Diclofenac sodium versus acetylsalicylic acid: a randomized study in febrile patients.

One hundred and twenty adult patients with high temperatures (greater than or equal to 38 degrees C) brought about by influenza viruses or other conditions were randomly treated with two different antipyretics: a) a 25 mg sodium diclofenac tablet (Novapirina) every 12 hours for 2 consecutive days; b) a 500 mg tablet of acetylsalicylic acid (Aspirin) every 8 hours for 2 consecutive days. Antipyretic action (assessed at 6 hours following the first administration) was found to be equally rapid and consistent in both cases but significantly longer-lasting in the Novapirina-treated group than the Aspirin-treated group (p less than 0.01). Mean temperature changes over the 48 hours of observation and the over-all judgement on the antipyretic effect expressed at the end of each day of treatment were similar for both groups and good in all cases. The antiphlogistic-painkilling properties of both drugs were found to be effective in improving the symptomatology accompanying the high temperature during the course of the bout of influenza. The effectiveness/tolerability ratio was found to be satisfactory for both groups: only one case of gastric intolerance to Novapirina was recorded and five cases of gastric intolerance to Aspirin.

Early detection of breast cancer recurrences through periodic follow-up--is it useless?

The authors report on a multicentric consecutive series of 1120 breast cancer first recurrences. Cases detected as subjectively asymptomatic thanks to periodic follow-up examinations are compared to cases detected as symptomatic. The relapse-free interval from primary treatment was shorter for asymptomatic recurrences, confirming that an earlier diagnosis was achieved in these cases. In spite of this diagnostic anticipation, median and actuarial survival from primary treatment did not differ when asymptomatic recurrences were compared to symptomatic recurrences. The study results did not show any prognostic impact of periodic follow-up in breast cancer and urge for prospective controlled studies on this diffuse and expensive practice.

Prognostic value of estrogen receptors determined by radiochemical vs. histochemical methods in breast cancer.

Estrogen receptors (ER) were evaluated in 634 breast cancer patients by the dextran-coated charcoal method (DCC). In 206, ER and progesterone receptors (PR) were also tested by cytochemistry (Lee method), and in 124 ER were tested by immunofluorescence (Pertschuk method). The median follow-up is 3.7 years. Comparisons have been made between receptor content and: anatomical and clinical features, disease-free survival (DFS), and survival. The following conclusions can be drawn: there is no correlation between ER determinations by DCC and by immunofluorescence or cytochemical methods; there is no evidence of association between ER and PR determined by Lee's method and anatomical and clinical features; a highly significant positive association was found between ER rich specimens and age, post-menopausal status, lobular and tubular histologic types; there is no association between ER values and TNM stage, WHO grading, pathologic prognostic factors of primary tumor and of lymph nodes; and the DFS was not affected by ER status, except for tumors with more than 50 fmol/mg protein.

Prognostic value of the staging system proposed by Merlini, Waldenström and Jayakar for multiple myeloma.

We have reviewed a series of 90 consecutive patients with multiple myeloma (MM) diagnosed in the period 1.1.1971-31.12.1980 and subsequently treated with an intermittent administration of melphalan and prednisone (62 IgG MM, 20 IgA MM and 8 Bence-Jones (BJ) MM). Unfavorable prognostic factors were: presence of micromolecular M component (K or lambda), serum creatinine greater than or equal to 2 mg/dl, serum calcium greater than or equal to 12 mg/dl, presence of BJ proteinuria, Hb less than 10 g/dl, diffuse osteolytic lesions, bone marrow plasma cell percentage greater than or equal to 30%. In particular, according to the recent staging system proposed by Merlini et al. [Blood 55: 1011, 1980], the most important prognostic parameters for IgG and BJ MM were: serum creatinine, BMPC % and serum calcium; for IgA MM the most important were: serum hemoglobin, calcium and M component levels. We have compared the survival curve of 45 patients who died, with the predicted survival curve according to the multivariate regression analysis of Merlini and co-workers; the prediction was equally precise for all the three types of MM considered, thus showing the high sensibility of this new staging system.

Radiosurgical treatment of uterine cervix carcinoma at the 1st Clinic of Obstetrics and Gynecology of the University of Turin, (in the period 1972-1978).

The Authors report the results of a planned prospective study on Radiosurgical treatment of carcinoma of the cervix at stage Ia, Ib, and IIa. The protocol adopted is based on preoperative Curietherapy performed by Ir192 on a after loading technique device, hysterectomy according the Wertheim-Meigs' technique with systematic lymphadenectomy for the internal and external iliacs and obturator stations, Telecobalt therapy in cases with hystological evidence of lymphatic spread. The Authors discuss the results after three and five years in term of survival according to stage and type of treatment, the anatomosurgical examination and the histologic findings of the lymph nodes. At the end, they analysed the complications according to the stage and the kind of treatment, studying only recovered patients and performing the analysis after three years, in order to allow all complications to appear.
