[Morphological aspects of parathyroid hyperplasia in patients with chronic renal insufficiency on dialysis].

The authors present an anatomic study of parathyroid hyperplasia in 15 patients presenting chronic renal failure treated by hemodialysis (mean time 6 years). The light microscopic examination (n = 56) confirms diffuse hyperplasia so occur in patients submitted to dialysis for a period inferior to 4 years while nodular hyperplasia occurs in patients submitted to dialysis for longer a time. Mixed hyperplasia may be considered a transitional form between diffuse and nodular hyperplasia. Electron microscopic examination (n = 31) reveals the secretory ability of chief cells containing lots of secretory granules, and the energetic power of oxyphil cells filled with mitochondria. The great amount of oxyphil and of vacuolized cells after long periods of dialysis means degenerative transformation of ancient chiefs cells. The authors emphasize the interest of per-operative identification of the type of hyperplasia in order to choose the fitting procedure of parathyroidectomy.

[Partial splenic resections and Nd-YAG laser].

Eleven partial splenectomies are performed in pigs. Simultaneous section and hemostasis are obtained with continuous 80 W power shoot. There is no hemorrhagic or septic complication and normal splenic weight is recovered in 5 months. Platelets elevation observed in post-operative. Platelets elevation observed in post-operative period returns to normal rate between 20th and 30th day. Acute cellular damage of cut splenic edge does not exceed 3 mm in thickness. Semi-thick and electron microscopic examination reveals at day 10 mitotic figures of lymphocytes (i.e. splenic regeneration) and at day 30 high density of mast cells and plasmocytes i.e. cellular biostimulation).

The anatomical value of technetium-thallium subtraction scanning in detection and location of parathyroid adenomas.

99mTechnetium-201Thallium subtraction scanning was performed in 24 patients with primary (N = 5) and secondary (N = 19) hyperparathyroidism. The preoperative scintigraphy (N = 12) detected 21 of 23 enlarged glands surgically removed and was helpful for detecting abnormal location especially in the mediastinum. Postoperative scanning in patients with recurrent hyperparathyroidism confirmed the excessive growth of the remaining half parathyroid after subtotal parathyroidectomy or a missing fifth parathyroid after total parathyroidectomy and autotransplantation. False negative results were due to tumor hyperplasia. The technique is recommended prior to repeated exploration in patients presenting persistent disease to predict the location of adenomas generally unsuccessfully detected by ultrasonography and computed tomography.

The morphological effects in pig liver after Nd-YAG laser resection.

The morphological effects of liver resection in 15 pigs performed with a Nd-YAG laser beam are reported. A regular cut surface was obtained and overlayed an ischemic layer which was thinnest at a 80 W power output and a short exposure time. Hemostasis was satisfying when the encountered vessels did not exceed 4,5 mm in diameter. Semi-thin and electron microscopic examination showed "ghost" cells in the layer of coagulation necrosis, shrunken hepatocytes and red blood cells in oedematous layer and "swollen" mitochondria in the deepest layer. Clamping of the hepatic pedicle - increasing local hyperthermia by suppression of cooling vascular inflow - majorated the depth of cellular alterations and the intensivity of the post-operative inflammatory reaction. Numerous biliary ductules and active hepatocytes meant parenchymal regeneration. Good healing evolution free of complications was observed after a one year's follow up.

[Influence of pedicle clamping in partial hepatic resection with the Nd YAG laser. Comparative study in a series of 15 pigs].

Liver resection is performed in 15 pigs by means of low divergent Nd YAG laser, the hepatic pedicle being clamped and not. Clamping does not modify the efficiency of laser hemostasis during resection but increases the thickness of the ischemic layer underlying the cut liver edge. The coagulation necrosis and the edematous layer are more thickened. Migration of inflammatory cells and young fibrosis present at day 3 are gaining ground until day 15. During the same period, biliary neoductules and nodules of hepatic regeneration appear. Hepatic inflow being conserved, these reactions are delayed and less intensive. The hyperthermia, the inflammatory cellular reaction and the deepness of parenchymal damages induces by pedicle clamping are suitable for antitumoral surgery. Cooling effects of hepatic inflow are better for traumatic surgery.

Hepatic resection with an Nd:YAG laser in pig.

Eleven hepatic resections were performed by means of a divergent Nd:YAG laser. The beam was transmitted through a fiberoptic delivery system without any handpiece. The shortest resection time and most limited ischemic damage (4 mm) were obtained with 80-W power shoots and a low divergent beam (4 degrees 2 for full angle). The hemostatic effects of the Nd:YAG controlled bleeding from veins of up to 4.5 mm in diameter, and parenchymatic oozing from the cut edge minimized blood loss. Histological examination revealed the importance of cellular deterioration in the ischemic layer, while electron microscopy showed selective destruction of cell organelles and protein denaturation. Marked elevation of hepatic serum enzymes suggests a high degree of cellular damage. Postoperative examinations confirmed an uncomplicated healing process. Finally, the use of a flexible fiberoptic enables easy surgical manipulation.

Experimental comparative study on morphological effects of different lasers on the liver.

Three types of laser radiation were compared as to their capabilities in hepatic surgery using 17 New Zealand rabbits. In four animals, subtotal scalpel lobectomy was performed, the hepatic pedicle being clamped. Hemostasis is obtained by means of argon or Nd YAG laser as long as the internal diameter of the vessels encountered is less than 4.5 mm. Nd YAG laser requires a power output four times higher than argon laser to obtain hemostasis. In six animals limited resection was performed by means of Nd YAG and CO2 lasers. CO2 radiation supplied insufficient hemostasis whenever the vessel diameter was more than 1 mm despite an excellent cutting power. Nd YAG radiation created an important necrosis zone due to thermal diffusion. In seven animals various focal hepatic lesions produced by the three types of laser were compared. Argon laser induced superficial and regularly round lesion, Nd YAG laser deeper, cauliflower-like lesions with thermal diffusion following especially vessels. Veins presented thrombosis of their blood content and coagulation necrosis of their wall. The amount of glycogenic granulation decreased. The CO2 laser induced very sharp punctual lesion without necrosis. Electron-microscopic sections revealed faint shadowy liver cells closet to the thermal source and bubbled mitochondria containing disrupted critea around the lesion. In later lesions Nd YAG laser produced more extensive fibrosis than argon or CO2 laser. After first clinical trials we believe that laser irradiation is actually an adjuvant safety factor in current hepatic surgery, whereas the Nd YAG laser could be of real interest in metastatic carcinoma destruction in liver.

[Development of the human parathyroid during the fetal period].

Study of te human foetal parathyroid in 10 specimens from 27 to 360 mms 1.06 to 14, 18 inches) in length. Macroscopically the gland can be located by the recognition of its arterial pedicle and in the fetus from 4 to 6 months old by the presence of pseudocysts of Kursteiner. The histologic investigation in optic microscopy shows the increasing density of cells with a dark nucleus placed beside the other cells without interposition of fat cells. The oxyphil cells and the clear cytoplasm cells are missing. Kursteiner's cysts are situated at the periphery of the gland. The organotypic cultures can be maintained during 5 to 7 days. The viability of the cells is checked with microscopy and with dosage of parathormon in the medium by radio-immuno-assay. The favorable results incite us to follow up the cultures in view of further grafts.

[Venous vascularization of the liver in the human fetus].

The authors have studied the veinous pattern of 30 livers of human foetuses from 190 to 390 mm V.C. The injection-corrosion method was used. The portal and sus-hepatic distribution is more frequently modal than for the adult. The portal anomalies touch more frequently the left branches: drainage of the right para-medial area by the left para-medial vein, agenesis or atrophy of the left segmentary branches. The right branch breaks up into several branches in 51% of the cases. The sus-hepatic anomalies are distinguished by the plurality of the veins. The right middle and inferior veins are present in 70% of the cases. Exceptionally large peripheral anastomoses between portal and sus-hepatic systems are observed.

[Anatomical and embryological aspects of gastric duplications].

Following study of 155 cases of gastric duplications, published up to now and examination of 22 human embryos, the authors discuss classic theories of pathogenesis. They oppose two anatomical forms: tubular forms in 23% of the cases communicating more often with the lumen of the digestive tract, and spherical forms in 77% generally closed. The site of implantation varies. ROWLING'S anatomical criteria are conserved. In embryos, the stomach appears at stage 9 of the embryonic period proper, with the ventral closure of the entoderm. Abnormal lateral longitudinal folding might explain the formation of tubular forms. In two embryos at stage 15, was found a diverticulum of the gastric lumen coated with digestive epithelium, that may explain the later formation spherical forms. No epithelialization or vacuolisation has ever been observed in the gastric wall.

[Gastric duplications. A case report (author's transl)].

The authors report an analysis of 154 true gastric duplications published up to now, and a personal case. This malformation occurs exceptionally and is generally discovered during the first year of life. Diagnosis is rarely performed before laparotomy, clinic signs being various. They insist on investigation by barium enema and fibroscopy. They try to oppose tubular to cystic forms, according to a different embryogenesis. Surgical treatment is specific to each case.

[Arterial vascularization of the left colon in the fetus].

Study on the left colonic arteries in 15 human fetuses (195 to 360 mm VC). The arterial system was injected 9 times with a mixture of gelatin and Indian ink, 6 times with red coloured stratyl. The left hemicolectomic resections are treated with the Spalteholtz method. The arterial axes are constant: left colic artery, the middle colic artery, the middle branch of the left colic artery, the sigmoïd arteries. A paracolic arch, often branched, was always found, an intermesenteric arcade was sometimes present (20%). Straight arteries became more numerous with function and age. Histologic study showed two types of muscular penetration by these arteries.
