[Vascularization of the ulnar nerve of the arm].

A review of 53 dissections associated with injections and radiographs was performed (by the authors) to specify the arteriovenous vascularization of the ulnar nerve of the arm. An arteriovenous pedicle is constantly to be found. The artery "collateralis ulnaris superior" arises from the inferior border of the "pectoralis major" at an average distance of 6,7 cm. The arterial depending vein most often directly rejoins the "vena brachialis" lower than "vena basilica" does, but usually more distally than the arterial emergence level. Thus constituted, the pedicle can be anastomosed, measuring generally more than 1 mm in diameter. The arterial distribution originates from an arcade parallel to the nerve from which arise some extremely fine ladderlike vessels. Finally, this pedicle vascularizes the whole brachial part of the nerve (roughly 23,6 cm).

[The Chinese anti-brachial radial flap: anatomic study].

Concerning twenty forearms of fresh cadavers, the authors describe a cutaneous flap irrigated by a radial artery whose ligature is without consequence on the vascularization of the hand. They specify the margins of the flap, its vascularization and innervation. They also specify the anatomical sacrifices necessary to its dissection and the different bony and tendinous elements which it is possible to include to form a complex osteo-musculo-cutaneous flap.

[Roos' axillary approach to the brachial plexus].

An anatomic study of five cadavers, that were dissected on a level with the two shoulders, was made to explore the possibilities of the Roos ' axillary surgical approach. This surgical approach permits almost complete resection of the first rib and dissection of plexus brachialis's inferior roots. (T1, C8 and C7).

Therapeutic aspects of limb lesions due to hunting weapons: a study of twenty cases.

The authors report 20 cases of limb lesions provoked during hunting accidents by traditional weapons (gauges 12 and 20) or by 5.5 mm bullets of 22 L.R. Lesions caused by shotguns (gauge 12, 20) are much more serious when done point-blank or at very short shooting distance. The treatment of skin and bone lesions shall be realized in several steps: emergency removal of the foreign bodies with cutting out of non viable bone fragments. Immobilization and depending on the development, two therapeutic solutions may be chosen: either skin graft then bone graft or Papineau's method. Lesions caused by 5.5 mm bullets are much less serious because losses of substances are less important.

[Osteosynthesis for fracture of the trapezium. One case (author's transl)].

Fractures of the trapezium are rare but their clinical and radiological features are perfectly known. By contrast, their treatment (orthopaedic or surgical) is less clearly defined. The authors wished to make their contribution to the study of this fracture by presentation of a case of fracture-dislocation of the 1st radiation of the hand. Such lesions are generally unstable and require osteosynthesis. In the present case, the latter was facilitated by the use of mini-screws which made it possible to begin physiotherapy almost immediately and to obtain an excellent result.

[Rupture or disinsertion of the inferior tendon of the biceps brachii. Repair via a Boyd and Anderson approach (author's transl)].

Low rupture of the tendon of the long head of the biceps is a rare lesion and surgical approach is hampered by constant anatomical features, in particular the radial nerve which is the most serious potential problem. The double anterior and posterior approach of Boyd and Anderson makes it possible to completely restore the action of the tendon of the long head of the biceps without running the risk of any nerve lesion. It is for this reason that the authors prefer it to other approaches, felt to be imperfect either in terms of their final result or their technical feasibility.

[Toxic megacolon and its surgical treatment. Results in 21 cases (author's transl)].

The authors present their experience of the toxic colonic dilatation after 21 cases. The frequency of this serious syndrom seems to be in augmentation, and is not any more exclusive of the ulcerative colitis. Actually, pathogeny is not yet known. Diagnosis is established by the standard abdominal X Ray. A continuous clinical, radiological and biological survey is essential, in order to avoid perforation, that is the main complication of the toxic dilatation. The surgical treatment becomes imperative as soon as the failure of the limited medical management is recognized. In this series, total colectomy with or without ileo-rectal anastomosis has been considered as the best surgical operation. Ileo-rectal anastomosis would be possibly protected by a temporary ilestomy.

[Crohn's disease. The indications for surgical treatment (author's transl)].

On the basis of immediate and long term results obtained by surgery in a consecutive series of 46 patients with Crohn's disease (1968-1978), the authors establish the salutory efficiency of a therapy which is often too delayed. Some of the patients could have advantage of an earlier resection complications occur. Thus the rate of mortality and morbidity could be lowered. This principle may become of great interest in opposite to the feebleness of the actual medical treatment.

Fatal peritonitis.

We analysed 117 cases of peritonitis in children. They were divided into two groups: -- Newborn and very young infants with peritonitis. -- Older children with peritonitis. The different causes in every group were studied, and it was attempted to find the particular criteria for each cause. The mortality in newborn and young infants is high, but resuscitation methods and appropriate surgical therapy are giving encouraging results. In older children the mortality from peritonitis is low and more or less resembles that in adults.

[Rupture of the extensor apparatus of the knee; 45 cases].

The authors describe 45 cases of rupture of the extensor apparatus of the knee (excluding fracture of the patella). Some new ideas are put forward concerning the frequent cause by road accidents and the contribution of certain predisposing features which cause degenerative changes of the tendon. Operation was performed in all cases. The extensor apparatus was reconstructed by suture, cerclage or reinsertion, together with the use of procedures described by Mac Laughlin and Scuderi. The results are presented in detail. 90% are very good or good but complications during operation or in the post operative phase were frequent and occurred in 27% of the cases.

[Anatomical and embryological study of a case of bladder exstrophy with duplication of the urethra].

An exceptional case of a double urethra associated with an extrophied bladder in a neonate boy is reported. This particular malformation allows an ideal procedure with the recovery of a good continence. After the anatomic study of the anomalies, the authors try to find an embryologic explanation. The usual theories of the development in the cloacal region are discussed. Only the embryologenesis as described by SOLERE can explain this malformation.

Bladder Exstrophy↗

[Tracheo-bronchial malformation associated with an aberrant left pulmonary artery].

A new case of pulmonary artery sling in a three months old boy is reported. After the sudden death of the patient, a precise anatomic study of the malformation was realized. With a complete cartilaginous ring usual in this malformation, there is a right upper lobe bronchus arising from the trachea above the anomalous vessel. The other anomalies of the tracheobronchial tree are discussed.

Bronchi↗

[Epidermoid cysts of the mandible (author's transl)].

The authors present three cases of epidermoid cyst of the mandible, one of which had undergone malignant change. These cysts are rare. They are most often situated at the angle of the jaw and present as a deep swelling of the cheek. The only treatment is excision, the authors advising the external approach which gave them the best results.

[CT diagnosis of acute gastric volvulus].

Acute gastric volvulus is an infrequent entity. Management is surgical. Diagnosis frequently is delayed because of the non-specific nature of presenting symptoms. The authors report a case of acute gastric volvulus diagnosed by computed tomography in a 92 year old woman that was confirmed at surgery. The pathophysiology, classification and different presentations of this entity are briefly reviewed. CT findings suggesting gastric volvulus are reviewed as well.
