Screening of newborns for congenital hypothyroidism in Croatia--organization and first results.

In 1985 a newborn screening programme for the detection of congenital hypothyreosis was introduced in Croatia in addition to the already existing one for phenylketonuria. The paper delineates the organization of the screening programme, the method used, and the first results. Clinical manifestations, somatic and mental development, as well as laboratory findings of the first eleven children with congenial hypothyroidism detected by the screening programme and followed-up regularly are presented in more detail.

[Primum non nocere--what does it mean in medicine today?].

The study analyzes bioethical principle of Primum non nocere and its importance and value in modern medicine. The criterion of maleficence for a patient, is defined in its real and ethical, individual, general and time contexts. The relation of ethical postulates of non-maleficence and beneficence and their correlation have been studied. The attention has been drawn to the inadequacy of isolated criterion of causing damage (non nocere) to secure benefit for a patient. The study expresses the necessity and obligation of medicine to estimate scientifically old and new perceptions and applies them to a patient, consonant with bioethical principles.

Plasma renin activity and plasma aldosterone concentration in infants with various sodium intakes.

Plasma renin activity (PRA) and plasma aldosterone concentration (PA) were measured in 20 healthy infants 2-12 months of age, after taking a commercial modified cow's milk formula with 8.2 mmol/l sodium for at least five days, and after at least five days on a home-made, undiluted acidified cow's milk formula with 12.6 mmol/l sodium. The mean sodium intakes were 1.20 and 1.87 mmol/kg of body weight per day respectively. The arithmetic means for the corresponding PRA were 6.16 and 1.08 ng/ml . h (difference not significant) and the corresponding arithmetic means for PA were 603 and 355 pg/ml, the difference being highly significant (P less than 0.01). It is concluded that one has to consider the sodium intake in the milk formula when studying plasma aldosterone in infancy.

Haplotype distribution and mutations at the PAH locus in Croatia.

Restriction fragment length polymorphism (RFLP) haplotypes and mutations at the phenylalanine hydroxylase (PAH) locus have been studied in 25 unrelated families from Croatia. The results of RFLP analysis demonstrated that 80% of the mutant alleles were associated with three haplotypes (1, 2 and 4). Eight mutations were detected on the background of six mutant haplotypes, comprising 68% of phenylketonuria (PKU) alleles in Croatia. The mutation in codon 408 was most frequent, as was the haplotype 2 allele with which it was associated. These data are in accordance with formerly published population genetic analyses at the PAH locus, and with studies revealing the molecular basis of the phenotypic heterogeneity of PKU. The codon 281 mutation was more frequent in Croatia than previously observed in other populations.

[The status of care of abused and neglected children in Zagreb at the end of the 20th century].

In this paper, the data on some medical, social and legal aspects of abuse and/or neglect of children in Zagreb are presented which, at least, partially illustrate this societies' care of mistreated children at the end of the 20th century. We reviewed questionnaires that were administered to 44 health institutions involved in the childrens' health control and/or treatment and to 14 social work services and analyzed cases of child mistreatment reported to the Zagreb Public Prosecution Office and final court judgements for 1987 to 1988. We conclude that all these institutions encounter cases of child abuse and/or neglect but that medical institutions identify only 10% of cases detected by social work services and that judicial system sentences with imprisonment about 30% of abusers. Underdetection of child mistreatment in Zagreb, in addition to poor coordination of services, are the possible reasons for insufficient concern for these children. Up to now, only mass media and scientific publications have spoken more loudly about this problem. The authors emphasize the need and obligation of all those professionals concerned with children and of the entire community to undertake relevant measures to protect abused and/or neglected children.

The aim of this paper was to get all the physicians involved in the care of children more thoroughly acquainted with the extent, manifestations and management of abused and/or neglected children. The necessity of an organized multidisciplinary approach to this serious medical and social problem is also stressed.

[Infant health care].

This paper presents some aspects of the infant health care in Croatia in the last decade of the 20th century. Neonatal and postneonatal declining of mortality, shift of the infant mortality into the first month of life and causes of mortality in the first and later months to the end of the first year of life are shown. The structure of health care on primary and secondary level is presented. Stressed are the problems of delivery without professional help, care of the newborn after hospital discharge, sine-medical mortality of infants and infants death from accidents and injuries. The author urges for further development of health care system for infants and infants death from accidents and injuries. [Inborn metabolic errors of urea cycle synthesis. Present possibilities of treatment].

The case history of an infant with congenital hyperammonaemia due to inherited ornithine-transcarbamylase deficiency is presented together with some basic data on his family. The metabolic cycle of urea synthesis is delineated with its enzymes and the possible inherited defects. The differential diagnosis of hyperammonaemia in the newborn infant is given. The successful contemporary method of therapy of urea cycle disorders is presented.

[The amazing neonate].

According to a questionnaire including 78 general practitioners, specialists in pediatrics and obstetrics, physicians specializing pediatrics and obstetrics, and 43 students of a High Nursing School, 21.8% of physicians and 58% of nurses think that the newborn does not see, 17.9% of physicians and 18.6% of nurses think that the newborn does not hear, and 44.9% of physicians and 79.1% of nurses think that the newborn has no feeling of the position of the body in space and of its movements. The existence of the sense of smell in the newborn is denied by 66.7% of physicians and 55.8% of nurses, and of the sense of taste by 44.9% of physicians and 41.9% of nurses. All the nurses know that the newborn has the sense of touch, while 3.8% of physicians think the opposite. Two thirds of those included in the questionnaire think that the newborn can memorize and the three quarters of them think it can learn. Much fewer nurses than physicians (32.4%:61%) consider the newborn a passive receiver of external stimuli. As many as 97% of physicians think that an early stimulation of the newborn helps its psychomotor development and a high percentage of them positively assess the majority of measures implementing it. Yet, only 66.7% of physicians consider it necessary for parents to be near the hospitalized premature infant, while 24.4% of all the questioned do not consider the prevention of the birth of undesirable children a measure by which the quality of the parent-child relation could be influenced.

[Hemorrhagic disease of the newborn].

The authors present 16 patients with the idiopathic forms of hemorrhagic disease in newborns, none of which, after birth, having prophylactically received K-vitamin. Seven of them had an early form, four a classical form, and five a late form of hemorrhagic disease. All of them were born at term, four were female and 12 male newborns. The diagnosis was established on the basis of the decreased prothrombin time (PV) and the activated partial thromboplastin time (APTV), as well as their normalization following the applied therapy. The PV values in these patients ranged from 0.009 to 0.65 and the APTV from 33 to 120. The authors point out the significance of the prophylaxis of hemorrhagic diseases in the newborn and the still inconclusive state of scientific dilemmas and clinical practice in this connection.

[Ethical problems in perinatal medicine: thoughts of a pediatrician].

This study contributes to the consideration of some ethical problems in perinatal and neonatal medicine. The prenatal and postnatal periods present the continuity of human life in its earliest period. All prenatal events are manifested eventually in a newborn human being and determine its ability to further develop until full biological and sociological maturity is reached. New possibilities of medicine, regarding diagnostics, treatment and prevention of diseases affecting fetuses and neonates, brought some crucial philosophical and ethical questions of life, death, human rights and human existence. They have to be answered by both gynecologists and pediatricians, together with the whole social community, as to apply those new achievements to their object-for the good of mankind.
