of the overlap factor and the topological transformation operated upon the retinal image by the retino-cortical connection. The type of filtering taking place between the retina and the visual cortex is analyzed and some simulations are presented. It is shown that such a filtering is of a bandpass space variant type, with center frequencies that decrease from the center (i.e. the fovea) toward the periphery of the visual field. This processing is "form invariant" under linear scaling of the input. Moreover, considering the properties of the retino-cortical connection, it is shown that the "cortical images" undergo simple shifts whenever the retinal images are scaled or rotated.
