[Diabetes and ignored disorders of glucose tolerance in primary care. Evidence from opportunistic detection. Group for the Study of Diabetes in Tarragona].

OBJECTIVE: To investigate the level of attendance for care of people with glucose tolerance disorders, both known and not, and to discuss their real impact on Primary Care. DESIGN: Simple prospective randomised sampling. Opportunist detection among spontaneous users over 40, excluding pregnant women. SETTING: Primary Health Care. PATIENTS AND OTHER PARTICIPANTS: Data was recorded using a pre-designed survey, which included the recognised Diabetes (DM) risk factors. MEASUREMENTS AND MAIN RESULTS: Of the 183 cases evaluated, DM was already known in 13 (7.1%). 15 (8.2%) with unknown DM and 27 (14.7%) with GIT. A strong link was demonstrated with the average age and a marked tendency to overweight and obesity only significant when comparing DM with the rest. As to the risk factors researched, there was a significant association with antecedents of prior tolerance abnormality, DM in pregnancy, hyperglucaemiant medicines and ischaemic disease, in cases where family antecedents of definite diabetes were not found. CONCLUSIONS: The frequency of attendance of users with diabetes and unknown glucose tolerance disorders is very high in spontaneous primary care consultations. These results suggest that opportunist detection should be encouraged in primary health care centres.

Changes in serotonin-immunoreactivity in the dorsal and median raphe nuclei of rats exposed to 2,4-dichlorophenoxyacetic acid through lactation.

Comparison of serotonin-immunoreactive (SER-IR) neurons in nucleus raphe dorsalis (NRD) and median raphe nucleus (MRN) of 25-d-old rat pups exposed to 70 mg/kg/d 2,4-dichloro-phenoxyacetic acid through mothers milk and control pups was made using an immunohistochemical analysis. Significant 2,4-D-treatment-related increase in size and density of SER-IR neuronal somata as well as in fiber length were observed. We postulate that exposure to 2,4-dichlorophenoxyacetic acid on the first day of life would modify the synthesis of 5-HT or the maturation of the brain serotonergic system.

[Esophageal intramural pseudodiverticulosis].

Esophageal intramural pseudodiverticulosis is an uncommon and benign primary disease of the esophagus. Less than 150 cases have been reported. We report a new case diagnosed by endoscopy during the investigation of an upper gastrointestinal bleeding, in a 48-year-old man who was assymptomatic from the esophageal point of view. We discuss the clinical, pathogenic, diagnostic and therapeutic aspects of this disease.

[Application of the high resolution chromosomic technic in patients in high risk of cytogenetic anomalies].

Since the beginning of cytogenetics, there has been a constant improvement of chromosomal culture and banding techniques. In 1976, Yunis described a high chromosomal resolution technique (HRC), that permits the detection of subtle chromosomal abnormalities. The present work, reports the results obtained when HRC was applied to the study of chromosomal abnormalities in patients with high risk of such. The study comprised 434 specimens of venous blood and 182 bone marrow aspirates. The samples were classified according to the presuntive diagnoses. The highest frequency of chromosomal abnormalities, was found in blood samples from patients with physical deformities with or without mental retardation (22.22%), followed by mental retardation autism and/or fragile X chromosome (13.66%), and in couples with reproductive disorders (5.8%). In bone marrow, the most frequent abnormalities corresponded to patients with chronic myeloid leukemia (78.43%), acute lymphocytic leukemia (62.10%), acute myeloide leukemia (61.9%), myelodisplastic syndromes (43.7%) and chronic lymphocytic leukemia (14.2%). The present results stress the need to apply the HRC technique when the probability of minute chromosomal abnormalities is high.

Canine cutaneous mast cells dispersion and histamine secretory characterization.

In view of the high incidence of canine cutaneous atopic disease and the relevance of mast cells to its pathogenesis, it was considered important to isolate firstly cutaneous mast cells from normal dog skin and to assess the histamine secretory activity, as this can be further used as a tool for the study of canine skin mast cell pharmacology in cutaneous atopy. The procedure for canine dermal mast cell dispersion following a skin enzymatic digestion (as for previous human skin mast cell dispersion methods) is described in detail. The number of canine cutaneous mast cells yielded per gram of skin was 2.31 +/- 0.21 x 10(5) representing 1.00% of the total cutaneous cells. The total histamine content per mast cell is 4.93 +/- 0.39 pg. Net histamine release owing to stimulation by calcium ionophore A23187 (1 microM) and concanavalin A (1 mg ml-1) was respectively 32.17 +/- 3.56% and 20.39 +/- 2.41% of the total amount per cell. Viability and reactivity to both stimuli of dispersed cutaneous mast cells were similar to the results found in humans. The present study allows further research on the role of mast cells immunopharmacology in allergy by investigation of cells isolated from canine skin in naturally occurring or experimentally induced atopy in the dog to be undertaken.

[Alcohol and the risk of injuries in 3 emergency services in Acapulco, Mexico].

The present study of 421 cases and controls was conducted in three hospitals in the city of Acapulco, Mexico, over a one-month period to determine whether alcohol consumption constitutes a risk factor in four types of traumatic events: assaults and fights, falls, traffic accidents, and domestic accidents. A total of 274 cases were selected from the population over 15 years of age who presented at the emergency services in the three hospitals for injuries sustained in these occurrences, together with 126 controls from the same age group who consulted the services because of occupational accidents, sports injuries, and animal and insect bites. The amount of alcohol consumed prior to the trauma was measured by blood alcohol concentration (determined by an alcoholometer) and the degree and state of intoxication reported by the patient himself. The relative risk associated with each variable was calculated using the odds ratio (OR) and 95% confidence intervals. Logistic regression was used to control the OR for sex, age, schooling, place of residence, occupation, and time of the week when the injury occurred. The results showed that moderate alcoholemia (10-99 mg/100 ml) constitutes a risk factor for assaults and fights (OR = 12.77; CI = 3.69-44.15) and traffic accidents (OR = 8.96; CI = 2.01-39.96). On the other hand, there was no significant correlation between the four types of injury and alcoholemia over 99 mg/100 ml.(ABSTRACT TRUNCATED AT 250 WORDS)

[Extracorporeal lithotripsy in sequelae of neurologic lesions].

We report on the results of shock wave extracorporeal lithotripsy in a series of 13 patients with urinary lithiasis and neurologic disease (5 tetraplegics, 6 paraplegics and 2 hemiplegics). Anesthesia, positioning and complementary endourologic procedures in a group of patients in whom many factors contribute to lithogenesis are discussed. Sixteen renal units were treated with good stone fragmentation in all cases. Seventy-five percent of the patients remained stone-free three months later. Complications were few (15%) and minor (mainly post-operative fever) and the hospital stay was short (average 5 days). We advocate this technique as the treatment of choice of urolithiasis in patients with neurologic sequelae because of its safety, relative noninvasiveness and good results, especially in calculi smaller than 2 cm.

Susceptibility to genotoxic effects of niclosamide in human peripheral lymphocytes exposed in vitro and in vivo.

Niclosamide added in 2-h pulses to lymphocyte cultures induced a small clastogenic effect in one blood donor, while in two other blood donors it inhibited mitosis. In the presence of the 'S9' metabolic activation system, the antihelminthic drug exhibited a dose-related increase in clastogenicity in 2 of 4 blood samples. A weak dose-related increase in S.C.E. was observed only in the lymphocytes from one of these blood samples. From 5 patients treated with niclosamide, 3 showed an increase in chromosomal aberrations after treatment; in none of them was an induction of S.C.E. observed. These results suggest differences in lymphocyte susceptibility to the genotoxic effects of therapeutic drugs and also underline the need for evaluating chromosomal aberrations as well as SCE in any study of genotoxic substances.

Frequency and types of induced and spontaneous chromosome aberrations in relation to cell kinetics.

Induced and spontaneous structural chromosome aberrations (SCA) were studied in a child accidentally radiated with a high dose of 192Ir, and in three sibs with Fanconi's anemia, analyzing by separate first division metaphases (FDM) and second division metaphases (SDM). The results showed that the number of SCA, number of cells with aberrations, and SCA per cell were markedly higher in FDM in all patients. Furthermore, for some type of structural changes like dicentric chromosomes and chromatid interchanges, the differences were particularly striking. The importance of ascertaining FDM identified with proper techniques, for the study of the clastogenic effect of environmental agents and some aspects related to the differences in cytogenetic features found in diverse tissues in Fanconi's anemia are discussed.

Osteoid osteoma: an unusual presentation.

An osteoid osteoma developed in the fracture callus four years after an open reduction and internal fixation of a femur of a 21-year-old man. The treatment consisted of en bloc excision of the lesion using intraoperative roentgenographic studies for localization of the lesion.

Body composition in hypopituitary dwarfs before and during human growth hormone therapy.

The clinical characteristics and body composition of eight hypopituitary dwarfs (10.2-21.6 yr) were analyzed before and after 6 and 12 mo of growth hormone therapy. 2 IU 3 times/wk. Before treatment, growth rate was 1.8 +/- 0.7 cm/yr, height age was 2.0-12.8 yr less, and bone age 2.0-11.1 yr less than chronologic age. Total body water (TBW), lean body mass (LBM), extracellular water (ECW), and intracellular water (ICW) were below normal for chronologic age, but normal for height. Muscle mass (MM) was below normal for age and height. During HGH therapy, growth rate was 7.1 +/- 1.6 cm/yr in the first 6 mo and 7.8 +/- 1.4 cm/yr during the next 6 mo; the ratio of change in height age to change in chronologic age was greater than or equal to 1.0 in all patients and the ratio of change in bone age to change in height age was 1.2 in one patient and less than or equal to 1.0 in the others. TBW, LBM, ECW, and ICW increased according to height increments; however, MM increased at a faster rate than expected from the height gains. Also, a relative or absolute loss of total body fat was recorded during the first 6 mo of therapy. It is suggested (1) that among the body composition parameters studied, muscle mass is the tissue most closely reflecting the lack of HGH and also its therapeutic benefits and (2) evaluation of body composition in hypopituitary dwarfs in response to HGH therapy shows striking changes not reflected by the determination of stature or weight alone.

Comparative study of serum luteinizing hormone levels in human subjects as measured by radioimmunoassay and radioreceptorassay.

A rapid, precise, sensitive, and specific radioreceptorassay (RRA) for serum luteinizing hormone (LH), using a 15,000 X g pellet from bovine corpora lutea, was developed. A comparative study of the serum LH levels in human subjects as measured by this RRA and by radioimmunoassay (RIA) using the same standard preparation was then conducted. The serum LH profile throughout the entire menstrual cycle and the pituitary responsiveness to LH-releasing hormone (LH-RH) stimulation during the two phases of the cycle were studied in two normal women. In addition, LH levels in two normal postpubertal men were measured in blood samples obtained before and after LH-RH administration. Similar, although not identical, LH profiles were found in all cases by both hormone assay methods. Higher RRA-assayable LH values were obtained throughout the menstrual cycle as compared with those obtained by RIA. A significant LH-RH pituitary response in terms of LH in both RRA and RIA during the luteal phase was observed as compared with that observed during the early follicular phase. In the two normal men, the LH values obtained with RRA were higher than those obtained with RIA. It is concluded that the LH RRA is a practical and efficient tool for clinical research.
